INDUSTRIAL CONFINED SPACE FATALITY – ENGULFMENT Man killed in corn silo identified as Pharr man (a worker, 21, was killed in a corn silo – he slid 15 feet below the surface of a milling silo that contained corn refuse before firefighters responded at 11:38 a.m. – they had to drain the massive container before they could free the worker, who was found dead – a preliminary autopsy report shows he died from asphyxiation – the mill did not have a confined-spaces permit that would have allowed a worker to perform tasks inside the silo, Edinburg Fire Chief said) LOGGING FATALITY Logging worker killed on the job in Jackson County (a logging worker died after a tree fell on him – dispatchers received a call at 11:11 a.m. that a tree had fallen on a worker who was trimming trees – when emergency personnel arrived at the scene coworkers were performing CPR – the worker was pronounced dead at the scene) OIL WELL WORK OVER PLATFORM Velma man hurt in oil field accident (a worker was seriously injured when he was crushed by a platform at an oil well site – he suffered multiple injuries after the accident – he is in stable condition at a hospital – he was crushed by a work-over platform, a device that’s lowered onto a well in need of repair – either a chain or cable failed and the work-over platform fell on top of him – the platform weighed between 500 and 800 pounds – it took four men to lift the platform off the worker) LOTO Worker’s fingers broken in factory accident (a worker, 52, broke two fingers when a production line was started while she was working on a stitching machine – the line had been stopped while an engineer investigated a mechanical problem – while this was happening, the worker went to the rear of the machine to re-thread one of the bobbins – when the engineer finished, he shouted that he was going to re-start the machine, but the worker was out of sight and did not hear him – when the machine re-started, the woman’s hand was dragged into the moving parts of the machine) ARC FLASH Two workers badly burned in electrical panel blast (an electrical fire critically burned two men as they were hooking up power to a circuit panel at 9 a.m. – the men had been doing some electrical work on the property, connecting the main building to the electrical panel – some kind of flash fire occurred at the electrical box – they were able to exit the immediate location and roll around on the ground to put the fire out – when firefighters arrived, they found one of the men on the ground and the other walking around – both men suffered first- and second-degree burns on more than 30 percent of their bodies – they were taken to a Medical Center burn unit in critical condition – there was no damage to the building other than to the electrical box, and no other injuries) UPDATE on CONFINED SPACE FATALITIES Two Abu Dhabi workers die after inhaling poisonous gases (two men died from asphyxiation at an industrial company – a third worker was injured in the accident – the two had inhaled poisonous gases while performing maintenance in a 10-meter-deep water tank – the men were exposed to oxygen shortage and emissions of hydrogen sulphide and methane which are both toxic – investigation proved that the company did not provide safety equipment such as gas masks and oxygen tanks which could have protected the workers against the poisonous substances) TRENCH FATALITY Gem pit worker dies (worker, 54, died in a gem pit when a mound of earth collapsed on him – seven men were working on the pit on that day – he had shouted to others to escape when he saw the earth collapse – they had tried to save him after climbing out but he could not be saved) CHLORINE Trinity Co. water treatment worker injured in gas leak (Fire Department managed to contain a gas leak before it go out-of-hand, but a water treatment worker is injured in the incident – the worker had gone in a room at the water treatment plant and did not close a chlorine gas valve – he managed to call 911 and is on the way to the hospital – firefighters were able to turn off the gas and keep it contained to that one room) FATAL EXPLOSION @ REFINERY City refinery blast kills 1 (one contractor died and another was seriously hurt in an explosion shortly before 5pm at the refinery – it was not immediately clear what had caused the explosion – the fire that followed the explosion had been extinguished – two injured workers had been working in the chimney where the blast occurred – colleagues attempted to resuscitate the man who died on the scene, with paramedics taking over when they arrived – the other man suffered burns and was admitted to a Medi-Clinic – the incident had occurred during a scheduled maintenance and inspection shutdown) ROOF FIRE Fire closes food plant (a fire on the roof of a food plant heavily damaged the roof and destroyed some of the food manufacturing plant’s electrical and mechanical equipment – operations at the plant were temporarily suspended pending repairs – no word on cause) FIRE Fire burns roof of copper plant (a fire broke out at a copper plant between 3:30 and 4 a.m. – the fire started when a machine that melts copper set fire to the roof of the building – embers from the machine got into the roof system, which contained some wood elements – the estimate cost of damage to the roof is $10,000 – workers who were in the plant at the time were evacuated or moved to the opposite end of the facility – no injuries were reported) FIRE Bama dough plant evacuated for small fire (facility was evacuated because of a small fire around 9 p.m. and about 110 employees were evacuated – no one was injured – the small fire affected the roof of an office building that is not attached to the production facility) AMMONIA Anhydrous Ammonia Leak in Santa Maria (an anhydrous ammonia leak at a commercial produce packing facility – the business was closed at the time and no employees were on the premises – a County Hazardous Materials inspector was notified and the room was ventilated – no one was hurt) CRANE Crane failure causes container crash (a shipping container carrying steel smashed to the deck while being loaded onto a vessel – a crane operator reportedly tried to squeeze the 12.2m container into a small space at the ship yard – it reportedly struck an exposed hydraulic hose, which severed, causing the crane to lose pressure) FORKLIFT Man injured at forklift accident (a man, who is in his 30s, has been taken to a hospital with serious leg injuries after he became trapped under a battery while servicing a forklift truck – he was taken by air ambulance to a Hospital suffering serious lower leg injuries) CONSTRUCTION TRUSS COLLAPSE Three Workers Injured in Construction Accident (three construction workers are injured after they fell 30-40 feet at a construction site just before 3 p.m. – the three men were working on an addition when the truss they were on failed and they fell to the ground – one man had to be taken by helicopter to a hospital – he is in critical condition and suffering from life-threatening injuries – the other two men were also taken to hospitals, but are expected to be okay) WORKZONE FATALITY 4 Outer Ring Road workers mowed down (four workers, including three women, who were leveling the ground along the Outer Ring Road were crushed to death by an unidentified vehicle – the four workers started work at around 7.30 am. – though police are not sure of the time of accident, they suspect that it might have happened between 8 am and 9 am when the workers took a break to have breakfast) LADDER Worker airlifted to hospital (a construction worker was airlifted to hospital after an accident at a farm – a man hired to do renovation work on an empty barn building was struck by a piece of equipment that fell from a height of 12 feet, where another employee was working on a ladder at 3: 30 p.m. – the accident victim received serious lacerations in the incident but is expected to make a full recovery) EXPLOSION FATALITY Worker injured in explosion dies (one of the workers injured in an explosion has died from his injuries – he was 33, and passed away at a Hospital – the man’s two cousins were also injured – the brothers remain listed in critical condition – the three had been working on a house – the blast did severe damage to the home, which was still under construction) FATAL EXCAVATION EXPLOSION Explosion occurs after Time Warner Cable contractor reportedly hits gas line in Kansas City (an explosion that killed a restaurant worker and injured 15 people occurred after a contractor accidentally hit a natural gas line while installing fiber-optic cable – the contractor had given notice that it planned to use a horizontal boring machine to connect fiber-optic cable to a building – officials are still investigating the explosion, which occurred more than one hour after the reported gas leak) Construction worker injured at site of new library (it took over an hour for the Emergency Medical Services and the fire department to lower an injured worker from scaffolding following an accident at a construction site – the man cut his knee and was not in critical condition – witnesses said the construction crew appeared to be trying out several different methods for lowering the man down from the scaffolding, such as using a crane and a forklift, before the paramedics arrived) Construction worker rescued in Naperville (Fire Department rescued an injured construction worker from the top level of a project on a Hospital at about 8:03 a.m. – a male worker had taken a fall while working on the upper level of a four-story addition – he did not fall to the ground and needed to be rescued from the top of the building – the worker was injured when construction material fell on him while he was on the roof – he was taken into the hospital for treatment of non-life-threatening injuries) CRANE Investigation of Bay Bridge crane accident under way (two cranes supported on barges were working in tandem to lower a 129-ton section of the temporary support structure for the bridge’s new suspension span via a large basket onto a barge when the accident occurred – for reasons that have yet to be determined, at about 12:15 p.m., the temporary structure fell out of the basket and onto the empty barge below, causing the crane and its 168-foot boom to lose its center of gravity and also topple over – the temporary steel structure punctured the 99- by 88-foot barge but did not sink it – there were four people conducting the work, two crane operators and two ironworkers, but no one was injured in the incident) MISCELLANEOUS FATALITY Local worker dies in industrial accident (an industrial worker died after suffering traumatic injuries – the accident happened at a local cabinet manufacturer) MANLIFT & ELECTRICAL Boston Fire Department Worker Shocked in Cable Accident (a Fire Department worker suffered an electric shock when the fire alarm system cable he was working on came into contact with a 25,000-volt line that powered commuter rail trains – he was taken to the hospital for examination – his injuries were not life-threatening – a second worker suffered ringing in his ears from a loud boom emitted during the accident – both men were later released from the hospital – the men were part of a crew running cables for the street fire alarm system – one of the men was in an aerial lift to cut the overhead cable while the other worker was on the ground to pull it in when it was cut – the man on the ground received the shock) TIRE EXPLOSION FATALITY Worker killed in Brunswick Auto Shop accident (a worker, 39, died after a tire reportedly exploded in his face – he was filling an industrial tractor tire with air at the time of the accident – he was pronounced dead at the scene) Worker burned at Orlando hotel (a portion of the hotel was evacuated after a worker was burned while repairing an air handler on the roof of the hotel – the man was taken to a hospital – several guests staying on three floors were evacuated due to a leak – the leak was quickly stopped) CHEMICAL 3 workers injured in canister blast (three municipal employees, tasked with combating the dengue-carrying mosquitoes suffered facial burns when a spray canister exploded in their faces – they were taken to a Hospital, with second and third degrees burns – there were previous incidents with the equipment but they managed to overcome them – they are not trained nor equipped to deal with such accidents – the explosion was caused due to heat) |
