STOP USE NOTICE – Moldex 7000 Series Gas/Vapor Cartridges

RE: 7000 Series Gas/Vapor Cartridges: 7100, 7200, 7300, 7400,(7500, 7600,(7140, 7640, 7101, 7102, 7103, 7111, 7112, 7113, 7171, 7172, 7173, 7601, 7602, 7603
As part of Moldex’s commitment to producing safe, quality and innovative products we constantly monitor our products to ensure the health and safety of our customers. During a recent internal audit we found that a limited number of cartridges used for our 7000 and 9000 series respirators had gas/vapor breakthrough which is below the NIOSH standard. While not all gas or vapor cartridge models experienced breakthrough below the NIOSH standard, in an abundance of caution we have decided to implement a voluntary recall and stop sale of the following Moldex 7000 series gas or vapor cartridges and assembled respirators with gas or vapor cartridges with any manufacturing lot number before 121112000.  Moldex will replace all returned product to you or your customer at no charge.

Model numbers are listed below. We have identified the root cause of this problem and have taken the necessary steps to correct it.

Please immediately do the following:

  1. STOP THE USE OR SALE of any of the following Moldex models: 7100, 7200, 7300, 7400, 7500, 7600, 7140, 7640, 7101, 7102, 7103, 7111, 7112, 7113, 7171, 7172, 7173, 7601, 7602, 76032, 
  2. IMMEDIATELY DO THE FOLLOWING:
    1. Send Moldex the list of your customers who have purchased these products and we will contact them regarding this recall on your behalf, OR
    2. Call and mail customers of these models and tell them to immediately stop the use of these products and return any unused product to you or directly to Moldex.
  3. Attached is a list of affected Moldex models that were purchased by your company. Please quarantine your inventory and return these to Moldex. Attached you will find a return authorization form with instructions on how and where to return the material to Moldex.  Moldex will pay the return freight charges.
  4. Please contact Moldex Customer Service at 800 421-0668 ext.550 if you have questions or to arrange the return of the affected Moldex models.

Please Note: this voluntary recall/stop sale does NOT apply to P100 filter disks, P100 cartridges or assembled respirators with P100 filter disks or N95 or R95 prefilters; models 7940, 7941, 7942, 7943, 7960, 7990, 8910 and 8970.
