Safety and Health Program Assessment Worksheet (HI-OSHA)

Another great OSHA State Plan resource; this time from Hawaii.  This is an excellent self-evaluation worksheet that covers pretty much all facets of a traditional occupational safety and health program/process.  If a business uses this tool to its full advantage, this assessment can paint a pretty accurate picture of the state of their S&H program efforts…....

OSHA issues citations for NH3 Refrigeration Fatality ($173K; seafood processing business)

UPDATE:  OSHA Settlement (Final) This week OSHA posted their citations (20 serious and 1 “other-than”) for a fatality involving Anhydrous Ammonia (NH3) at a seafood processing business in Boston, MA.  None of the citations were for 1910.119 (PSM) indicating that the process contained less than 10,000 pounds of NH3.  However, OSHA did issue two (2)…...

OSHA Citation Analysis for PSM, LOTO, PRCS (2015)

My postings back in January were so popular I decided to put the data into EXCEL spreadsheets so that you all can play with the data as you see fit.  I only did PSM (1910.119), Permit-Required Confined Spaces (1910.146), and Control of Hazardous Energy (LOTO 1910.147).  Each workbook contains three (3) worksheets: 1) By #…...

OSHA’s investigation of lathe operator’s fatal injuries finds machine guards were bypassed at machining facility

A few weeks back I posted a OSHRC decision where an OSHA citation for not treating the CNC doors as guards was upheld.  Since then I have received numerous e-mails and a few phone calls telling me I am crazy, as if it was my decision.  Then last week the Video of the Week was…...

Fire Extinguisher used as an “air receiver” catastrophically fails and claims three (3) legs!!!

A fire extinguisher is often times looked upon as a safety device and therefore many overlook these devices potential hazards.  It is true, they can save lives, but they can also take lives and limbs as was the case in this accident.  Back in February, as I reported in the Incident Alerts, two workers were…...

Machine guarding CNC Mills and Lathes (OSHRC Decision)

This case is significant in a number of ways:  1) manufacturer of the equipment put warning signs on their machines pointing out the hazard(s), 2) manufacturer considered the “doors” on their CNC mills and lathes to be “guards”, 3) manufacturer installed interlocks on the “doors” (i.e. guards), 4) company bought the machines “used” and many of the interlocks were not operational at…...

Post-Accident Drug Testing and OSHA’s New Reporting Rule

This past week I worked with a client who’s safety contact is the HR manager and we had several discussions about OSHA’s revised recordkeeping requirements and all the recent reporting changes.  One thing I noticed is that ALL MANUFACTURING facilities are considered “high risk” and are on the list of NAICS’s number that must report.  So the…...

OSHRC can not agree on what a “machine” is as it applies to LOTO (OSHRC)

Holy cow batman, have we really come to this in the safety profession?!?!?!  An apprentice is crushed to death while standing inside (or technically under) a “machine” looking at some fans he was going to be working on, while the facility technician was performing the LOTO on “the machine”.  To get this “machine” to the…...

OSHA Penalty Adjustments to Take Effect August 1, 2016

In November 2015, Congress enacted legislation requiring federal agencies to adjust their civil penalties to account for inflation. The Department of Labor is adjusting penalties for its agencies, including the Occupational Safety and Health Administration (OSHA).  OSHA’s maximum penalties, which were last adjusted in 1990, will increase by 78%. Going forward, the agency will continue…...
