Plant 2- Combustible wood dust (Kst = 20.75) had accumulated in amounts greater than 1/32 inch on the pipes on the ceiling, and on the equipment that employees ground, drilled and cut wood which created a potential deflagration and/or fire hazard.

Facility – Employees weren’t provided with and didn’t wear flame-resistant, non-static generating clothing (FRC) and static dissipating safety shoes in areas of the facility where employees were exposed to potential flash fire hazards when conducting operations that included cleaning equipment and other surfaces of combustible wood dust (Kst = 20.75).
