DRILL RIG FATALITY Fatality in accident near Parachute (employee died as a result of an incident of a drilling rig – no other details)

OIL PLATFORM FATALITIES Three Azeri workers die in offshore platform accident (three workers were killed and one is still missing after the accident at one of the offshore oil and gas platforms – one of the platforms collapsed during repair works and a small wagon-house fell into the sea, damaging and causing a fire on a pipeline – there were 41 workers on site: 37 of them were evacuated by a rescue team, while four fell into the sea)

CONFINED SPACE FATALITIES Update: Men killed in wastewater plant accident were making repairs (two men, 34, and 40, had been performing repair work in a pump pit between two clarifiers at the plant – a leak occurred in the pump pit, it began filling with water – one of the workers went into the pit to attempt to close a valve, but was not able to remove himself from the pit – another worker entered the pit and also was unable to escape – both men were contract workers)

GAS DRILLING BURNS Worker sprayed with hot gas at East Texas well site (a man was flown to a hospital following a drilling well site accident at around 9 a.m. – worker was opening a valve to relieve pressure at a well and he was sprayed with hot gas – he was burned, but his injuries are not expected to be life-threatening)

FATAL EXPLOSION Construction worker dies after accident at Drax (one construction worker, 32, died and a second worker suffered minor injuries at Biomass plant at approximately 6:30 p.m. – the two individuals suffered injuries while conducting tests at the site – the second worker received minor injuries and was treated locally)

FORKLIFT Jamesburg worker injured in forklift accident (a worker, 30, is in critical but stable condition after he was injured in a forklift accident – when police arrived they found a worker who had been operating a forklift on the property was trapped in the seat after the lift portion of the forklift crashed onto the roof of the cab area trapping him in his seat)

LOTO/GUARDING AMPUTATION Oregon Worker Severs Fingers At Potato Plant (a female employee severed four fingers on her right hand in a workplace accident at a potato plant – she was cleaning around machinery used to cut potatoes when the accident occurred)

FORKLIFT FATALITY Woman killed in forklift accident is identified (worker, 22, was killed while operating a forklift – she was operating a forklift and apparently it malfunctioned and subsequently caused her death – she’s only been on the job a couple weeks just started on the forklift a couple days ago just trying learn everything)

CRUSHED Worker crushed between two trucks (worker, in his 30s, was crushed between two trucks just after 9.15 a.m. – that followed an incident in which a sub-contractor, 50, fell and fractured his leg about 8.55am. – the crushed worker had been part of a team cordoning off an area where they were to begin work strengthening power poles – police believe he had been standing next to a stationary truck when he was crushed by another as it drove by)

MINE FATALITY Worker killed at North Antelope Rochelle Mine identified (contractor, 51, was thrown from haul truck when as it went over a high wall – the contractor was driving a haul truck when it went over a high wall and was thrown from the truck)

FATALITY Pilgrim’s worker dies in accident (accident happened apparently between noon and 1 p.m. – victim’s body was found by his fellow employees – worker, 55, was pronounced dead at the scene, in the “ice room” of the plant – first impression was that the victim had succumbed to a heart attack, but upon closer examination it appeared he had been struck by something)

ELECTRICAL Electrical worker shocked on the job in Ottawa Co. (an electrical worker, 22, is in the hospital, after being shocked on the job – a gust of wind broke his measuring tape, which then hit a high-tension wire, shocking him – co-workers had to help him off the tower, more than 100 feet in the air – he was taken to a Hospital for burns on his arms)

HOTWORK EXPLOSION Two injured in flash fire at biodiesel producer (two workers were apparently making welding repairs to the tank – one of the injured as having “multiple lacerations” from the blast – ethanol residual in the top of the tank reportedly caught fire and flashed on the workers – workers were doing some welding repairs on top of one of the roughly 50-foot-high tanks when some ethanol inside the tank caught fire)

CONFINED SPACE FATALITIES – H2S Worker likely died in ‘valiant but futile’ bid to save friend (a leather factory worker had likely entered a pit of toxic wastewater in a “valiant but ultimately futile” bid to save his friend – worker, 26, and his friend and colleague, 37, were pulled from the pit, unconscious – both died in hospital nine days later – one had likely entered the pit, known as a sump, in search of his countryman, after realizing that his friend could have fallen into the 3m pit while retrieving two plastic filters directly above it – he had likely fallen into the 2.5m-deep wastewater, which contained chemical by-products from the leather tanning process, after being momentarily disoriented by a “moderately high concentration” of hydrogen sulphide fumes around him)

CHEMICAL RELEASE Chemical leak contained at Linden refinery (a chemical leak at a refinery did not pose a threat to the area – officers responded to a report of a chemical cloud around 8:30 a.m. – refinery employees sheltered in place because there was a release of a 15 percent solution of ethyl aluminum dichloride)

FIRE Fire contained at Elkhart County plant (the fire started in the front of the building because that is where the heaviest fire was at the time – crews have now extinguished most of the blaze, but remain on the scene putting out hot spots – there was at least one explosion in the structure during the fire – it was an acetone barrel – no injuries were reported and despite a lot of smoke coming from the blaze, no evacuations were issued for nearby residents)

EXPLOSION Explosion At Plant Leads To Minor Injury (Fire and EMS departments were dispatched at 5:02 p.m. for a report of a press explosion and fire – the cause of the initial explosion remains under investigation – the initial explosion ignited a sizeable solvent fire in a press)

AMMONIA Villisca ammonia release contained, recovered (ammonia released from an agricultural cooperative is contained and being recovered – an unknown amount of anhydrous ammonia was released into a city storm sewer intake, then flowed into a small unnamed tributary)

CONSTRUCTION

FALL Worker rescued from fall into SR 520 pontoon (worker fell 15-20 feet into the hollow concrete flotation support – he sustained non-life threatening injuries to his lower body in the fall and he was transported to a local hospital for treatment)

FATAL FLOOR COLLAPSE Worker killed in construction site accident in Gaziantep (worker, 25, died after the mezzanine floor of a building being constructed collapsed – when the mezzanine floor collapsed he became trapped between steel and concrete sections of the building – he was pulled out by other workers at the site and rushed to the hospital, where he subsequently died)

CRUSHING FATALITY Construction worker dies on-site in southern Saskatchewan (worker, 43, died in a construction-site accident around 12:35 p.m. – investigators believe the man was working at the construction site when he became trapped under machinery – he was pronounced dead at the scene)

CRANE JIB COLLAPSE Man injured as crane collapses at Wates site in Newcastle (worker was taken to hospital with minor injuries following a crane jib collapse – the crane had not been in use when the incident happened, and was being lowered at the time – the collapse was caused by a snapping cable)

WORKZONE Construction worker struck along I-475, vehicles flees (a construction worker was injured in a hit-and-run along I-475 – worker was taken to a Medical Center after being struck – the vehicle fled the scene after striking the worker – the suspect vehicle was located a short while later; however, the driver was no where to be found)

EXCAVATION EXPLOSION Excavation worker killed in Ludwigshafen gas pipe explosion (a gas explosion has killed an excavation worker and injured more than 10 people – several houses have been damaged – it created a crater 10 meters (33 feet) in diameter and 5 to 6 meters deep – workers had attempted to dig down to a buried line near apartment blocks at the time – as well as near a factory site – it remained unclear what had damaged the pipe)
