This document, believed to be a sales solicitation or advertisement, states that there is a fee of $295.00. The document is crafted in a manner that has led some to conclude that it is an official notice from a government agency and that the recipient is required to pay the fee. The document is not in any way a communication from the Industrial Commission of Arizona, the Arizona State Labor Department or the Arizona Division of Occupational Safety and Health (both Divisions of the Industrial Commission of Arizona).
