Leading Indicator Safety Measurements

Below are examples of “measurable” and “auditable” safety activities that have proven to be effective when properly implemented, managed and

measured.

A) Job Safety Analysis –   Each operation is required to perform _____(#) JSA’s a ________ (shift, day, week, month, quarter, etc.).  These JSAs are required to be signed off by the Department Manager and filed for retention.  Any corrective actions identified on the JSA are to be tracked to closure on the EHS Action List.

B) Behavior Observations- The Department is required to perform at least _______ (#) observations a _______ (shift, day, week, month, quarter, etc.).

C) Incident reporting- Each Department must report at least __________(#) near misses per __________ (week, month, quarter, etc.).

D) Property Damage reporting – Each Department must report at least __________(#) property damage per __________ (week, month, quarter, etc.).

E) Safety/Environmental Regulatory Training- Each employee in the Department must have successfully completed ____________% of the required safety/environmental training within the quarter.  This includes ALL regulatory training, training on SOPs, and training required by Management of Change compilations.

F) Safety Equipment Inspections- Each operation must successfully inspect _________% required safety equipment on the following schedule: eyewash/showers-weekly, ladders-monthly, harnesses-monthly, exit lights-monthly, emergency response equipment-monthly.  This will be verified during the EHS Field Audits performed by the EHS Department.

G) Permit Auditing- Each Department will perform ________(#) permit audit(s) a _________(shift, day, week, month, quarter, etc.). The Department Manager is responsible for communicating to the Supervisor who issued the permit any areas for improvement.  The permit audited must be identified as having been reviewed for verification purposes with the auditor’s initials and date of the audit on the audited permit.

H) Contractor Field Audits- Each ______________ Department must perform at least ____________ (#) contractor field audit(s) every _______________ (shift, day, week, month, quarter, etc.). The permit audited must be identified as having been reviewed for verification purposes with the auditor’s initials and date of the audit on the permit.

I) Safety Committee- Each Department must have an active safety committee that meets monthly (at least 10 months a year).  This committee must be chaired by an non-exempt employee, have a 60-40 split of non-exempt to exempt personnel, and supported by the Department Manager.  Each Department Manager, or delegate, must attend all meetings.

J) Ergonomics- Each Department must have an active ergonomic team which performs at least ___________(#) ergonomic study(s) on a specific job task each _________(shift, daily, weekly, monthly, quarterly, etc.).  This study is to be reviewed and approved by the Safety Manager and Department Manager.  Corrective actions are to be tracked to closure on the “EHS Action List”.

K) EHS Auditing- Each Department must designate at least __________(#) delegate(s) to participate in the EHS “Walkthroughs”  which is conducted every Wednesday in specified buildings.

L) Corrective Action Closure-  Each Department must completely close _________% corrective actions from HAZOPS, PSSR’s, Incident Investigations, Safety Reviews, Safety Committee meetings, etc. by their assigned due dates.  Exceptions to this are those that require ordering parts and requiring large capital, but documentation must be provided to show that the parts are the only hold-up or that the capital request has been made.

M) Safety Talks- Each Department must perform _________(#) safety talk(s) each _____________(shift, day, week, month, quarter).  Any issues that arose must be assigned to a responsible party with a completion date and tracked to closure using  the EHS Action List.  The Department Supervisor is responsible for this meeting, attendance documentation, and returning info to EHS Department.

N) Internal Housekeeping- Each Department must perform _________(shift, daily, weekly, monthly, quarterly, etc.). housekeeping audits in their areas.  The Department is responsible for designing and implementing these housekeeping work sessions.
