This is an excellent presentation showing what is believed to have happened leading up to and after the Amuay Refinery Vapor Cloud Explosion on 8/25/12. Excellent modeling, but based on assumption, showing how a vapor cloud progresses. To date, the loss of life now exceeds 50 with many more severly burned. The off-site devistation was massive! UPDATE on 10/4/12 – A member of the task force remarked, “It seems very clear that proper actions were not taken upon the leak. In implementing safe operations, which must be a priority in Pdvsa, continued operation of any facility working in unsafe conditions is absolutely unacceptable.
