OSHA updates their PSM-VPP questionnaire

Over the last several years, OSHA has asked, in the annual Supplement B questionnaire, specific questions designed to help share information about the site’s safety and health management systems (SHMS), but mainly about the critical areas of PSM programs and established procedures used to maintain, collect, and analyze PSM systems/process metrics.

In 2016, OSHA developed a Fact Sheet, “The Use of Metrics in Process Safety Management (PSM) Facilities.”  The Fact Sheet includes information provided by VPP-PSM participants from the 2014 and 2015 Supplement B responses. Since 2019, the PSM Supplement B results have been presented at Regional and National VPPPA events.  The feedback we received from the attendees during the interactive sessions has been very valuable. We continue to accept feedback from the participants and our field staff, and have modified several of the questions accordingly.  OSHA hopes to have similar discussions in the future to improve the collection of the best information possible about PSM programs in relation to the Safety and Health Management System (SHMS).

Within this workbook are the VPP PSM Supplement B questions for Calendar Year 2022.  All answers, unless otherwise specified, should reflect the period from January 1, 2022 to December 31, 2022.  Each site will receive the Supplement B questions in a spreadsheet format.  The spreadsheet will have six sections this year.  These sections and the questions therein differ from those in the previous year’s Supplements, so it is very important you submit the correct form for 2022.  Please respond to each question with separate responses.  The responses for questions in all six sections will be predominantly, Yes/No/NA, a numerical percentage, or a number.  In Section VI there are a two questions where a brief narrative response is required.  Everyone may not collect all of the metrics listed in each section.  This 2022 Supplement B builds on the responses we obtained from the previous years’ Supplements.

The information provided will be used to identify points for discussion about leading indicators that relate to how the sites in your industry evaluate confidence in the sites’ PSM and in associated safety and health programs.  If asked about a metric you do not collect, please respond NA.  Responding with a zero (0) in a field where no information is collected relating to the information will skew the data analysis.  Also, if a field is left blank, it will not be counted during the analysis.  And remember, there are no wrong answers.    Please return the spreadsheet to your VPP Manager with your annual self-evaluation responses on or before March 15, 2023.   

 

EACH SHEET HAS A SECTION LABELED “SITE IDENTIFICATION”.  PLEASE MAKE SURE THAT YOUR CORRECT SITE INFORMATION IS ENTERED IN EACH TAB.  EACH TAB HAS A COMMENT SECTION.  PLEASE ADD ANY NARRATIVE INTO THE COMMENTS FIELD.  THE GREEN ROW IS FOR THE USE OF OSHA REPRESENTATIVES ONLY, AND WILL BE USED BY OSHA DURING ON-SITE EVALUATIONS.

IF YOU ARE A RESIDENT CONTRACTOR, THE CORRECT SITE NAME INFORMATION IS YOUR SPECIFIC COMPANY NAME, NOT THE NAME FOR YOUR HOST SITE.  THE COMPANY NAME FIELD SHOULD INCLUDE YOUR COMPANY NAME @ THE NAME OF THE HOST SITE. THE SITE LOCATION WOULD BE THE HOST SITE’S LOCATION. 

FOR THIS YEAR (CY 2022)  RESIDENT CONTRACTOR PARTICIPANTS ARE REQUIRED TO RESPOND TO     QUESTIONS ONLY IN TABS IV AND VI UNLESS THERE ARE APPLICABLE QUESTIONS TO THEIR OPERATIONS IN OTHER TABS.

RESIDENT CONTRACTORS ARE GENERALLY CONSIDERED TO BE SITE-BASED PARTICIPANTS.  IF YOU ARE A RESIDENT CONTRACTOR, PLEASE ALSO RESPOND YES IN THE SITE-BASED FIELD, AS WELL AS IN THE RESIDENT CONTRACTOR FIELD.
