When preparing a Permit-Required Confined Space (PRCS) for entry, either by reclassification or by permit, it is incumbent upon the Entry Supervisor to ensure that ALL forms of hazardous energy has been identified and properly isolated. In this event, the vessel had a level indicator that used a radiation source of energy that was not identified, leading the entrant(s) to receive a radiation dose. Speaking from experience, these devices have shutters that can be closed and locked closed as part of the Energy Isolation Plan (LOTO) for the PRCS.

Aspects:
- Some level detectors on tanks use radioactive radiation, which can be very dangerous when exposed to it.
- See the international sign of radiation hazard. Personnel and contractors must be informed and aware of the hazard.
- Each site using radioactive radiation must have someone specialized on the topic, managing the radiation source & hazard.
- The tank entry permit asks the question of whether a radioactive source must be shut before tank entry. This was not identified or mentioned on the permit in this case.
- Assure vessel entry is physically blocked when a high-energy radiation source is active.
