INDUSTRIAL FedEx worker injured in plant accident (a worker was injured on the job at a company plant – details regarding the incident are unknown at this time, but the man was transported to a Medical Center) TRUCK LOADING FATALITY Worker dies in tank accident at Abilene company (a worker, 38, at an oilfield services company has died after he was struck by a tank being loaded on to a trailer – he was helping load the trailer when the tank rolled – he suffered a head injury and was dead at the scene) CRANE Another accident at Bhushan Steel site, three hurt (close on the heels of death of four workers in a fire mishap, another accident occurred at the Steel plant injuring three contractors, 25 & 26 – the accident took place while an iron beam being lifted by crane collapsed – the three workers standing on the ground were injured) MINE FATALITIES Gas leak leaves 8 dead in Turkish coal mine (a gas leak inside a coal mine in northern Turkey killed eight workers – the miners were working on opening up space to reach a coal deposit when they were hit by a sudden methane gas discharge – eight workers died from breathing the gas or getting trapped under coal dust that blanketed the mine after the discharge – one worker was rescued with injuries, and six others were evacuated unharmed) UPDATE on FATAL BURNS UPDATE Temporary worker to blame for his fatal burns (a manufacturer of household and grooming products, denies negligence in a wrongful-death suit filed by survivors of the worker – the company that was supervising the temporary worker when he suffered fatal burns in 2011 says the man failed to take “proper precautions” in its factory and calls his conduct “the sole proximate cause of the injuries” – worker, 50, was scalded over most of his body with a hot citric-acid solution November 17, 2011 and died three weeks later in a burn unit – the death triggered inspections by OSHA and led OSHA to fine the company $473,000 last May for 14 alleged violations – an internal OSHA memo, obtained as part of an investigation says more than 98 minutes elapsed after the incident before the worker reached the hospital – factory officials refused to call an ambulance as the worker awaited help, shirtless and screaming, OSHA investigators contended – instead of calling 911, the company had another temporary worker drive the injured to an occupational health clinic that was not set up to treat life-threatening burns – as the worker cleaned a 500-gallon tank from which the acid solution erupted, “he knew or reasonably should have known of the possible risks of personal injury,” the company filing adds. “By voluntarily undertaking to work with such substances, [XXXXX] elected to accept such possible risks.” FATAL FIRE @ WASTE DIESEL Plant Blaze kills factory employee (a worker, 22, died in a fire at a waste diesel factory – cause of the fire was still under investigation – the fire occurred about 9:20 am when the factory was processing waste diesel – the fire was put out by firefighters, who found the worker’s burned body in the factory) FALL into CHEMICAL VAT Man saved from vat of boiling chemicals (a co-worker saved a man from being completely submerged in a vat of boiling chemicals after he fell partially into the cleaning solution at a manufacturing plant – the co-worker immediately stripped off the victim’s exposed clothing and followed proper decontamination procedures before the injured man was transported to a Medical Center – he was later transported to a burn center – the worker, in his early 20’s, fell into the vat shortly before 6:30 a.m. – the chemical solution, which was 375 degrees, is used for cleaning metal machine parts – the chemicals covered both of the man’s legs up to just above his knees before his co-worker grabbed him and pulled him out – the two employees climbed onto an elevated tank, about 7 feet above the floor, to recover metal parts that had fallen into the 5-foot-deep container after being dipped for cleaning (sodium hydroxide and potassium hydroxide) – they were trying to fish out these parts from the bottom – ‘That was not the normal procedure to recover parts that have fallen off”) FALL into SHIP HOLD Worker falls into ship’s hold (a worker, 54, was airlifted to a hospital after falling into the hold of a ship – he fell anywhere between 10 and 17 metres at about 1:30 p.m. – his injuries were not considered life-threatening, though he’s still undergoing tests and X-rays – he landed on the steel hull at the bottom of the ship, which had no load on board – the worker, a subcontractor installing a scaffold system in the hold, was put into a basket and a rope system was used to bring him to the deck of the ship) ARC FLASH Two Electrical Workers Treated After Willow Power Line Blast (two electrical workers who were blown away from an electrical pole by an explosion were taken to a hospital for treatment – a journeyman lineman and an apprentice were knocked away from the pole by a “flash-type explosion” – the men were wearing safety harnesses and didn’t fall to the ground – other members of the crew used a cherry-picker truck to retrieve them, and they were flown to a Regional Medical Center – the lineman was treated and released while the apprentice was kept for observation – no cause has yet been determined for the explosion) UPDATE on POWER PLANT EXPLOSION No charges in 2011 coker blast that injured five workers (no charges will be laid after an explosion at an energy site that injured five workers – the January 2011 blast in the coker at the site sent flames and smoke more than 100 metres into the air – Alberta Occupational Health and Safety investigated and had two years less a day to file any charges, but department officials say they have decided not to take any action as there was no likelihood of a conviction) STRUCK by FORKLIFT Man struck by forklift at Leetsdale plant (a forklift accident at a local industrial plant has left one man, 51, hospitalized – worker was walking in a red zone, that’s a danger zone, and he wasn’t paying attention – a fellow employee was driving a forklift and didn’t notice the worker walking behind him – he was hit and got lodged underneath the forklift – it took rescue crews about an hour and fifteen minutes to free him from the forklift – he was taken to a hospital – his condition was unknown) FIRE @ NUCLEAR POWER PLANT Fire breaks out at nuclear power plant in Texas (a fire broke out at a nuclear power plant, forcing one of two units to be shut down – no injuries were reported in the fire, which erupted in the Unit 2 main transformer – the fire was quickly put out – the fire posed no hazard to the public or to plant workers) FIRE Fire damages Galva, IA plant (fire has damaged a soy plant – the fire broke out in some pipes inside the plant at about 5:00pm – firefighters put it out quickly and spent about two hours on-scene – no one was hurt – no word on damage, but the plant is shut down) BOILER EXPLOSION Nobody hurt in ADM boiler explosion in Peoria (authorities are working to determine how much damage was done when a boiler exploded at a plant, resulting in a gas leak and fire – the boiler explosion was reported at about 3:40 a.m. – no injuries were reported) CHEMICAL Vapor leak at bleach plant contained (a chemical warning sensor inside the plant alerted officials that chemical readings within a confined production room were above normal – the fire department received the call around 4:16 a.m. – firefighters and plant officials found that a product at the plant had released vapor inside a confined room – the vapor could have come from bleach or chlorine – there were no chemical readings outside the plant and no threat to the general public – fewer than 10 employees on site at the time of the incident – all employees were evacuated from the plant as a precaution prior to firefighters’ arrival – no injuries were reported) FIRE Workers get out OK at Pa. plant with furnace fire (workers got out safely and no other injuries have been reported after an electronic furnace used to melt raw materials malfunctioned and caught fire – the furnace recently had a new liner installed – workers were melting materials in the furnace to season the liner when the fire broke out) FIRE @ NUCLEAR POWER PLANT Fire at Sydney nuclear plant extinguished (a fire that broke out at a nuclear reactor has been extinguished and is not being blamed on hot weather conditions – a “smoldering fire” began in electrical cabling at 10.30 a.m. – it was a short-circuit in the wiring) CHLORINE 9 workers fall ill in chlorine gas leak (at least 9 workers fell ill due to leakage of chlorine gas at a Thermal Power Station (TTPS) – the workers were sent to a Hospital and all of them were out of danger – as the chlorine gas leakage incident came to light, the authorities evacuated all the workers from the plant premises and repaired the leak) FORKLIFT FATALITY 1 man killed in industrial accident (one man is dead after an industrial accident at a recycling warehouse – he was crushed when a forklift tipped over onto him – no other details) CONSTRUCTION WORKZONE FATALITIES 2 Highway Workers Struck, Killed (a deadly accident has killed two highway construction workers – the driver slammed into two construction workers in a well-marked work zone, killing both of the workers – State police don’t know what caused the collision – the workers were both contractors for the State Highway Administration – they were pronounced dead at the scene) CEILING COLLAPSE Ceiling Collapse In Historic Murray Hill Building Injures Three Workers (three male workers were injured in a construction accident around 1:40 p.m. on the third floor of a building where renovations were taking place – the workers were positioned on a scaffolding on the third floor when the ceiling above them gave way – fire officials had to bring in a special unit to help free one of the workers who was trapped under the debris – the worker trapped under debris was in serious condition, another worker had a serious head wound and the third man had minor injuries) SCAFFOLDING 3 Injured in Scaffolding Accident at Austal (three workers were injured in a scaffolding accident around 4 p.m. – two people were seriously injured and a third suffered a minor injury – it’s unclear exactly what happened – the scaffolding that the workers were on, either fell over or collapsed – they were about 10 feet in the air) BOBCAT (the style NOT the brand!) FATALITY Worker Dies After Being Hit By Construction Vehicle (a worker has died after he became trapped under a piece of construction equipment – witnesses reportedly told police that somehow the person was run over by an earth mover and was trapped beneath it – the cause of the accident is under investigation) UPDATE now a FATAL FALL Greenwood worker dies of injuries from fall (a city employee, 52, has died of injuries suffered in December fall – he died this past week in a hospital – he fell Dec. 12 while replacing a stop sign at the city intersection and hit his head on the ground) CRANE Seven construction workers hurt after crane collapses onto Long Island City, Queens work site (a construction crane owned by a firm with a checkered past collapsed, pinning three workers and injuring four others – the crane toppled at about 2:20 p.m. – three workers were trapped under the heavy metal apparatus, but there were no life-threatening injuries – workers said they heard cables snap and watched the 380-foot red crane buckle high in the air – the crane was erected just four days previously – workers were loading the crane with wood planks when the accident occurred – the crane is owned by the man who was acquitted of criminally negligent homicide charges stemming from a 2008 crane collapse that killed two workers – the five workers, ages 23 to 55, were expected to leave the hospital) FALL Shelton construction worker falls 45 feet down well (a construction worker, 33, was rescued after falling through rotted boards down a 45 foot hole from an abandoned well – he was demolishing a shed and was standing on what he thought was a four-foot deep sump when the ground gave way – those first on the scene were able to get a flotation device and protective clothing to the man, who was treading water to stay afloat) NAIL GUN Construction worker shot in the head with a nail gun (the man was working on a home when another worker accidentally shot him in the head with a nail gun – the injury was so unique; the man had to be transported to the University of Iowa hospital for special care – no word yet on the man’s condition) MISCELLANEOUS Tree Cutter Killed In Greenwich (a tree worker, 26, was killed when a tree trunk he was cutting fell onto him – he was working with his family’s landscaping business when the accident occurred at a private property) FATAL FALL from TREE Tree Worker Dies After Fall In Greenwich (a man died after a fall of nearly 30 feet – a third party from an adjacent property called 911 at about 3:05 p.m. – rescue crew arrived on the scene and performed CPR – he was transported to an emergency room, where he was declared dead) FRYER FIRE Flash fire at Ocean Fish Bar in Altrincham (another warning has been issued over the danger of chip pans and deep fat fryers, after a fire alert at a chip shop – it is believed a build up of heat caused a flash fire in a deep fat fryer – flames were prevented from spreading when a worker slammed shut the fryer’s lid – no one was injured) |
