The two deceased workers, identified as Mr K (43) and Mr A (50s), were subcontractors conducting scheduled maintenance on a hydrogen pipeline. The task involved inserting a blocking plate into a pipeline to isolate flow, a process requiring the loosening of flange bolts. The explosion is believed to have occurred during this step, when hydrogen that remained in the pipeline escaped and ignited.

10/23/25 UPDATE: RIP Two (2) of the four workers have passed. Criminal charges may be brought against the company executives.

Four (4) contractors have been seriously injured after a hydrogen pipe exploded and caught fire at an oil refinery. The incident happened at the hydrogen production facility, which produces grey H2 from steam methane reforming. The methane-cracking facility had been undergoing scheduled maintenance work, and the facility believes that the fire started when a pipeline that was thought to be empty but actually still contained hydrogen was opened by contractors

Four contractors were said to be seriously injured with second-degree burns and were being treated for burns at a specialist hospital unit, while a fifth sub-contractor suffered minor injuries.
