19100146 D02 – Identify and evaluate the hazards

of permit spaces before employees enter them

19100134 D01 III – The employer shall identify and

evaluate the respiratory hazard(s) in the workplace;

this evaluation shall include a reasonable estimate

of employee exposures to respiratory hazard(s) and

an identification of the contaminant’s chemical state

and physical form. Where the employer cannot

identify or reasonably estimate the employee

exposure, the employer shall consider the atmosphere

to be IDLH.

procedures, and practices necessary for safe permit

space entry operations, including, but not limited to,

the following:

for compliance with paragraphs (h)(3) and (i)(5) of this

section;

needed to comply with paragraph (d)(9) of this section,

except to the extent that the equipment is provided by

the permit space into which entry is authorized for the

duration of entry operations

employer has reason to believe that the measures

taken under the permit space program may not

protect employees and revise the program to correct

deficiencies found to exist before subsequent

entries are authorized

available and that the means for summoning them

service of the hazards they may confront when

called on to perform rescue at the site;

service selected with access to all permit spaces

from which rescue may be necessary so that the

rescue service can develop appropriate rescue

plans and practice rescue operations

documented and utilized for the control of potentially
