INDUSTRIAL CONFINED SPACE FATALITY – OVEN Worker cooked to death in oven ‘tragic loss,’ Bumble Bee says (a seafood plant worker, 62, was cooked to death in an industrial oven about 7 a.m. – employee died at the scene – he had worked for the business for six years – an initial investigation indicated that he “was fatally injured when he was cooked in an oven” but it was unclear how he ended up inside the oven, resulting in this “horrendous tragedy”) FLASH FIRE caused by STATIC Worker severely burned from acetylene fire (a man was burned on the job – he suffered first and second degree burns to his upper body when he was cleaning out bottles that previously held a highly flammable gas when they ignited – investigators think static electricity caused the explosion ELECTRICAL Xcel Energy Employee Hospitalized Following Morning Accident in Grand Forks (an employee, 40, was seriously injured while working on electrical equipment – was taken by ambulance to a Hospital and later airlifted to a Hospital where he remains in good condition – it appeared the worker had suffered a severe electrical shock while working in or around a transformer box) CRANE FATALITY Worker dies in accident at Rockhampton site (a worker, 37, died after being crushed by a metal beam – the man’s death was instant – a steel beam came loose from an overhead gantry crane, falling on the man) Two GM Employes Seriously Injured In Accident At Bedford Plant (two people were injured at a Casting Plant at 3 p.m. – the two men were flown by helicopter from the plant at 4:47 p.m. – no other details about the accident were available and that a health and safety team would be called to investigate the accident) PROPANE TANK FIRE 2 workers burned at Norfolk food plant (two workers have been burned in an accident at a food plant at about 1:20 p.m. – the propane tank fire that prompted the 911 call already had been extinguished, but as workers checked the propane system, propane spewed out of a pressure relief valve and was ignited by a pilot light on a nearby machine – that new fire soon ignited gas coming from a pressure relief fitting on another machine – two plant workers suffered second-degree burns on their faces, heads and arms – they were treated by firefighter medics but not hospitalized) BIO-MASS BOILER EXPLOSION 1 treated in Sitka boiler explosion (a contractor was briefly hospitalized when a boiler exploded at a Coast Guard hangar- the Coast Guard says in a release it reported the biomass boiler explosion at 12:30 p.m. – the contractor was taken to a local hospital, was treated and released and back at the air station later in the day – two other biomass boilers were taken offline as the Coast Guard investigates the cause of the explosion) FATAL HOTWORK EXPLOSION One killed in tanker fire during repair (one person was killed and two others were seriously injured when an empty tanker caught fire during welding – suddenly the tanker caught fire and there was an explosion – three persons were on top of the tanker) REFINERY FIRE Lightning strike causes fire at Venezuelan refinery (a fire broke out at an oil refinery – a lightning bolt hit the refinery during a thunderstorm and started the fire, which was quickly brought under control by plant employees and firemen – the fire affected the refinery’s extraction plant – the fire also caused explosion in a secure area” of the plant, without mentioning any casualties) TRANSFORMER EXPLOSION Massive fire at BHEL plant in Bhopal (a massive fire broke out at a plant when a 200 kW transformer exploded and the 19,000 litres of oil that it contained caught fire around 9.45 am – the transformer was being tested – with the impact of the explosion the roof of the electrical panel building was blown off – the transformer contained 19,000 litres of oil which spread in the area including the underground cable gallery – as a result a massive fire engulfed the entire area) AMMONIA Ammonia leak sickens workers at Fresno winery (authorities say they have fixed an ammonia leak that sickened workers at a winery, but the area remains cordoned off – investigators determined that a pipe was the source of the leak at the plant – 10 workers were treated for ammonia exposure after the leak created a vapor cloud – three were taken to a local hospital – the winery is in an agricultural area, so there are no homes within the 1-mile evacuation zone – residents nearby have been advised to stay indoors, turn off their air conditioners and keep their windows closed – last month, an employee at another winery died after being exposed to ammonia, which is used as a cooling agent) AMMONIA Chemical leak at C&D factory in Longford (an ammonia leak led to the evacuation of the plant – two units of fire services were called to the pet food manufacturing plant after locals reported a strong smell of ammonia in the town and in a children’s playing area near the plant – fire services worked for four hours to help contain a leak of ammonia at the plant – production has now recommenced and the alert is over) CONSTRUCTION TRENCH FATALITY Man killed at Burnaby construction site identified (one man, 28, is dead after a retaining wall collapsed on a construction site – he was working in a trench below ground level when a retaining wall gave way, trapping him and another worker – he was pulled out and taken to hospital but died – the other worker survived) CRUSHING FATALITY LA freeway construction worker crushed by beam (a freeway construction worker, 42, was killed after a 3,000-pound steel beam fell and crushed him as it was being unloaded from a big rig – the accident happened on a closed off-ramp that is part of a 10-mile widening and carpool project – ten or 15 co-workers witnessed the accident, which happened just before 2:30 a.m. – the men were working on sound walls and retaining walls on the off-ramps – no one else was hurt) WORKZONE Knoxville TDOT worker in serious condition after work zone accident (a DOT maintenance worker, 34, is in serious condition after being struck by a car – he was working on US 11E when the accident happened – still waiting to get details on what caused the accident from the Highway Patrol – he suffered a broken arm, a broken leg, a cracked pelvis and a fractured skull and was flown to a Medical Center) FALL Worker falls from 5th floor at Baptist construction site (a worker was rushed to a local hospital after falling from the fifth floor of a building that’s under construction – the accident happened about 10:30 a.m. at the construction site – the extent of the worker’s injuries was not immediately known, but hospital officials said he was conscious when taken by ambulance to the Medical Center and was last listed as critical – the iron worker was wearing safety cables and harnesses at the time of the accident) PARKING GARAGE COLLAPSE FATALITY Trapped worker freed in deadly Fla. garage collapse (a construction worker was pulled out alive from under a huge slab of concrete 13 hours after a parking garage collapsed, killing two others – one worker was still unaccounted for and rescuers said they expected to find another body in the rubble – the survivor was in “extremely critical condition” and was to be transported by helicopter to a trauma center – seven workers were brought to the hospital with unidentified injuries while an eighth was treated on the scene and sent home – an internal review was being launched to determine the cause) FATAL FALL from WORK PLATFORM 2 Wacker Polysilicon Employees Killed In Fall At Charleston Facility (two construction workers lost their lives in a work-related accident while performing their job responsibilities at the plant – they were pronounced dead at the scene by EMS – the two construction workers fell to their deaths about 140 feet after a work platform failed) LOTO FATALITY – CLEANING Cement Mixer Worker crushed in cement mixer (a labourer, 40, who was cleaning a cement mixer was crushed to death when his brother-in-law accidentally turned on the machine – he was cleaning the machine around 5pm at the site of an underground parking – the brother-in-law reportedly turned on the machine and left for a drink of water – when he returned in a few minutes and found the machine had stopped working, he checked and found his brother-in-law inside) FLASH FIRE Lacquer fumes ignite, contractor injured in Libertyville basement fire (a construction worker was injured when a fire sparked in the basement of a home where he was working at 5:14 p.m. – contractor was using lacquer to seal the concrete floor when vapors reached an ignition source and the fumes ignited – the fire was quickly extinguished, but not before the contractor who was working on the project suffered minor burns and smoke inhalation – the contractor was taken to a Medical Center for treatment of minor, nonlife threatening injuries) TRENCH FATALITY Man dies in Fresno Co trench accident (a man, 57, is dead following a bizarre trench accident – he was helping his friend dig a trench when the sides collapsed on him – investigators say dirt buried and killed him – fire crews were called in to recover his body) MISCELLANEOUS TREE CUTTING FATALITY Worker killed by falling tree in Tynedale is named (a worker, 23, is the victim of a fatal tree-cutting accident – he was killed when the tree he and a colleague were cutting down suddenly split and fell on him) FARM FATALITY Man Killed in Farm Equipment Accident (a man, 53, died while working on farm equipment around 11:30 p.m. – the victim was in a barn working on a combine-head at the time of the accident – the combine head was detached from the combine itself, it was lifted up by a forklift, however while the male was working on the piece of equipment, the combine head slipped off the tongs of the forklift and fell on top of him – another worker inside the barn did his best to free him, but by the time he was able to lift the piece of equipment off the man it was too late) LOTO Disneyland Worker Hospitalized After Accident (machinist, 68, is hospitalized after being struck by popular roller coaster – he remains in a critical-care unit – he suffered head, chest and back injuries as he worked on the ride – Resort spokeswoman says the ride didn’t malfunction and was back in operation the next day) HOT WATER HEATER EXPLOSION Blackhawk students evacuated after water heater explosion (approximately 600 students were evacuated from a School after a small explosion involving a water heater – a maintenance worker was working on the water heater in a boiler room when the accident happened at 9:37 a.m. – he was uninjured but was taken to a hospital by car as a precaution – an accumulation of natural gas in the water heater’s firebox might have led to the explosion – there was no fire or structural damage to the building, but children were evacuated when gas fumes penetrated some portions of the building) |
