FIRE EXTINGUISHER FATALITY
Youth killed while demonstrating safe use of fire (a man, 22,
demonstrating a fire extinguisher during a fire safety drill at a
railway workshop suffered an accident that left him brain dead – the
accident happened when a contract worker was demonstrating to the
staff in the workshop how to handle the fire extinguisher during a
drill – the faulty extinguisher has been sent to the lab to
ascertain if it had anything to with a manufacturing defect – NOTE
to Members: there are a couple of accident reports of other fire
extinguishers failing and causing death in the members area.)
FALL
2 workers rescued after falling in deep hole at south side plant
(two Water Reclamation District employees fell into a 20-30 deep
hole at a water plant requiring a rescue by the Fire Department –
they were working on settling tanks at the time of the accident –
one of the workers was listed in serious condition with head
injuries – the second worker was reported in fair condition – the
two workers fell into the hole at the plant around 7:30 a.m. – it
wasn{apos}t clear how the accident happened – it took about an hour for
firemen to get the two men out)
INTERLOCK BYPASSED
Worker suffers horrific burns after safety lock bypassed (a
worker, 24, suffered severe burns to his face, neck, left arm and
hand when he opened the industrial oven at his workplace – the oven
was low-oxygen and was used to remove paint from metal – his burns
were so serious that he had to be taken by air ambulance to hospital
where he remained on the high dependency ward for several days – it
was thought that when he opened the door to the oven the influx of
oxygen caused a flash fire from the 400 degree oven – when the
Health and Safety Executive (HSE) investigated the accident it found
that the safety device, which prevented the oven door from being
opened if it reached a temperature of more than 260 degrees, had
been deactivated)
CRANE
Crane driver hurt in Seaforth Dock accident (a dock worker, 29,
was seriously hurt after a 40ft container carrier he was driving
fell over – he received head and arm injuries in the fall and was
left trapped in the cab of his “straddle carrier”, a kind of mobile
crane – it took emergency workers half an hour to free him after the
accident – he was taken to a hospital where he was kept in
overnight)
FATAL HOTWORK EXPLOSION
Worker killed in explosion at metals recycling plant (a
contractor, 30, died in an explosion at a metals recycling company –
no other injuries were reported in the 1:45 p.m. incident – the
explosion was sparked during a welding operation – he was cutting on
a tank and it exploded – officials didn{apos}t know what was in the tank)

Peco worker injured in Pottstown power outage (an electrical
worker that was working on underground equipment was injured in a
2:45 p.m. incident that cut power to 602 customers in the area and
caused an undetermined amount of underground damage – worker was
taken to a hospital)
Pay loader strikes paper plant worker (a pay loader ran over the
midsection of a worker, 40, at a paper plant – he was in serious
condition in the intensive care unit – he was struck at about 1:20
p.m. – the driver of the 12-ton vehicle didn{apos}t see the worker, who
was sorting paper and cardboard – he was conscious when rescuers
arrived)

Two dead in Indonesia mine accident, U.S. co says (two miners
were killed in flooding in an underground mine – the incident
occurred in the DOZ (deep ore zone) mine where wet material flowed
unexpectedly and killed two workers)
GAS DRILLING
Gas company says worker jumped up and hit head, wasn{apos}t hit by
pressure hose at W. Pa. site (an official with a gas drilling
company is clarifying reports by emergency dispatchers about a man
hurt at a gas well – a contract worker wasn{apos}t hit in the face with a
high-pressure hose as was reported – the worker reacted when
pressure was released from a well head and jumped up, hitting his
head on a piece of equipment – he was taken to a nearby Hospital and
is expected back at work this week)

Worker{apos}s arm severed at Ririe potato plant (a worker, 44,
remained hospitalized after his arm was severed during an accident
at a potato plant – he was in fair condition at a Medical Center –
he was hurt after one of his arms got caught in an auger at the
plant – his arm was severed above the elbow)
FATAL FALL
Rig worker dies on old platform (an investigation is continuing
into a fatal accident on a natural gas platform – worker, 27, died
after he fell through a deck opening – the incident happened at a
platform that had not been operational since 2008 – he was was
assisting in abandonment operations)
MINE FATALITY
Labrador mine worker killed (a worker at an iron ore mine died
on the job – he was a contracted employee working on a power line at
the mine – it was not immediately known what caused his death –
special equipment was needed to retrieve the body from a high
elevation)
Man died of head injuries at Caterpillar (a worker, 34, was
crushed by the machine he was servicing at a facility – he died of
head injuries – he was pronounced dead at a Medical Center about 40
minutes after being crushed by the machine – it isn{apos}t clear exactly
what happened to cause the machine to crush the worker)

dead on scaffolding at Motiva (authorities are investigating the
death of a contract worker at a Refinery – the man{apos}s body was found
on scaffolding shortly after 6 p.m. – there are no obvious signs the
worker was involved in an accident or was injured at the plant)
Workplace accident kills man (a worker, 65, was dismantling a
large quonset structure along with other workers at a Dam when he
was struck in the head by a metal crossbar – on-site emergency
medical responders attempted to revive him but he died of his
injuries)
HOT WATER BURNS
Worker suffers burns to his feet at sugar refinery (an
investigation is under way into determining how an employee, 23, at
a sugar plant suffered second-degree burns to his feet after being
on the job only two days – he stepped into a sump containing boiling
water while being shown the work site – the sump is a component used
in the process of extracting sugar from sugar beets – even though he
was wearing protective rubber boots, the liquid was so hot that he
was burned during the 5:39 a.m. incident)

Worker Escapes Injury In Tulsa County Oil Tank Fire
(firefighters worked to get an oil tank fire under control – no one
was injured in the fire – one worker was on the scene at the time of
the explosion – the man had just stepped away when the explosion
happened and was not injured – the man was using a portable device
to mix up the oil in the tanks when one of the hoses to the tanks
overheated and exploded)

Mill explosion injures one (a worker was hospitalized after an
explosion at a mill shortly before 5 a.m. – the fire was out when
firefighters arrived, but one worker was injured and needed to be

employee was treated after an accident in the kitchen – the worker
was spraying oven cleaner and got some in his eyes – he was taken to
the Health Sciences Centre where his eyes were flushed)

condition after an explosion – he was cutting a 55 gallon drum when
the drum exploded – he received head and facial injuries and was
rushed to a Medical Center – there was some oil left in the drum and
possibly several other substances, which caused the explosion)
Rozzi Worker Hurt in Fireworks Mishap Remains Hospitalized (a
fireworks employee, hurt in a smokestack malfunction at Great
American Ballpark remains hospitalized in critical condition – the
fireworks technician suffered burns to his face when the stacks
misfired in the sixth inning of the game and is still undergoing
treatment)
