The title is NOT a type-o! Yes, the EPA Office of Inspector General will audit the U.S. Chemical Safety and Hazard Investigation Board’s (CSB’s) investigation process. The EPA OIG’s objective is to determine if CSB has an effective system for managing its investigative process. The OIG plans to conduct its work at CSB headquarters and Denver offices. The project will be conducted using applicable Generally Accepted Government Auditing Standards. The anticipated benefit of this project is to help CSB identify areas for improving its investigation process.
Information Requested for the CSB Investigation Process Audit
Please provide the following information in electronic format by October 25, 2012:
- The number of CSB investigation reports, case studies, and videos planned for completion during FY 2010, FY 2011 and FY 2012, and those actually completed during FY 2010, FY 2011 and FY 2012. Please provide the names of the investigations, studies, and videos to support the numbers.
- Written criteria and/or Board Order used by CSB’s Board and investigators to decide whether to adopt a final product as a case study, investigative report, safety bulletin, or video.
- CSB’s Board Order or formal system for measuring actual performance and promoting accountability in the investigation process.
- The Board-approved current or final work plans for the following six investigations from our preliminary research (and identify where this information is kept at CSB):
