Two contractor fatalities to start 2015. Seven contractor deaths occurred in 2014. On January 11, 2015, a contract shaft miner was killed while riding on top of a skip in a ventilation shaft. The victim was caught between a support beam in the shaft and a handrail on the skip.  On January 8, 2015, a sales representative was killed helping to change a screen liner on a screen deck. ELECTRICAL SAFETY ALERT (MSHA)
In the last two months there were three electrical incidents at underground coal mines that resulted in miners being transported to the hospital. The first incident occurred when an electrician was shocked and burned while attempting to repair a damaged continuous mining machine cable, with the circuit breaker closed and the cable coupler connected to the receptacle. The second miner was shocked while...

How do you comply with debriefing contractors after they exit your PRCS?
This is a slippery slope for us and OSHA to walk.  It is clearly a requirement when contractors enter our Permit Required Confined Spaces (PRCS), but how do we demonstrate that we are indeed “debriefing” these contractors as required by 1910.146(c)(8)(v)?  What do we have on hand that can show all entries involving contractors involved in a “debrief” at the conclusion of entry operations?  Several...

2015 Video of the Week #7 (Loading and unloading 25' tank)
This is not necessarily a “shocker” video, but it is an interesting way of loading and unloading vertical storage tanks.  Notice the tank has been modified in order to be loaded and unloaded in this manner.  I am guessing there is some type of locking device at the back of the truck that grabs onto the lattice structure on the back of the tank and then the winch pulls the tank...

2015 Photo of the Week #7 (Scissor lifts and Ladder)
WOW!  The use of the scissor lifts and the ladder of course standout but look at how close the one manlift is at going down the stairs.  Sort of adds a bit of excitement…. NOT!

Does it matter what type and size fire extinguisher my Fire Watch is equipped with?
In a previous article I posted the Fire Watch requirements found in OSHA, NFPA, and some state Fire Codes; however, the one critical path that is high on our list of findings during audits is the method of extinguishing the fire watch is provided.  We have, on many occasions, found fire extinguishers that had not been inspected in months, some were years past due for their annual maintenance/inspection,...

When is a Fire Watch actually required?
Fire watches are usually the youngest and lowest paid crew member on the job.  But why is this?  After all they are the difference between a job done safely and the potential loss of an entire business.  Most people think hotwork fires are rare – and they would be WRONG!  Anyone who subscribes to my incident alerts can tell you that each week I report two or three significant...

How long do you keep your hotwork permits?
In just about all our audits, we get this question.  We all know that OSHA requires us to maintain our PRCS Entry Permits for one year to be used in our annual program review, but they do not mention HW permit retention.  We always advise maintaining HW permits on a rolling 12-month schedule; however, at some very large facilities, this can take a lot of filing cabinets!  So I am asked...
