INDUSTRIAL HYDROCHLORIC ACID Explosion at 3V Chemical injures one worker (a worker was transported to the hospital after being severely injured during an explosion – according to reports from the scene, the man was blown three or four floors down an elevator shaft as a result of the explosion – he also had rebar pierce his leg – rescue workers freed the man from the shaft and placed him on a backboard – he was transported to the hospital where he was decontaminated and was expected to then be transported to a trauma center) GUARDING and AMPUTATION Worker loses fingers in accident, company to pays fine (worker lost four of his fingers in an accident at the plant – he had lifted a loading hopper from the machine, when he accidentally stumbled resulting his hand to get caught between two rollers amputating four fingers from his right hand and also severely breaking his right thumb – the damaged hand requires screws and plates to be inserted and “degloving” the hand to his wrist) CRANE FATALITY Ship Crane Accident Kills Port worker (a man died and another was seriously injured after a container fell on them while passing under a ship docked at the Port – the fatal accident was caused by the ship crane which is usually used for loading of containers broke hence releasing the container in midair – a port official confirmed to us both men were port workers) MINE FATALITY Worker dies at Harmony mine (a mine worker was killed in an accident – the employee was fatally injured after a fall of ground accident at the mine) RAILCAR Riceland Foods worker hurt in industrial accident (a worker was critically injured after he was run over by a railcar – the worker was walking on train tracks that run through the plant property when the worker slipped on the tracks and was accidentally run over by a railcar that was driven by an employee – worker was taken to a Medical Center in critical condition, but police say the man was alert and able to call 911 for help after the accident) CONFINED SPACE Workers rescued from tank at dairy farm in La Trobe Valley (at least nine people, including emergency workers, are believed to be injured following an industrial accident at a dairy farm – one person is in a critical condition – emergency crews were called to the dairy farm shortly after 1pm and it is understood workers on the farm were trapped inside a water tank before being rescued – it is believed they became trapped in the tank and suffered a reaction to its contents – emergency workers trying to rescue them were also overcome) GUARDING/LOTO Worker’s arm trapped in factory accident (worker, 48, at a tyre factory had his arm caught in a rolling machine and was flown to a hospital – he had his whole right arm stuck in the machine – the 45 minutes it took to release his full right arm from the rolling machine the ambulance paramedics and CareFlight doctor worked to stabilize him) TRUCK LOADING FATALITY Edmonton worker killed by rolling pipe (a worker, 37, was killed when he was struck by a pipe that rolled off a truck – a pipe rolled off the deck of a flatbed trailer as workers were adjusting a load) EXPLOSION Leduc Industrial Park Explosion Severely Burns 2 Workers (two workers have been severely burned in an explosion at an industrial park at an industrial services and hazardous waste business – there was a fire which caused the explosion – the two workers were in stable condition in a hospital) HOTWORK EXPLOSION @ GAS FIELD Wyoming explosion in gas field leaves 5 injured, with one fighting for life (an explosion has left one man fighting for his life and four others injured – the gas field explosion happened about 10:30 a.m. – officials don’t know what triggered the blast – one worker was in critical condition after the blast – another worker is in serious condition – two other workers were taken to area hospitals and listed in stable condition – one of the injured was treated and released – officials report that there was some welding going on at the time of the blast, but it is not known if this is what caused the large explosion in the gas field) FATAL FALL from POLE Worker dies after plunge from pole (workman, 63, fell to his death from an electricity pole – accident happened as the man was clearing tree branches from around power lines – after he had done one pole he moved his ladder to another and, after climbing up, he fell – it looks like he might have accidentally cut through his harness) TANK EXPLOSION Two Workers Injured at Mosaic Plant Explosion (two workers were injured after an explosion was reported at the processing plant – fire officials report a hundred-foot cloud rocked the plant – workers were doing routine maintenance of an empty acid tank which ruptured, causing the top to fly about 100 feet and crash down on the ground – two employees sustained injuries that were non-life threatening and were transported to an area hospital – one worker who had been standing on the structure was struck in the chest by shrapnel – the other injured worker was on the ground) FATAL EXPLOSION @ REFINERY 2 killed in blast at Belgian refinery (an explosion tore through an oil refinery killing two people and partially closing down operations -local media accounts said they were contract workers that had been called to the refinery to fix a steam turbine leak – after the blast, the refinery was evacuated and partially shut down) AMMONIA Ammonia leak contained at refrigerated warehouse in Fairmont City (authorities evacuated an industrial area in the northwest portion of the community after a police officer on patrol about 5 a.m. discovered ammonia venting from a leak at a refrigerated warehouse – no injuries were reported and no residential areas were affected – hazardous materials team found a leaking gasket on a refrigeration compressor and shut down the system) CHLORINE Chlorine spill at Fonterra factory (leak happened just before midday at the milk factory – it appears that the incident was caused by chlorine coming out of cleaning tanks from a leaking valve or pipe and spilling on the floor – area was evacuated and the people closest taken to hospital as a precaution – six of the seven people taken to hospital have since been discharged, and the remaining person is expected to be released later in the day) FORKLIFT Hillside man injured when Kearny forklift operator accidentally runs him over (worker, 49, was injured at a warehouse after a forklift driver accidentally ran over his legs on a loading dock – he was reportedly in fair condition with non-life threatening injuries – including two broken legs and a cut going from his thigh to his ankle – following the 9 a.m. accident – the accident occurred when the unidentified 36-year-old forklift operator was heading toward the loading dock and struck the worker) CONSTRUCTION EXCAVATOR on BARGE FATALITY Delta barge worker dies in accident (a construction worker, 49, was killed after a piece of equipment he was working on fell into the delta – he was operating an excavator from a barge around 1 p.m. when the machinery fell into the water – he was trapped inside the submerged excavator and was pronounced dead at the scene) WORKZONE FATALITY Construction worker killed on Selmon Expressway (a construction worker died after he was hit by a dump truck – he was working in a construction area of the expressway at about 4:44 a.m. when he was struck – he was taken to a hospital, where he later died) TRUCK FATALITY Construction Worker Killed In Nevada (a man was killed at a construction site – he was run over by a water truck and pronounced dead at the scene) SCAFFOLDING COLLAPSE 3 workers injured in BGC construction site accident (a structure that was supposed to ensure safety at a construction site instead sent three workers to the hospital – a heavy meshwork of steel and wire designed to protect pedestrians from falling debris gave way at the hotel worksite around 1:30 p.m. – three men suffered injuries mostly on their arms and legs and were brought to a nearby Medical Center – four other workers were then under the structure but were able to run away as it gradually fell to the ground) MANLIFT and ELECTRICAL Worker burned by power line in Terre Haute (construction workers hit a power line with a boom lift – one man was taken to a hospital with burns after the accident – two men were in the lift when it hit a high-tension line in an alley along a building where they were working on the roof – the other man in the lift wasn’t hurt) UPDATE now a FATAL FALL Worker died in fall from Fort Mill school roof (worker, 28, fell from the roof of a school has died several days after being injured – he died three days after he fell from the roof – he worked for a drywall company – he was working on the roof around 7:30 a.m. when he fell to a lower portion of the roof, then to the ground) TRENCH FATALITY Man killed when trench collapses (workers were working in a six-foot trench and installing an electrical conduit when a wall on the trench collapsed and buried a worker, 35, – by the time he was uncovered by emergency personnel, he had passed away due to the injuries sustained in the collapse) SCAFFOLDING COLLAPSE Two men taken to hospital after scaffolding accident in Northampton (two men were taken to hospital after scaffolding collapsed – the men were both at the top of the scaffolding and fell from a height of up to 15 metres – one patient, a 55-year old man, sustained serious chest injuries as well as a potential head injury – the second patient, 42, sustained injuries to his lower back) Miscellaneous GUARDING FATALITY w/ PTO on TRACTOR Oakley Horse Breeder Killed in Ranch Accident (a horse breeder, 71, was killed while digging post holes near his home – he died as a result of injuries caused by a PTO-driven auger on a farm tractor after he became entangled in the auger and died at the scene) CONFINED SPACE FATALITY in MANURE PIT Farm worker dies in manure pit (a man, 60, was found unresponsive and was pronounced dead at the scene just after 11:30 a.m. – a worker at the farm had been found unresponsive in a manure pit by a co-worker) ELECTRICAL Worker’s pulse stops after being shocked at Goldman Sachs in Bridgewater (a worker, 49, shocked while on a maintenance job was revived by police and then sent to a burn unit for his injuries – he received an electrical shock while doing maintenance work on two spike protection devices, called snubbers, on ground transformers – power had been transferred from the primary snubber to the alternate snubber so the primary could be cleaned – the job foreman, left the area for a moment when power to the facility was lost, but when he came back to the transformers, he saw the worker to the ground and stop breathing) FORKLIFT Officials investigating forklift accident in Thurston County (officials are investigating a forklift accident where a woman is believed to have sustained injuries – no other information was immediately available) |
