A worker was operating a Computer Numeric Control machine, commonly called CNC. As you will see (NOT graphic) he OPENS the door to the machine, which was an interlocked door, and leans into the machine – something he did hundreds of times a day. The interlock fails for unknown reasons, the machine cycles, and his head is crushed by the machine (NOT graphic). He was unable to reach the e-stop and remained in the machine for 30 seconds. The machines alarm goes off because it “was jammed” and this brought a co-worker over to assist and he found the worker in the machine and released him. The worker was pronounced dead at the scene.
