We say this for so many of these maintenance tasks… LINE BREAK PERMIT brings so much safety to maintenance work that it should extend well beyond PSM/RMP-covered processes. The Safe Work Permitting process, when developed, implemented, and managed PROPERLY, will go a long way to reduce the chances of this type of accident.
At approximately 8:00 a.m. on February 9, 2022, a 61-year-old employee was on a step ladder replacing a boiler circulation pump. The water supply to the pump was not effectively isolated, and the employee sustained burns over multiple body parts below the neck and above the waistline, including the front and back of the upper torso and right extremities. The water temperature ranged from 190 to 200 degrees Fahrenheit. The employee was hospitalized to treat these injuries.
