FOOT INJURY FROM OUTRIGGER
On February 28, 2012, a contract rigger was injured when a carry deck crane outrigger was lowered onto his right foot. The contractor was examined at a local hospital where x-rays revealed two fractures to the foot. The incident is under investigation. Click Here for the pdf
Does your facility “Car Seal” or “Lock Open” Valves? Do NOT use your LOTO Locks!
It is becoming more common to find facilities using or at least “experimenting” with some “car seal” program or, as some call it, the “Lock Open – Lock Closed” program. These are the programs where a valve has been identified as a safety critical valve and needs to either stay closed or open until certain
