INDUSTRIAL BAND SAW Man airlifted to hospital after industrial accident in Hatch Beauchamp (worker was airlifted to hospital after he trapped his arm in industrial machinery at 2.49pm – he was trapped in a band saw and firefighters used hydraulic rescue equipment and small tools to release him) LOTO FATALITY New Bedford man dies in shucking machine accident (worker, 35, died after getting tangled in a shucking machine at a local fishery at about 1:30 a.m. – he was pronounced dead at the scene – he was in the process of flushing out a shellfish shucking machine when the accident happened – he became entangled in a rotary turbine engine and it took the Fire Department approximately an hour to free the man after dismantling parts of the machinery) MINE FATALITY Contract worker dies in accident at Tasmania mine (an accident occurred at the mine fatally injuring an employee of a mining contractor – it was believed that the loader operator was caught in a mud rush in the lower levels of the mine – this was the third fatality at the mine in less than six weeks, after two employees were killed on December 9 following a 35 m fall from a maintenance platform in the mine’s main shaft) LOADING PIPE Accident hospitalizes worker in Duquesne (an accident sent a worker, 27, to the hospital where he is listed in good condition – he sustained injuries to his trunk when he was struck by falling pipe around 8 a.m. – he sustained three broken ribs, an injury to his left eye, and his arm was temporarily immobilized – they were loading pipe, and there was one that he put into place – he turned and it broke free – three of the pipes fell on him) LINEMEN ARC FLASH Workers suffer electrical burns in Maui industrial accident (two men, both 26, suffered electrical burns while working on power lines – one of the workers reached for a temporary jumper line when witnesses heard a loud noise followed by an electrical arc – the men were initially treated at a Medical Center and then taken to a Hospital for further treatment) ACETONE FIRE @ PAINT PLANT Fire at paint manufacturing plant in St. Louis burns worker (a fire fueled by a pipe carrying acetone burned a worker on the legs – the fire at the paint manufacturing plant left firefighters scrambling to find a shut-off valve to the acetone pipe that was fueling the fire – the fire, which started at the pipe, was contained to a small area on the first floor of one of the buildings, but was fueled by acetone running through the pipe – one fire company kept the fire contained while another fire company went around the building for about 25 minutes trying to find the appropriate shut-off valve – the worker was in stable condition in the ambulance) FIRE Explosion at Vulcan plant in Foley injures workers, two seriously (an explosion at an aluminum plant seriously injured two workers – incident happened about 9:40 a.m. at the facility – fire was isolated to one piece of machinery but the two workers assigned to the equipment suffered substantial burns – one was flown immediately to a burn center and the other was taken by ambulance to a Regional Medical Center and later flown to a burn unit – about 50 others employees were treated for precautionary measures at the scene but did not require assistance beyond that) FATAL EXPLOSION Blast in Silvassa unit kills four workers (four persons were charred to death and four received serious burn injuries in a blast at a polythene plastic product factory – at the time of the incident, the workers from the night shift handed over the charge to those in the morning shift – there were around 20 workers inside the factory premises when the blast took place – the exact cause of blast is yet to be ascertained, primary investigation suggests that it might have occurred during the mixing of the chemicals for the manufacturing of polythene bags) FATALITY Worker found dead at Suncor oilsands facility (worker had been missing for several hours before body found – emergency personnel with the company were called in around 7 a.m. to look for the worker – the worker was found dead on the scene a few hours later – employee was found in the sand-dump area, a drying area that is part of our extraction process) UPDATE on TANK EXPLOSION Lawsuit: Hurt worker seeks up to $1M from crude oil tank explosion in Galveston (a man, 50, seeks up to $1 million in damages in a lawsuit that claims he was injured in a crude oil tank explosion nearly two years ago – he suffered injuries that weren’t life threatening during the Feb. 23, 2012, explosion of a crude oil tank – city officials said five people were hurt – four workers were treated for smoke inhalation, and a firefighter suffered heat stress – lawsuit claims negligence, among other things) No Injuries in DuPont Plant Incident in Texas 1/15/14 (a loud noise and flaring drew emergency responders but a company official says there was no explosion or fire in the unit – no workers were hurt – the emergency shutdown has closed one polyethylene unit amid the company investigation into the incident – the sound was decompression and the unit shutting down as the safety system worked – firefighters responded but had no damage reports and never entered the plant) CHLORINE Camano Island chlorine tank leak capped; most evacuated residents allowed to return home (a leaking canister of chlorine gas was safely capped overnight and most neighbors who had been evacuated were allowed to return – 8 people were treated for breathing difficulties at a Hospital – three or four more were evaluated by medics at the scene – about four-dozen people were evacuated because of the strong odor of bleach – about a dozen stayed at a temporary shelter – the leak was caused by a man who cut into a 4-foot cylinder he planned to salvage) CONSTRUCTION MANLIFT FATALITY One man killed in construction accident on Interstate 40 near exit 42 (employee has died at a hospital after an accident at an Interstate 40 worksite – accident happened at a bridge near exit 42 – worker reportedly was pinned between a lift machine and the underside of the bridge) WORKZONE Authorities say driver hit road worker on Interstate 80 near Lincoln before taking off (authorities are looking for a driver whose vehicle hit a road worker on Interstate 80 – the road worker was hit as he put up a traffic barrier – he was taken to a local hospital with serious but non-life-threatening injuries – the driver took off after the accident) Construction accident injures worker (a construction worker became trapped after an accident – he was operating a front-end loader when it tipped over – fire department had to extricate him through the front windshield of the loader – he was then taken to hospital with non life-threatening injuries) FATAL FALL from ROOF Construction worker dies following 40-foot fall (a construction worker, 57, has died following a 40-foot fall from the roof of a Middle School construction site – emergency crews responded to a report of a man who fell 40-feet from the roof – he was unloading some insulation from a forklift on the steel decking of the roof at about 1:45 p.m., when he lost his footing and fell backwards over the edge landing on a snorkel lift – “He was not wearing his harness, our workers are instructed to wear their harness in those situations and for some reason he chose not to wear the harness” – he had been employed for about four years) FATAL FALL from ROOF Worker dies after N. Idaho apartment complex fall (a worker,67, fell at an apartment complex under construction – he was deceased when officers arrived – he had been working as a sub-contractor on some new apartment buildings – he was on the roof of the complex measuring for rain gutters when he fell – witnesses reportedly saw him fall and told authorities he was not wearing a safety harness) Construction worker dies in Madison accident (a construction worker has died at the site of a hotel renovation – the worker died after a “serious accident” at the site around 2 p.m. – declined to say what the worker was doing or where the worker was on the site) FATAL COLLAPSE Worker killed in Target Canada construction site accident identified (a worker was killed in the collapse of a roof at a construction site – it has not yet been determined what caused a metal roof at the site to fall down, badly injuring and trapping him underneath for about an hour – rescue efforts using large machinery quickly commenced at about 11:30 a.m. but he was pronounced dead by the time he was pulled out) Two workers injured in Calgary construction accident Monday morning (an accident at a residential construction site has injured two men – the victims were construction workers who were injured when several walls collapsed at a three story home under construction – Fire Department crews discovered one of the victims had fallen about 15 feet when the walls fell over – another construction worker was also injured in the incident – both men were treated by EMS personnel and transported to hospital – incident is being blamed on high winds) COLLAPSE during CONCRETE MIXING One construction worker killed in Mexican building collapse (a building collapsed, killing a construction worker and seriously injuring 20 others – the workers were mixing cement on an upper level platform for a new three-story building – the platform couldn’t stand the weight of the concrete mixture and collapsed, taking down the workers working above and crushing others below – a 30-year-old construction employee died and 20 other workers were taken to nearby hospitals with blows to various parts of their bodies – 14 other workers escaped major injuries) FATAL FALL Construction worker falls from catwalk, dies (a construction worker, 60, died after falling from a catwalk – he was working on top of a catwalk inside an auditorium at the Performing Arts Building, when he fell about 30 to 40 feet – he was taken to a Medical Center, where he was pronounced dead – investigators say it is possible he suffered a medical episode, which caused him to fall off the catwalk) Miscellaneous CONFINED SPACE FATALITIES Fourth worker in Cujira STP accident dies (worker, 35, who fell critically ill after getting into a hotel’s sewage treatment plant (STP) died on Thursday – on Wednesday, three employees of the hotel died while cleaning the STP – the postmortem reports of all four, said the cause of death was asphyxia with signs of drowning in sewerage material) CONFINED SPACE FATALITY Worker suffocates to death (a worker, 75, who was cleaning a guttter pit died of asphyxiation – a group of about 15 labourers were working – one had got down into the 3×3 feet pit – when he did not come out for a long time, a few other workers got into the drainage pit and dragged him out – he was unconscious by then – he was shifted to a nearby hospital where he was declared brought dead) HOTWORK EXPLOSION Welder Injured in Explosion at Auto Shop (a welder working on an oil storage tank outside an auto shop suffered severe burns in an explosion – three explosions were heard just before 9 a.m. – employees at a neighboring tire shop ran over to find the man on the ground – the worker was doing welding and grinding work on the tank used for storing used engine oil – the side of the tank is now ruptured – the auto shop and three cars at were heavily damaged by the fire) FORKLIFT Luke Bryan stage collapse injures crew: forklift crash results in damage after country star’s Ohio State show, Kentucky concert postponed (following a concert the stage collapsed, resulting in several crew members being rushed to the hospital for injuries – a forklift crashed into the stage while workers were dissembling the concert setup – the entire structure crumbled due to the impact – four individuals (including members of the country star’s crew and arena staff) were rushed to a nearby hospital to treat injuries – medical statuses still remain unknown) |
