I am not sure why I am so fascinated with the Human Factors aspect of process safety, but I blame my pals Bob and Steve at Unwin Company for beating it into my head during the first PHA I did with them way back in 1999. They ripped me apart with how our procedures were written and taught me so much (still do today) about human factors in process safety (20 years and still learning!). But...

Found another ppt from years back. This was one from my time with BFGoodrich Speciality Chemicals. The Corporate Safety Manager was one of the most down-to-earth CIHs I have ever worked with. Unfortunately she has now retired, but she was an absolute treasure of information and her super power was making difficult and complex matters related to chemical exposures very simple and rational. ...

As many of you know I have been in the ER field since the late 1980’s and then spent 16 years in petrochemical and semi-conductor industries as a safety engineer/manager before starting my consulting business. As part of...

This should be known by ALL HAZMAT Technicians and for those who are new to Process Safety, it may prove to be helpful in explaining the design criteria for process vessels. Here is are examples from...

This is WATER damage! It is on an NH3 process, but this was found to be external damage from treated water with an incorrect water treatment “recepie”. The condenser was not being maintained properly either and the two failures produced this corrosion and a tad bit of erossion as the water was dripping down from about 15′ above. The system was shutdown and the piping...
