As we have discussed here many many times, pressure testing of our primary containment systems for our hazardous materials is an absolute necessity to ensure the integrity of our construction/installation and/or alteration/repair. But these tests come with some serious risks, EVEN HYDROSTATIC TESTING is a very hazardous activity. And as shown in the OSHA investigation when things go wrong, life can be lost. We need DOCUMENTED and APPROVED pressure testing procedures that are reviewed and certified just like our operating procedures and maintenance procedures – in fact, these pressure test procedures should be managed as either part of the operating procedures or the maintenance procedures. Case in point…
At 11:00 a.m. on August 15, 2018, an employee was removing some blinds from pressure vessels that were undergoing some hydrostatic testing. The employee suffered blunt force trauma to his chest and shoulder area when struck by the remaining pressure and was killed.
