This is an IMPROPERLY wired disconnect switch. Suspicions are that it was done on purpose so that management could not recognize the machine was still powered on while maintenance/servicing was taking place. But if ALL workers did not know about this and just turned the switch off and did NOT verify zero energy this could have led to a very serious accident. |
