INDUSTRIAL FATAL FALL Worker killed in elevator shaft fall at cement plant identified (worker was killed when he fell down an elevator shaft at a cement plant – he opened the door to the elevator, but there was no elevator car and he somehow stepped in and fell four stories) GUARDING AMPUTATION Blackburn bakery worker loses fingertips in pasty-making machine accident (worker, 35, lost part of his fingers after a pasty-making machine modified to speed up production cut them off – he was feeding cheese and onion mixture into the top of the machine when his right hand was struck by pistons inside – investigation found part of a metal guard had been moved to allow workers to add fillings to the machine while it was still operating, negating the need to power down the machine) ELECTRICAL Workers shocked in construction accident (one worker was working with a construction concrete boom laying down cement when that boom touched a high intensity electrical wire – he was jolted, knocked unconscious to the ground, and that’s when a second worker went to try and help him – coworker saw him go to the ground and ran to him, and when he touched him, he also was shocked – ended up having two people unconscious, one obviously that was not breathing – a police officer arrived on the scene and called 911 – he put the phone down and gave c.p.r. – he was able to breathe life back into the man – both men are in the hospital) STEEL MILL FATALITY Liquid steel spill kills worker at Arkansas plant (an estimated 38 tons of liquid steel spilled and killed a worker, 49 at a steel plant – he was killed after a ladle of liquid steel apparently spilled while being moved in the facility’s melt shop – a cable on the crane that was moving the kettle of liquid steel broke, causing the kettle “to fall and tip” over, spilling the liquid, which caused the workers’ death – the melt shop spilled an estimated 38 tons of liquid steel) AMPUTATIONS Report: Ultra-Poly worker loses 3 fingers in accident near Portland (a worker accidentally cut off three of his fingers – accident was reported 6:30 p.m. at the plant – the worker, a man in his 20s, was conscious and alert, and had not lost much blood – his severed fingers were packed in ice in the hope they might be re-attached) HOT HYDRAULIC OIL Worker suffers burns at Ford Stamping Plant (OSHA is investigating after an employee at a Stamping Plant suffered burns in a workplace accident – initial reports described the incident as “an explosion” but OSHA says it was an unexpected release of hot hydraulic oil – not an explosion – one worker suffered burns as a result of the accident) OIL FIELD FATALITY Oilfield worker dies after accident (worker, 19, succumbed to his injuries at a hospital – he and another worker were pumping crude from a vacuum truck to a tank at a site when a hatch blew open, hitting him in the chest and face) ARC FLASH Worker burned after electrical “arcing” accident (a worker contracted to do electrical work at a plant was burned after a wire arced in front of him – the worker was burned in his face and arm and was taken to a Hospital, but was awake and talking while firefighters were taking care of him at the scene) FORKLIFT FATALITY Aerospace worker killed in El Cajon industrial accident (an employee of an aerospace firm was killed in an industrial accident – a security guard discovered the victim at about 7:15 p.m. – the man had been pinned between a forklift and a large piece of equipment – the engine of the forklift was running, it was in gear and the parking brake had not been set – it appeared the worker got off the forklift and it then backed over him) MINE FATALITY PotashCorp worker killed in mine near Saskatoon, facility shuts down (a worker, 31, has been killed at a mine – some ore fell on top of the man while he was underground – co-workers tried to resuscitate him and brought him to the surface – another person was also taken to hospital, but was not seriously injured – mine has temporarily shut down and workplace safety officials investigate – it’s not clear what caused the accident) GUARDING/LOTO on MULCH GRINDER Worker hurt after being caught in mulch grinder at landscaping company (a worker, in his 40s, is hospitalized after ending up in a mulch grinding machine at a landscaping company – the man was working when he somehow became entangled in a mulch grinder – workers were able to turn off the machine and untangle the man from the machinery, but firefighters had to use a basket to retrieve him from the grinder’s hopper, which was several feet deep – he was transported to the hospital, but officials said his injuries were non-life-threatening) EXPLOSION Explosion of hot gas at Utah plant sends 2 workers to hospital with serious injuries (an explosion of hot gas at a plant sent two workers to the hospital with serious injuries – the workers were taken to a Burn Center after the accident – a supervisor in his 40s and an employee in his 30s suffered “thermal burns” to their upper torsos – a sudden release of hot gas occurred as the employees worked to bring a piece of equipment back on line after several days of maintenance work) EXPLOSION One employee severely burned in explosion at Kilgore Flares plant in Toone (the explosion left an employee in critical condition – he was flown to a Regional Medical Center due to severe burns – only the one employee was injured – plant makes flares for the military – in September 2010, three people were severely burned in a fire at the plant) ARC FLASH Shock seriously burns Qld worker (a worker is being treated for serious burns after suffering an electric shock at a substation – he is being treated for serious electrical burns – he was one of three injured at the substation at about 1.30 pm – the other workers have since been released from hospital – the trio were burned on the head and hands) Flash fire causes scare at Upstate plant (at 7:30 a.m. the fire was out and no one was injured – the glass tech machine that shoots steel shot, or balls, across the fabricated steel, caught fire, causing minimal damage) HOTWORK Firefighters Contain Large Fire Near Phoenix (firefighters have contained a large fire burning at a plumbing supply and welding company – fire started shortly before noon – two employees were welding when the fire broke out – one of them suffered minor burns – the fire consumed three structures in about 20 minutes – there were several explosions during the fire) AMMONIA Roof collapse causes chemical leak (a section of roof collapsed at a distribution center triggering a chemical leak with six employees treated for ammonia exposure – at about 11:08 a.m. firefighters, police responded to a report of a roof collapse and anhydrous ammonia leak) CHLORINE CYLINDER 4 people treated following chlorine spill in Smithfield (a total of four people were treated at a hospital following a chlorine spill – a 150 pound chlorine cylinder ruptured just after noon – crews transported one person to the hospital due to difficulty breathing and vomiting – three other people were being treated at a local hospital) FORKLIFT Worker hurt in forklift mishap at City of Hope in Duarte (a man may lose his arm after it was crushed by a forklift in an industrial accident in a parking lot shortly after 8 a.m. – it initially appeared the man, who was part of a crew contracted to perform construction work at the hospital, was helping unload a forklift from a truck – his left arm became pinned between the forklift and a hydraulic leg used to stabilize the piece of construction equipment – the man was freed at 8:48 a.m. – his injuries were not believed to be life-threatening, though it was not clear if he would lose his arm as a result of the mishap) CONSTRUCTION FATAL FALL Worker hospitalized after fall off north NJ bridge (a construction worker was hospitalized after he fell off a bridge around 9:30 a.m. as workers were doing preparatory work for a major rehabilitation project – crews were installing temporary shielding meant to catch falling tools or bridge debris – the worker fell about 20 feet and landed on a ramp leading to the skyway, which was closed at the time – it was not immediately clear what caused the accident) FALLING PLYWOOD FATALITY Construction worker dies in accident at UK campus (a construction worker, 24, died after he was struck by a piece of falling plywood while work was being done on a new student dorm – wind gusts caused the piece of wood to slip from a workers hands then fall five stories before striking him) WORKZONE Road Worker Injured in Morning Accident (a Public Works road crew worker was injured directing traffic in a morning accident – the city employee was guiding cars around 8:30 a.m., in an area where crews were working to repair potholes and clean the street – a car that was stopped was rear ended by the car behind it – the first car was forced into intersection and collided with the worker, who rolled up and over the hood of the car, across the median and was pinned under his own work truck – he broke his pelvis and was taken to a Medical Center) WORKZONE FATALITY Roadway Accident Kills VDOT Worker from Vienna (a Department of Transportation (VDOT) contract worker had pulled his truck off to the right shoulder and was standing at the rear of the truck near the spreader when he was struck by another DOT dump truck – rescue personnel transported the contract worker, 32, to a Hospital where he died) ELECTROCUTION Construction Firm Owner, Worker Die After Truck Hits Electrical Wires (owner, 55, of a construction company and one of his workers, 22, were electrocuted after a dump truck’s bed hit overhead electrical wires – both received electrical shocks after the truck hit the wires on the company’s property about 11 a.m. – it appeared that the worker was injured after he raised the dump bed and it touched the wires – the owner apparently was shocked later when he went to his aid – one of the men got out of the energized truck, and when he did, he touched the vehicle and was shocked – the second person, who apparently was trying to assist the first one, also touched the truck and was shocked – they also suffered electrical burns – worker had been cleaning out the dump bed when the accident happened – he was preparing the truck for a trip to Maine to pick up road salt) UPDATE Second worker faces murder charge in Philly building collapse (a second demolition worker has been charged with murder in the deaths of six people killed when a brick wall being taken down collapsed onto an adjacent thrift store last summer – a judge upgraded charges against the backhoe operator to six counts of third-degree murder after hearing new evidence at a preliminary hearing – a city inspector testified that the driver had previously been warned at another job site not to use heavy equipment near an unstable wall, especially with an occupied building nearby – testimony about the previous warning, four months before the collapse, shows recklessness and warrants the third-degree murder charges against the operator) Miscellaneous AIRPORT TUG FATALITY Baggage tractor driver dies in LAX airfield accident (a worker, 51, was killed at an International Airport when he lost control of a baggage tractor on an airport service road – he may have been ejected from the tug and then run over by it – accident happened at about 5 a.m. – the driver of the ground service equipment vehicle either had a medical issue or somehow lost control of the vehicle – the tug came to a rest near an airfield fire station, and fire personnel immediately responded – they found him unconscious, and paramedics couldn’t revive him) CRUSHED HAND Dock worker ‘crushed’ at Fremantle Port (a worker was injured when his hand crushed in an incident on a container ship – paramedics assisted a man who had been “crushed’ somehow but was in a stable condition and had been taken to hospital) CO POISONING FATALITY New Yorker Dies From Carbon Monoxide Fumes (a mall worker is dead after he is overcome by carbon monoxide fumes – the gas leak happened inside a strip mall – the man who died was a manager of a seafood restaurant – twenty-seven other people, including three first responders, were also treated for carbon monoxide poisoning – it appears the leak started in the mall’s heating system) CHLORINE from REACTION Chlorine leak forces Park Hyatt hotel evacuation at The Rocks (a hotel was evacuated and six people were taken to hospital when chemical fumes spread through the air conditioning system at 9.30am after someone incorrectly mixed two chemicals in the pool area causing a pungent smell of chlorine to spread through the building – about 100 guests and staff were evacuated from the building and six people were taken to hospital after being overcome by the fumes) |
