Another worker dies from N2 asphxiation while wearing an air-line respirator

At 11:43 a.m. on February 24, 2025, Employee #1 was spray painting aircraft parts while wearing a supplied air respirator. Nitrogen backflowed into the breathing air system, displacing oxygen and causing the employee to be asphyxiated. Employee #1 was fatally injured.

29 CFR 1910.134(i)(1)(ii)(A): Breathing air did not contain 19.5-23.5% oxygen (volume/volume): 
On or about February 24, 2025, in Plant III, employee(s) were exposed to asphyxiation and death in that the compressed air system did not meet requirements for Grade D breathing air in the following instance:
1) In Paint Booth 5 the supplied air hood had oxygen levels recorded at 5.3% concentration.
2) In Paint Booth 7 the supplied air hood had oxygen levels recorded at 7.8% concentration
3) In the Carousel/Prime Booth, the supplied air hood had oxygen levels recorded at 5.3% concentration.
29 CFR 1910.134(i)(8):An asphyxiating substance was introduced into breathing air lines: 
On or about February 24, 2025, at the Paint Booth #5 in Plant III, employee(s) were exposed to respiratory health hazards in that the piping configuration and the open compressed air ball valve of the Autoclave #10, connecting the nitrogen and compressed air systems, allowed the introduction of nitrogen gas into the breathing airlines used at the Paint Booth #5, asphyxiating an employee and also exposing employees at Paint Booth #7 and the Carousel/Prime Booth.
29 CFR 1910.134(i)(5)(iv):The tag containing the most recent change to the sorbent beds was not maintained at the compressor: 
Compressor used for breathing airline system did not have a tag showing most recent change date of the sorbent beds and the signature of the person authorized by the employer to perform the change at the following instances:
1) Sullair LS-20S located at Plant III
2) Sullair DSP-75AT6N-M125 (AC-1) Maintenance No. 303080 located at Building 46
3) Sullair DSP-75AT6N-M125 (AC-2) Maintenance No. 303081 located at Building 46
4) Sullair V130 DS Maintenance No. 10048198 located at Plant IV
5) KAISHAN Krof 200 Maintenance No. 46898 located at Plant IV
6) Sullair Maintenance No. 300420 located at Plant I
