INDUSTRIAL BOILER ACCIDENT 2 workers injured in Pa. brewery boiler mishap (production at a Brewery has been halted after a boiler mishap left a pair of workers with minor injuries – the accident happened just after 12 p.m. at the facility and emergency services responded to the scene – one worker in the boiler room was hit with either smoke or steam and that it had agitated his eyes while another worker was hit on the head – neither was taken to a hospital – the brewery’s 100 or so workers were evacuated to the parking lot and production has been halted while officials investigate the cause of the incident) AIR HOSE FATALITY Man killed while clearing Milford road slip (a worker, 65, died following an industrial accident near a Tunnel while he was clearing a slip at the time – at about 12.50pm a worker for a contract company was killed while clearing a slip – the man sustained a “very serious head injury” following an incident with a compressed air hose – he was treated at the scene by paramedics who had flown to his aid in a rescue helicopter – he was unable to be revived – two other workers also received minor injuries in the incident and were treated at the scene) EXPLOSION Worker injured in explosion at East Providence plant (a worker was taken to the hospital after he was injured in an explosion at a print works at about 3 p.m. – a glass flask exploded, injuring the worker – the worker received cuts to his hand, abdomen and head) CRANE FATALITY Donjon Shipyard Accident Results in Fatality (a worker, 48, at a Shipbuilding & Repair yard was killed in a shipbuilding accident – the man became entangled in machinery while working on a crane at the shipyard – he was suspended 100 feet in the air at the time of the incident; rescue crews had to use a fire department ladder truck to retrieve the body – he died of trauma injuries) UPDATE on GRINDING WHEEL EXPLODES Employee left with 83 staples in head after grinding wheel accident (an employee was left with 83 staples in his head following an accident with a grinding wheel – he suffered a fractured skull and facial injuries after the accident at the factory – he began his job as a fettler in August 2010 – he received between three and five hours of basic training when he started work and was supervised by an experienced employee to begin with – he was not given training or information on how to change the grinding wheels needed to carry out his job – company took the decision not to send him on an external training course until it had been decided whether he would stay on at the factory – on the day of the accident, he was using a grinding wheel he had changed himself the previous day – the wheel fractured, flew towards his head, smashing his visor) ELEVATOR FATALITY Man killed in grain elevator accident ‘loved to work’ (worker, 44, died shortly after 4:30 p.m. after being injured while servicing an exterior elevator – another worker, 25, was moderately injured in the incident – the two men were working on the elevator shaft when a cable snapped and the elevator fell several feet, injuring the worker – cause of the accident is still undetermined) CAUGHT BETWEEN Worker Hurt In Salina Industrial Accident (a man was pinned between some equipment and that an employee suffered a leg injury – the exact circumstances as to what happened are still under investigation – the employee is expected to make a full recovery) LOTO FATALITY Worker dies in accident at Yadkinville pita and bagel chips factory (a worker, 42, was killed when he became trapped in machinery at about 7 a.m. at a food-processing plant – preliminary reports indicate that the worker was trying to free a pan that had become stuck – the worker crawled under a conveyor belt and got caught in machinery – the worker was an employee of a temporary staffing agency) LINE BREAK/PROCESS OPENING Workers reportedly OK following chemical exposure at BASF (two contractor are reportedly OK after they were exposed to a chemical while working on a pipe – they are reported to be OK and were taken to a Hospital as a precaution – according to a preliminary report, the individuals were taken to a decontamination shower – the accident happened when workers were connecting a pipe line – “When they broke into the line there was some material flushed out and they were exposed to it. At the time we weren’t expecting there to be something in the pipe. It may have been a clean-out material that had been there from a previous process,” said the plant manager – a sample of the material found in the pipe was analyzed and tests revealed the substance was “95 percent water with a small amount of product.” “The way we respond to it when we’re not 100 percent sure (the type of material) is we initially assume the worst and that’s why we treated them the way we treated them and we sent them out to the hospital. I think that all works to the best interests of the people involved,” he said) AMPUTATION Meat worker’s finger reattached (a worker, 38, a smoker since he was 11 – has vowed to stay off the cigarettes to honour a large medical team at the Hospital who reattached his finger after a work accident earlier this month – he was using a hock cutter at the meatplant when he sliced through two fingers – he arrived at the hospital with the left little and ring fingers hanging on by a bit of skin at the second joint – doctors wanted to operate to reattach the fingers, the worker wanted them off completely so he could have a cigarette – after 20 hours of surgery by a team of about 20 and “amazing” care afterward, he was adamant his smoking days were over) MOLTEN METAL BURNS Liquid metal burns Springfield industrial worker (a man suffered burns while pouring liquid metal into a mold – he was transported to a Hospital aboard CareFlight, but his condition is not currently known – the man was pouring liquid metal into a mold around 12:15 a.m. when some of the metal began pouring into the victim’s boot – the metal reportedly burnt the man’s foot) UPDATE on MOBILE EQUIPMENT FATALITY Harlesden worker crushed to death at one of Europe’s biggest recycling firms (worker, 25, was killed instantly at a recycling plant when a digger driven by an unauthorized driver ploughed into him – he was killed almost instantly when the 35-ton loading shovel hit him before crashing into the beams holding up an overhead conveyer belt – the driver of the shovel was inexperienced at the helm and was unsupervised despite not being fully trained to handle the machine) FORKLIFT FATALITY Worker fatally crushed by forklift (a worker, 35, has died after a forklift accident at a tire shop – he was fuelling up a forklift around 1:30 p.m. when he became fatally pinned between the forklift and a nearby shed – he was taken to hospital, where he was pronounced dead) UPDATE on FATAL EXPLOSION 2nd Worker Injured in Oklahoma Refinery Blast Dies (a second man who was injured in an explosion at an oil refinery has died – he was injured and another worker was killed in the Sept. 28 explosion of a boiler at the refinery – officials have said the explosion occurred as the boiler was being restarted following scheduled maintenance and upkeep) HOTWORK EXPLOSION Man critical after tank battery explosion (a tank battery explosion severely injured a man, 51 – he was burned around 9 a.m. when the tank battery he was welding on exploded – they were welding on a tank battery that they thought was dead – they were welding on it the day before – they thought it might have had a little residue that got stirred up and when he hit it with the torch it ignited – he suffered thrid degree burns to over 80 percent of his body – another employee nearby at the time received burns on his hands – the other employee was treated and released at a Medical Center) EXPLOSION Fire at Vale Inco plant (an explosion at the plant has sent two workers to a hospital – the workers are being treated after inhaling chemicals – 20 other workers were being assessed at the scene by paramedics – the explosion and resulting fire were being fueled by sulphur inside the refinery) LOTO/GUARDING FATALITY Employee Dies In Accident At Seaboard In Guymon (first responders were called to the scene at about 2:30 p.m. – an employee in the plant fell onto a shaft in the rendering department and died) CHEMICAL Chemical accident at German Kraft plant sparks mass evacuation (an emergency alert has been issued by authorities after a chemical leak at a food factory – employees accidentally poured acid into a tank containing sodium hydroxide solution causing a chemical reaction – this created nitrous gases that are considered dangerous if they come into contact with the human body – more than 1,400 people have been evacuated from the area – nobody has been injured – hundreds of fire fighters are working to cool the container and avert an explosion) CHEMICAL Chemical Leak Sends Del Rey Workers To Hospital (more than a dozen workers were treated after inhaling fumes at a juice and processing plant just after 8:30 a.m. – fifteen people were treated and at least six of those were taken to the hospital – it appears muratic acid and chlorine were accidentally mixed together causing the fumes – firefighters are trying to determine why it happened) AMMONIA Ammonia leak triggers bottling plant evacuation (an ammonia leak forced the evacuation of a bottling plant, but there were no injuries, and the fumes were contained to the building – about 100 workers fled the plant – the leak was reported at 7:14 a.m. – no workers were exposed to the fumes and the plant is surrounded by open desert with no nearby businesses or homes) CONSTRUCTION TRENCH FATALITY Hit by Rockslide, Construction Worker Dies (a construction worker, 26, died after an avalanche of dirt and rocks fell on him – the accident happened when four workers were welding an iron foundation under a water tank dig site around 11 AM – the victim with three other workers was welding a foundation down below in the depth of 6-8 meters – in the middle of their work, the right wall collapsed – the other three workers were able to escape, but the victim got stuck on a piece of iron until dirt and rocks fell on him) Man Hurt In Accident At Old Delphi Site In Limestone County (a man was hurt in an industrial-type accident at an old plant site about 10 a.m. – a beam or pipe fell on a man and hit him in the neck – the man wasn’t seriously hurt) FALL in ELEVATOR SHAFT Worker plunges 25 feet into elevator shaft (a construction worker, 46, was injured after falling 25 feet down an elevator shaft at 8:43 a.m. – he had plunged through an opening in the second-story floor – paramedics treated the patient for his injuries, and he was “prepared for removal from the shaft – the employee is stable and in intensive care) TRENCH and CRANE Two Workers Taken to Hospital After Construction Accident (two town workers were transported to the hospital after a construction accident – the workers were in a hole installing new storm drainage when a large catch basin fell from above – at the time of the accident, the catch basis was being lowered in to the hole for installation – the two workers were bumped into when the basin fell into the hole – their injuries didn’t appear to be serious) WORKZONE Construction Worker Hit by Semi (State Police are investigating a construction zone accident that injured a worker, 56, around 9:10 a.m. – he was struck by a semi traveling through the area – he was flown to a Hospital for treatment of his injuries – charges are pending) TRENCH & EXCAVATOR FATALITY Contractor dies after industrial accident in Mississauga (a worker, 56, has died following an industrial accident – he was doing excavation work on a residential property and became trapped after he somehow became pinned down by an excavator – rescue crews tried to remove mud and stones from the surrounding area, so that heavy machinery could be brought in to help free him – a tow truck was eventually brought in and was used to lift the large digger off the worker – the entire recovery effort took around five hours) EXCAVATION Gas leak triggers fire in Mexico, 2,000 evacuated (a leaking gas pipeline exploded, forcing the evacuation of 2000 people, and injuring at least three, as crews battled to put out the blaze – the pipe was ruptured by construction crews working in the area – the accident injured two firefighters and an employee of the state-owned energy company) CRANE Fallen scoreboard pins worker at Phoenix park (a worker was crushed while attempting to install a scoreboard at a park – two workers were using a crane to put a scoreboard into place when the scoreboard fell on top of the workers, trapping one of them underneath – fire crews were able to get the victim out, but he said he didn’t feel anything in his legs – the worker was transported to a trauma hospital in critical condition – the other worker suffered minor injuries and did not need to be transported) HOTWORK Fire damages Uptown Kingston commercial building (fire damaged a building housing offices and a jewelry store – the fire is believed to have been started accidentally as a worker was using a torch in a nearby parking lot and it is believed that started the fire that started in the rear of the first floor and worked its way to the second floor – firefighters were able to bring it under control in about 20 minutes – there were no injuries) MISCELLANEOUS ELECTRICAL Port Authority worker injured after touching high-voltage line (a rail worker was injured after coming in contact with a high-voltage line – the employee had been working on a T light-rail car at the garage – accident occurred between 9 and 10 a.m.) FATAL FALL Two cleaning workers fall to death in Capital (two workers plunged to death after the suspended platform from where they were cleaning the windows of a tower crashed to the ground – the accident happened as the duo were cleaning the windows on the 15th floor – the cradle crashed just around 3.20pm. – both the workers died on the spot with the platform completely smashed – their bodies were cut into pieces by platform metals and due to the impact of the crash) AMPUTATION Worker installing lights loses leg in accident (a worker was critically injured as he was stringing cable over a street for holiday lights – the worker was stringing cable at 12:40 a.m. and one cable was dangling over the roadway when it became entangled on a passing Transit System bus – the worker’s leg then became tangled in the cable and one of his legs was severed) ZOO Elephant crushes Sydney zoo worker (a zoo keeper was pinned by the elephant during a routine training session about 11.30am – two other keepers heard her call out, moved the elephant away and immediately called an ambulance – it took five minutes for paramedics to get the keeper’s heart beating after she suffered a serious chest injury from being crushed – she was taken to a Hospital in a critical condition and was accompanied by senior colleagues – it was so far unclear why the animal had set upon the keeper) |
