We see it happen every once in a while, a worker has their face in the opening of the space. They are exposed to the hazardous atmosphere within the space as the only air flow out of the space (which is at higher pressure) is right past their breathing zone. They are exposed and unfortunately, sometimes these exposures are fatal. Take this 2018 OSHA Case, at 5:00 a.m. on September 27, 2018, the employee was unloading and washing a truck tanker with water that had contained poultry blood. The employee was found unresponsive on top of the tanker with his head over the tanker opening. The employee was killed from exposure to ammonia and hydrogen sulfide vapors from the tanker truck.
