As most of you know, I am a big James Reason fan and have been using his Mistakes/Errors/Violations model since the early 1990’s. This SafeFlix tool is a bit different; it looks at 12 elements of an organization (many of these 12 are the elements of a functioning SMS) and assesses where the organization is at in each of the 12 and where there are opportunities to IMPROVE HUMAN FACTORS (or in other words REDUCE Mistakes/Errors/Violations).
