Take note: Six (6) of the top ten are from HAZCOM, Machine Guarding, and Respirator standards. And sadly, the HAZCOM issues are NOT related to the new GHS changes, but the same old issues: No written program and not having SDSs! Machine Guarding issues were related to improper or lack of guarding. And respirator issues are not new: no medical evals and no written program.
