INDUSTRIAL H2S FATALITY Outsourced worker dies after leak of H2S gas at Venezuelan refinery (contractor, 38, was reported dead after a leak of hydrogen sulfide (H2S) at refining complex – he was working as a foreman for a contractor firm – although there is no information about how the accident took place, the man is said to have died after inhaling the gas while evacuating area) OIL FIELD EXPLOSION Oil Field Worker Critical After Being Burned in Robertson County Explosion (worker, 25, was badly burned after an explosion at an oil field site – some oil tanks caught fire – an oil well was being capped – the man was the only one on the site at the time of the fire) LOADING Man killed by metal spool in industrial accident (worker, 38, had been loading a large spool of metal onto a crate when the support for the spool came loose and fell on him – he was pronounced dead at the scene) FATAL COLLAPSE of ELECTRIC PYLON Six Dead in Electric Pylon Collapse in Brazil (six technicians died when an electrical pylon collapsed – the men were working up on the tower, which was 69 meters (226 feet) high, when the structure collapsed after one of the suspension cables broke – five of the workers were pronounced dead at the scene, while one was transported to a regional hospital, where he died a few hours later) CHEMICAL 1 injured in south Jersey chemical plant blast (a small explosion at a chemical plant forced the evacuation of employees and left one person injured just after 5:30 a.m. – a process line in a chemical production area broke and a spill occurred – there were no major injuries – three employees in the immediate area were examined and one was sent to the hospital for most tests – no word on chemical) COOLING TOWER CHEMICALS Man taken to hospital after gas accident (contractor was taken to hospital after a chemical incident at a malt whisky distillery – there had been an accidental mixing of two substances that resulted in the production of chlorine gas – incident had been at the distillery’s cooling tower, and not inside the plant – one man was taken to a Hospital for treatment suffering from breathing difficulties and was kept in overnight for observation) CHEMICAL Authorities: No danger following chemical release at Citgo (unknown amounts of butane and pentane were released into the air at the refinery – there was no danger to the public and the release was secured by 5:30 p.m.- no world on cause) CHEMICAL @ LAB 3 hospitalized after chemical exposure in Pomona (three scientists were involved in a hazmat incident at a pharmaceutical lab – a scientist was mixing potassium cyanide, a potentially dangerous chemical, in a flask form when it boiled over and splattered onto his face and mouth – two other scientists were also exposed – all were rushed to a Hospital and are in stable condition) CHEMICAL Chemical accident injures one (accident at a chemical plant injured one worker and caused officials to briefly close streets around the plant around 6 a.m. – workers were mixing chemicals in a reactor – “We don’t know exactly what happened but the lid did not shut completely or did not shut properly” – chemical fumes escaped from the reactor, overcoming one worker – four chemicals were being mixed: maleic anhydride molten low acid grade, alfa olefin 20-24, di-tert-butyl peroxide and aromatic 100 fluid) HOTWORK EXPLOSION One injured in oil field tank explosion (a worker had used a cutting torch when an oilfield storage tank exploded and left him critically hurt – the worker was removing bolts from a catwalk atop the tank – the 12,000-gallon tank contained oil sludge during the blast – the worker was wearing a safety harness as he was thrown clear during the explosion, then the safety line slammed him face-first into the hot steel – the fire burned through a strap and the man fell to the ground – he was in critical condition when transported to a hospital) Machine malfunction at Brentwood Industries sends man to hospital (a machine malfunctioned, sending one employee to the hospital at 10:25 p.m. – a material handling machine was opened allowing highly pressurized air to escape – spokesman said an employee was taken to a local hospital for observation but denied the employee was injured – a medical center spokeswoman said the worker was transferred to a Burn Center where he was listed in critical condition) FLASH FIRE Two workers hurt in Chevron Phillips refinery fire (one of the two people injured in a fire at a refinery is recovering at a hospital – the fire broke out around 8 p.m. – the two workers suffered injuries in what appeared to be a flash fire – a medical helicopter took one of them to a Hospital) CONSTRUCTION Hit-and-run leaves man injured, driver at-large (a construction worker, 32, was seriously injured following a hit-and-run – he is recovering from a broken tail bone and a fractured hip – accident happened where Hahn was inspecting a trailer that broke down – the trailer was pulled off the road with safety cones in place to warn motorists) Power restored in South Orlando after construction accident (a construction worker hit a power line causing an outage – no details on injuries or cause) Worker dies after construction accident in Mesa (a worker has died after a construction accident around 7:30 a.m. – some equipment fell on the worker, who was then treated by paramedics and transported to a hospital – worker died at the hospital) WORKZONE FATALITY KZN road worker killed on duty (a road worker died after being struck by a car in – events leading to the accident were not immediately clear) FATAL COLLAPSE Worker killed in ceiling collapse at Haltom City building (a worker, 28, was killed and two others were slightly injured when a ceiling collapsed inside a vacant building that was being remodeled – structure is 20 feet tall – about a quarter of the steel and concrete floor of a second story fell – crews lifted the heavy debris to find his body) WORKZONE FATALITY Maintenance Worker Killed on NJ Turnpike (maintenance employee, 32, was retrieving a piece of tire from the roadway when he was killed by a driver around 10:25 am. – he had stopped his orange 2012 Pickup Truck on the right shoulder with the overhead amber lights illuminated in order to pick up the debris – the men were traveling north in the right lane of travel when the car hit the worker, who was pronounced deceased at the scene at 11:03 a.m.) FALL from SCAFFOLDING Worker hurt in fall at Downtown construction site (a worker, 24, fell 40 to 50 feet from scaffolding at a construction site suffering serious injuries around 1:30 p.m.) SCISSOR LIFT Man injured in industrial accident (a worker is recovering in hospital after he was trapped between an I beam and a scissor lift at a construction site just before 9:00 a.m.) JACKHAMMER CHISEL FREAK ACCIDENT SENDS CHISEL THROUGH GYM WINDOW (a woman working out at a fitness center suffered cuts from shards of glass after a piece of construction equipment crashed through the gym’s fourth-floor window – a jack hammer chisel came crashing through the window around 9 a.m.) UNLOADING ACCIDENT Worker injured in accident at Evansville industrial facility (workers were moving large wooden objects used for construction when the man untied a chain without following safety procedures and nine of the objects fell on him – he was taken to the hospital with leg injuries) Work accident kills one at airport construction site in İstanbul (worker was found dead when the bulldozer fell into a pond at the construction site – incident took place when the worker drove a bulldozer carrying filling earth) HOTWORK Town Square blaze was accidental (fire has been ruled an accident – one building of the proposed complex was decimated by a blaze that began about 6 p.m. – losses are estimated at $15 million to $20 million – authorities think it was started by employees of a welding subcontractor cutting with a trapping him up to his chest) MISCELLANEOUS WOOD CHIPPER Man recovering after West Boca wood chipper amputation (a man lost part of his arm in a wood chipper accident – his arm was amputated at his elbow – the man has been with the company for about four and a half years – it’s not the first wood chipper accident in the area, an accident turned fatal after a man, 53, became entangled in a wood chipper in 1997 – he had been working for a Tree Service for four months when he was caught in the machine near the 17th hole of the country club’s golf course and died) CONFINED SPACE FATALITY – ENGULFMENT Wisconsin boy, 9, dies after being trapped in grain bin (a 9-year-old boy was killed after he became trapped under several feet of corn inside a grain bin on a farm at about 11:15 a.m. – the boy did not have a pulse after fire department personnel extricated the boy about an hour after the accident occurred – rescuers had to move 10 to 14 feet of corn inside the bin to reach the boy – an initial investigation showed that the auger was stuck and inoperable when the accident occurred inside the half-full bin – the boy and several family members were attempting to fix the auger when the boy became trapped inside the grain bin)
