How many threads of a bolt must be showing outside a nut to meet RAGAGEP?
NOTE:  Members should also read Process Safety and our pipe flanges (type of bolts, threads showing, etc.) Some may claim this to be a trick question.  I assure you it is not meant to be, but as I will point out in this article, having too much bolt extending beyond its nut can be an issue, as well as not having the bolt flush with the outer surface of the nut.   Have you ever encountered...

2012 Photo of the Week #6 (PFAS Lanyard)
I took this photo last week at a company, which I will not name, where they were using a chain as a safety lanyard on the harness for the stock picker.  The operator demonstrated it for me and did not mind being photographed wearing it!  I refused to assess the applicant for his license until the company changed the chain for a legal lanyard which took them approximately 3 minutes to find,...

Roof work and proper Fall Protection
Let me be clear… I am not a Fall Protection expert in any stretch of the imagination, so I am all about engineering out the fall hazards whenever possible.  A few weeks ago we received a call from a new client who had just suffered a bad accident where one of their star employees was on the roof working on a piece of equipment.  The roof is perfectly flat and has a raised edge of 27″...

Change(s) that REQUIRE a PHA revalidation AND RMP Update
There is some language in the EPA’s Risk Management Plan program requirements that tends to cause some confusion… The owner or operator of a stationary source shall revise and update the RMP submitted under ยง68.150 as follows: (5) Within six months of a change that requires a revised PHA or hazard review So when are we required to “revise our PHA” other than when we have...

Interesting RMP Statistics - Where does your process rank?
NOTE: this data is a bit dated, but for those interested…An overview of some basic statistics derived from the RMP*Info database is informative and will help place more detailed assessments in context. As of this writing, the RMP*Info database contains information on a total of 14828 facilities containing 20210 chemical processes. Of these processes, 17529 contain at least one toxic chemical...

Another how NOT TO RESPOND video (Ammonia Tanker)
A NH3 Tanker Truck rolled over and lost it’s load.  A GREAT video to see the behavior of a Liquid Spill of NH3.  We can see the liquid pouring our of the tanker and immediately flashing off to a gas.  Auto-refrigeration is observed as the gas hangs low over the pooling liquid.  AMAZING FOOTAGE at 1:53 into the video and another CRITICAL mistake at 2:06 into the video.

Test your risk assessment skills
I am sorry but I could NOT resist posting this from a longtime and very wise safety friend!  THANKS Fred! You are on a Horse, galloping at a constant speed. On your right side is a sharp drop off. On your left side is an Elephant traveling at the same speed as you. Directly in front of you traveling in the same direction, is a galloping Kangaroo and your horse is unable to overtake it. Behind...

Air-Purifying Respirator
What type of respirator do I need?
One of the most common questions I used to get on emergency response scenes is: “How do you determine when we can stop wearing SCBAs and begin wearing Air Purifying Respirators”?  The decision is often a heated debate due to the user’s lack of knowledge.  The scary part is that these are trained HAZMAT Technicians who should know better.  But, after a quick survey...
