LOTO FATALITY Worker killed in explosion at Stimson Lumber (a worker, 60, was killed and two others were injured from an explosion – two other workers suffered injuries that were not life-threatening – one of the workers, 58, is believed to have suffered a broken leg, along with other injuries – the third worker, 40, suffered a shoulder injury and was transported to a Hospital – the two employees are in stable condition – the deceased was a mechanic, who’d worked for the facility for 30 years – a hydraulic accumulator exploded – the hydraulic accumulator is a pressurized cylinder, about 6 feet long and made of heavy metal – the workers were dismantling the cylinder, which they thought was not pressurized, when the end of the cylinder blew off under the high pressure)
ELECTROCUTION Rig site worker in fatal accident ID’d (a man, 53, died in a late night accident on a rig site – he was pronounced dead upon arrival – the nature of the accident was reported as an electrocution)
AMPUTATION with SAW Worker Severs Fingers in Circular Saw Work Accident (a man, 55, suffered serious personal injury when his hand came into contact with a rotating circular blade – he was cutting wooden discs, which are used in the production of play area mini-roundabouts when the saw he was using, suddenly ‘kicked back’ and his hand was dragged along the exposed blade – his injuries were so severe that doctors were forced to amputate four of the fingers on his damaged right hand)
FATAL EXPLOSION Fire at container yard in Uran; 1 dead, 20 hurt (one person was killed and 20 injured in a major fire following an explosion in a container yard – of the 20, six were injured seriously, while the others sustained minor injuries – there was a chemical combustion in one of the plastic drums, which resulted in an explosion that could be heard several kilometres away – fire then spread to the other containers, which also had chemicals in them – more than 35 containers must have been destroyed in the fire)
Accident at Prairie State Energy Campus puts worker in hospital (a contract employee was injured and taken to the hospital – the employee was evaluated and treated by health care professionals who work full time at the site before being transported to a local hospital and then airlifted to another hospital)
LOTO FATALITY Worker killed at Kansas thermoformer (a setup foreman, 45, was crushed to death when he attempted to correct a problem on the machine he was working on – he was a three-year employee at the custom thermoformer – he was instantly killed after climbing over the top platen to correct what was believed to be a problem with a part that he was working on – “It was quite shocking. He was such a good employee and used good judgment most all the time,” said Huffman, who was on the scene moments after the accident occurred – why he climbed on the machine, will never be known, but it was probably to save the part on the machine – the incident occurred near the end of his shift and that he had been planning to go away for the weekend)
FORKLIFT MODIFICATION Somerset Man Seriously Injured in Forklift Truck Accident (a worker, 36, suffered serious personal injury, following a forklift truck accident at a metal engineering firm – he was guiding a forklift truck loaded with angle irons when the forklift toppled over and lost its load, which landed on top of him – he suffered serious fractures to his left knee and ankle in the accident and his knee was so badly broken that it required the insertion of a metal pin to hold it together – an investigation into the incident revealed the company was using an extension on the forklift which had been manufactured in-house but had not been tested to ascertain its maximum load)
UPDATE All 14 Trapped Mexican Miners Found Dead (Mexican search teams have recovered the bodies of all 14 coal mine workers killed in a gas explosion in northern Mexico)
UPDATE on FATAL FALL Space shuttle worker{apos}s death ruled a suicide (a space shuttle engineer, originally believed to have died in a fall from the Endeavour launch pad, took his own life, according to a new report – he fell 130 feet from Launch Pad 39A at the Kennedy Space Center on March 14, where space shuttle Endeavour was getting prepared for an April launch – according to a report released by the medical examiner had a note in his wallet to his wife and children in which he explained that he was going blind and did not want to be a burden)

TUNNEL FATALITY New train tunnel claimed its third life (one worker died when he was pinned under a drilling rig – the accident occurred in connection with work with the new train tunnel – the rig overturned when the man was moving it – he is reported to have died instantly)
PIPE UNLOADING Construction accident hospitalizes worker (a truck driver was hospitalized with severe injuries after being crushed by four 2,100-pound pipes at a construction site – he was unloading the pipes from his truck when they rolled on top of him, trapping his body underneath – Fire Department was able to remove the man from under the pipe – he was removed from the scene by Care-Flite helicopter and transferred to a Hospital)
CRANE 2 construction workers hurt in crane accident (a construction worker was seriously injured when a crane tipped over and struck him in the chest at a bridge project – the worker was taken to a Medical Center in serious to critical condition – a second worker, the operator of the crane, was also taken to an area hospital and is in good condition)
FALL Worker falls through roof of burned building (it took about 45 minutes to rescue a worker who had fallen through the roof of a burned-out building – the man was working on the roof of a building that was damaged by fire back in March when he fell through and landed on the second floor – when medics arrived they were unable to go up the stairs for safety reasons, so they had to bring the victim out through a window – he was taken to the hospital by helicopter – he is believed to be suffering from multiple fractures)
FATAL FALL Polish worker dies during construction of Euro 2012 stadium (a construction worker, 30, was killed while working on a stadium – he fell some 30 metres to his death as he was working on installing a glass roof over the stadium)
FATAL FALL Construction worker dies in fall from townhouse’s 3rd floor (a construction worker, 31, fell to his death while working from the third floor of a townhouse being constructed – he sustained a fatal skull fracture from the fall – he was cutting wire when he slipped and lost his balance and fell head first on the ground floor)

FATAL FALL from FORKLIFT BOX Man falls to death at work site (worker, 38, dies in job site accident – he fell to his death after falling more than 9 feet off a forklift – a wooden box that was not properly chained to the forklift fell on him – medical examiner said the cause of death was blunt force trauma consistent with the box falling on the victim – he was clear that it was not the fall itself that lead to the man’s death – the victim had been standing inside the wooden box, which was resting on top of the forks of the forklift that was in the backyard near a rear set of stairs – he was applying a concrete stucco finish to an outdoor cooking area while standing inside the wooden box – he walked to one side of the box, causing it to flip off of the forklift and onto the ground – officials estimated the box to weigh between 200 and 300 pounds)

PTO GUARDING Farm worker mangled while spraying (a dairy farm worker, 24, had his clothes torn from his body after falling against a power takeoff (PTO) driveshaft between a tractor and an a spray unit – he was injured after toppling off the trailer carrying the spray unit against the spinning driveshaft – he was taken to a Hospital with lacerations to his legs and grazes to the rest of his body – he was in a stable condition)
Kansas man dies after tree he was cutting down falls on him (a man, 46, died when a large tree he was cutting down fell on top of him – he was a temporary worker and was working alone cutting trees behind the business when he was killed – no one else was injured)
