SCISSOR LIFT FATALITY
Worker Killed, Another Seriously Hurt In Scissor Lift Accident
(a contractor was killed and another was seriously injured in a
scissor lift accident at an industrial facility at about 10 a.m. –
one of the workers was pinned beneath machinery – the worker who
survived was airlifted to a hospital – maintenance work was being
done to some machinery inside the business at the time of the
accident – the victims are subcontractors who were working on a
crane – the workers were up about 40 feet in the air when the
scissor lift collapsed for an unknown reason)
FATALITY – either a FATAL FALL or
HYDROGEN SULPHIDE POISONING
Worker killed in accident at Valero{apos}s Norco refinery (a contract
worker, 30, at a Refinery died late after being hospitalized
following an incident at the refinery – two contractors were working
on a new construction project – one was a safety man, the other was
grinding metal as part of new construction – the surviving worker,
28, reported to state police that he thought he smelled hydrogen
sulfide and immediately began an evacuation of the area – he passed
out and said when he regained consciousness he was half way down a
ladder guard – his coworker was on the ground level – the refinery{apos}s
emergency response team conducted a rescue and the two workers were
taken to the hospital – both workers were wearing personal monitors
that did indicate hydrogen sulfide exposure – the fatality had a
head trauma, so they{apos}re not sure if he died due to exposure or due
to the fall – they were working in the refinery{apos}s millisecond
catalytic cracking unit, which was shut down at the time of the
incident)
Logging
Accident Kills Big Island Man (a deadly logging accident at
about 1:00 p.m. found the victim, 72, at the top of the mountain and
he was pronounced dead at the scene – he was cutting a tree when it
fell into another tree and a limb from that tree fell onto him)
FLYING MACHINERY PART FATALITY
Mansfield man dies in Americarb industrial accident (worker, 55,
was pronounced dead at a Hospital following the accident – he
suffered an abdominal injury – crews found him on the floor with a
person believed to be a fellow employee applying direct pressure to
a wound – he was not breathing and the squad performed CPR before
taking him to the hospital – it appears he was struck in the abdomen
by a chunk of metal that broke off a piece of equipment – he
suffered internal bleeding and the preliminary cause of death has
been listed as an exsanguinating hemorrhage – autopsy is being
performed to determine an exact cause)
FORKLIFT PLATFORM FALL
Worker Injured
in fall from Height Accident (a man suffered serious personal
injury, when he fell from a makeshift platform, which was balanced
on the forks of a forklift truck – he was attempting to change a
marker light at the back of a high sided wagon – unfortunately the
step ladder he had been provided with was not tall enough for the
task, so he decided to place a metal cage onto the forks of the
forklift and asked a colleague to raise the forks, while he stood on
the improvised platform – as he was being raised to the required
height, the metal cage slipped and came off the forks, sending him
plummeting to the ground – he suffered a fractured elbow and badly
bruised his shoulder in the incident and still suffers with ongoing
back problems)
FALL into retention pond
Cobb Water Plant Worker Falls into Retention Pond (an employee
of a waste water treatment plant was recovering in the hospital
after falling into a pond at work – the worker spent two hours in
the concrete trap before he was rescued – rescue crews worked to
rescue the plant employee – after two hours, the man was freed – he
suffered a broken arm and leg as a result of the accident)

Man{apos}s legs
pinned in industrial accident in Niles (an industrial accident
sent one man to the hospital with serious injuries – the worker had
both legs pinned in a hopper at the plant – it took half an hour to
free the 38-year-old victim – no word on his condition)
MINE FATALITIES
Mine accidents leave 3 dead, 1 trapped (two mining accidents
have left three workers dead and one trapped – the most recent
accident took place in a mine as the result of an accumulation of
gases – the two workers entered the tunnel before an explosion
fatally wounded one and trapped the other – though the injured man
was taken to a health center, he died minutes after arrival – a
second accident occurred approximately 24 hours earlier killed two
men – authorities are unsure how events occurred, as the mine was
supposed to be closed two weeks earlier for operating illegally –
rescue workers managed to remove the bodies of the two workers)
SOUR GAS EXPLOSION
Gas well explosion injures 12 in Alberta (energy officials were
investigating a natural explosion that injured 12 workers – the
blast occurred midday – the injured sustained trauma and burn
injuries, but none were life-threatening – the explosion involved
so-called sour gas – subcontractors were preparing to start a
process of fracturing rocks to access the gas deposits when the
blast happened)

Gillette worker burned after oil tanker catches fire, destroys
building (an oil worker was hospitalized with burns after a
tanker caught fire – the worker taken to a hospital but it{apos}s unknown
how serious his injuries are – the tanker was being used to treat
oil – investigators don{apos}t yet know what sparked the tanker to catch
fire – fire may have caused an estimated $25,000 in damage)
BULLDOZER FATALITY
Edson-area worker crushed by bulldozer (woman, 32, dies during
snow-clearing job – she was working as part of a snow-clearing crew
at an energy site – she worked as a bulldozer operator – she died
after she stopped and exited the machine she was operating – she
stepped out of the cab and onto the machine’s tracks – the bulldozer
moved forward, dragging the worker underneath)
REFINERY FIRE
Sunoco Philadelphia Refinery Contains Fire in Vacuum Tower
(refinery emergency response personnel have contained a fire at in a
vacuum tower – the unit has been isolated – no injuries)
Fire Crews Battle Molten Glass Leak (a leak at a glass plant
took several fire departments a number of hours to control – a
molten glass leak was reported at a number two tank about a quarter
after midnight – it was the second leak at the facility in the past
four days, though this leak was in a different area of the plant –
no injuries were reported)
LANDFILL FIRE
Blast, fire hit pump site at Bath landfill (firefighters
responded to an explosion and small fire at a leachate pump station
at the rear of a municipal landfill – landfill uses a gas management
system to burn landfill emissions as a way to combat nuisance odors
– the structure that was damaged houses instruments used to monitor
leaching – the situation seemed to involve some type of trapped gas
explosion, whereby gas within the building ignited)

Fire at tire recycling company (firefighters were called to a
fire at a tire recycling business just before midnight – some rubber
crumb became overheated in a grinding machine – it then spread to a
conveyor belt – no one was injured in the incident – a preliminary
estimate indicates the fire caused about $40,000 to $50,000 damage
because the conveyor belt will be shut down while repairs are done)

Fire Officials Investigating Chemical Plant Explosion
(firefighters arrived shortly after 2 p.m. to find a trailer, in the
loading docks, on fire – officials are investigating what caused an
explosion outside the plant – the factory, known to repackage
agricultural chemicals arriving by rail cars was closed at the time)

Three killed in boiler explosion (three persons were killed and
six received serious burn injuries following a huge explosion in a
boiler at a plywood factory – the impact of the explosion at the
factory was such that a dismembered body was found several metres
away from the accident site – all the three persons were working at
the factory at around 1.30 am when the incident took place – factory
specialises in pressing, taping and finishing of plywood)

occured inside a business that applies spray-on truck bed liners – a
vehicle was on fire inside the business – the fire was being
contained by a sprinkler system and was quickly extinguished –
chemicals used in the spraying process are oil-based and corrosive,
and damaged firefighters{apos} gear – cause of the explosion is under
investigation)

Firefighters extinguish blaze at Ford Transmission Plant (Fire
Department extinguished a fire inside the transmission plant – no
one was hurt in the blaze, which started in a machine used to clean
parts – the fire broke out at about 6:15 p.m. when the plant was
closed – it activated the fire alarm and overhead sprinkler system,
which contained the fire to the machine area – the machine was
located deep inside the building so firefighters drove the fire
engine inside the plant to the area of the machine and put the fire
out within 15 minutes)

39, was seriously injured when a winch he was operating
malfunctioned and struck him in the head – he was taken to a Medical
Center – hospital listed him in critical condition)

Worker hurt in fall from scaffolding in Greenwich (a worker was
taken to a Hospital with what is believed to be non-life threatening
injuries after falling from scaffolding at a work site)
UPDATE SCAFFOLDING POLE HITTING WORKER
Worker suffered brain injury as scaffold fell at Radisson Blu hotel
in Bristol (two building firms involved with the construction of
a hotel face huge fines after an accident left a worker, 53, with
severe brain injuries – he now needs 24-hour care for the rest of
his life after a scaffolding pole being used as a makeshift plumb
line fell 15 storeys down a lift shaft and hit him in the head – the
7lb (3.7kg) pole was being used to measure the shaft when a line
holding it, made of either plastic or piano wire, broke and it fell
to the shaft pit – he was wearing a hard hat at the time, suffered a
{amp}quot;catastrophic brain injury{amp}quot; that required part of his skull to be
removed)

worker was seriously injured after a 25-foot platform fell on him at
a construction site at about 3:15 p.m. – the worker was taken to a
Hospital in serious-to-critical condition)
CONCRETE POUR COLLAPSE
Seven injured as roof collapses (seven workers were injured when
the concrete roof of an under-construction building caved in – the
accident happened at around 9pm when the contractors where pouring
concrete for the third floor of one of the multi-storey buildings
under construction – four workers were admitted to the hospital as
others got discharged after getting treatment)
