At 12:19 p.m. on September 18, 2024, Employee #1, 42, was cleaning the paper machine rollers with an air hose. While performing the task, the employee was caught between the top roller and the mat. The employee was crushed by the rollers, resulting in fatal injuries.

29 CFR 1910.147(c)(4)(ii): Procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance including, but not limited to, 29 CFR 1910.147(c)(4)(ii)(A), (c)(4)(ii)(B), (c)(4)(ii)(C) and (c)(4)(ii)(D): a) Paper Machine Room – On or about September 18, 2024 and at times prior thereto, the employer did not develop specific procedures that outline the techniques to be utilized for controlling hazardous energy on machines. Employees perform a lockout on a number of machines, including but not limited to the paper machine, to perform maintenance and cleaning.
29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: a) Paper Machine Room – On or about September 18, 2024 and at times prior thereto, the employer did not conduct periodic inspection of the lock out/tag out procedures. The control of hazardous energy program was last revised in 2014, and the program does not include specific procedures for lock out.
