Human Factors 101

I am not sure why I am so fascinated with the Human Factors aspect of process safety, but I blame my pals Bob and Steve at Unwin Company for beating it into my head during the first PHA I did with them way back in 1999.  They ripped me apart with how our procedures were written and taught me so much (still do today) about human factors in process safety (20 years and still learning!).  But this picture, shared with me this morning, says a lot about the human species…

I hear the supervisor saying, “Bryan, please fill the salt and pepper shakers before you leave,” So I do just that – my way, using my “common sense,” the best way I know.  After all, I was never trained in how my boss wanted the salt and pepper shakers filled, and I had never done the job before, so what many will assume is a simple task will never be able to understand how this happens…
