Using Reason’s Human Failure model in our causal analyzes

I have written extensively about James Reason’s Human Failure model and how it applies to almost all workplaces.  Today I want to discuss specifics on how to use his model in our causal analysis. 

For those already using some form of a Fault-Tree Analysis, such as TapRoot, you already have this capability built into your software.  But many are not using commercial software to aid in this ability. So this article is for you and your introduction to defining Errors, Mistakes, and Violations associated with incidents.

So remember, we have Errors, Mistakes, and Violations.  The following set of questions will help a team performing a Human Factors analysis of an incident determine what types of failures played a role.  From there, the team can use the HF Worksheet of Solutions based on the type of error(s), mistake(s), and/or violation(s) found.
