INDUSTRIAL LOTO/GUARDING FATALITY Worker killed at Pilgrim’s Pride plant in Canton (authorities are investigating the death of a worker, 37, at a chicken plant – he apparently was trying to dislodge a piece of debris in some equipment when he was killed around 2 p.m.) LOTO/GUARDING FATALITY Worker Dies at Quarry (a worker, 70, has died while working at a quarry around 1:30 – he died after being trapped in a rock crusher – a rock crusher is used to crush rocks into smaller pieces, usually for gravel or some other road or building application) FORKLIFT FATALITY Worker dies in forklift accident at Nebraska mine (a worker, 52, has died in a forklift accident at a mine around 10:15 a.m. – he lost control of the forklift while hauling a large, empty trash bin when it fell off and the forklift kept going – when the forklift hit a concrete base, the forklift rolled on its left side and pinned the worker – he was pronounced dead at the scene) CRANE FATALITY Worker dies in crane accident at Port of Oakland (a worker suffered fatal injuries at the Port after he was crushed while inspecting a crane – the man was with another worker inspecting a crane when he was crushed by a moving portion of the crane – the other man was not injured – accident happened around 9:30 a.m. – the worker was taken to a Medical Center where he later died – the worker was crushed in the chest and suffered a head injury) CRANE FATALITY 1 Dead From Fallen Pipes in Port of Tampa Ship Accident (a worker, 56, was killed after pipes fell onto him and three other men working inside a ship docked at the port – he was pronounced dead on the scene – only one of the three other laborers involved had minor injuries – they were at the bottom of the ship’s cargo hold bundling steel pipes for a crane to lift them when a strap on the bundle broke, sending the pipes crashing down on them) CYANIDE Two treated for cyanide poisoning after accident (two workers have been treated for suspected cyanide poisoning after an incident – one of the men reportedly smashed a bottle containing the noxious substance used in pest control, as he tried to decant it – two ambulance workers called to assist the workers were also treated as a precaution) FATAL FALL from GRAIN ELEVATOR Man killed in grain elevator accident in Buffalo Lake (worker, 28, was killed after falling 60 feet from a grain elevator he was working on at 10:48 a.m. – he was not an employee of the grain elevator, but instead was a contract worker there on an expansion project for the elevator) FATAL FIRE Two oil field workers’ deaths await autopsy results (both men died at the scene, one from burns sustained at an oil well and the other from unknown and possible health reasons – worker, 60, died at an oil well site from burns sustained while repairing a leak at the well – workers told the investigating deputy that he went to retrieve more tools and when he didn’t return, they searched, saw smoke and fire near the tank batteries and found him already engulfed in flames – the co-workers used a fire extinguisher and summoned emergency response – all three workers were covered in oil) LINEMAN ELECTROCUTED Lineman electrocuted in Southland (worker, 48, died after he was electrocuted while working on a power line – he went with a colleague about 9.30am to repair a power line on a when he was electrocuted) HOTWORK FIRE – WELDING ON METAL WALLS Welding leads to fire (at approximately 3:30 p.m. firefighters arrived to find a metal storage shed with large amounts of smoke rising from inside – the fire was a result of the owner welding on the side of the shed – the person was trying to weld some storage brackets on the side of their metal storage shed and some of the contents had caught fire – nobody was inside of the shed at the time – “The heat from the welder that he was using to weld the metal brackets to the metal storage building transferred into the storage building itself. We’re not sure whether he actually had welded a hole through the side, or the heat just transferred”) HOTWORK FIRE Freightliner fire won’t stop work (a fire broke out around midnight and was under control by 2:30 a.m. – the small fire did not damage any production equipment or the building – flames broke out deep inside the building in a filtration system for a welding area – fire department reported estimated damage of $75,000 to $100,000 although the company would not confirm the accuracy of those reports – a manager at the plant estimated the company lost $1 million in production time – no one was injured but more than one employee said some workers went home due to the smell) FIRE Fire briefly shuts down Mercury Marine Plant (fire briefly shut down a plant around 6:50 a.m. – a spark ignited oil in one of the machines and started on fire – the blaze was contained to one of the machines – the machine’s operator was evaluated after the fire, but returned to work without any injuries – the plant was shut down for about 90 minutes because of the incident – the building had no structural damage – there was no damage estimate given for the machine) CHEMICAL Chemical Spill At French’s Plant In Springfield (several employees at the plant have been decontaminated at a Hospital after some sort of chemical spill at the plant – there doesn’t appear to be any serious injuries – don’t known what kind of chemical or substance the workers were exposed to) ELECTRICAL FIRE Ames Transformer Fire Quickly Extinguished (a transformer caught fire about 5:30 a.m. Thursday, but flames were quickly extinguished thanks to a fire protection system – firefighters responded to the fire at a transformer – a fire protection system surrounding the transformer contained the fire, and fire crews didn’t have to do anything else – the cause of the fire is still under investigation) AMMONIA Ammonia leak prompts response (a small anhydrous ammonia released occurred at a business around 8 a.m. – that response was cancelled at 8:20 a.m. after a unit of the Fire Department reached the scene and deemed there was no emergency – a small amount of anhydrous ammonia was released due to a faulty valve – the leak was stopped remotely) HYDROCHLORIC Investigation launched after acid leak (an investigation is under way to determine what caused a storage tank to leak hydrochloric acid that created a vapor cloud and prompted a 5½-hour shelter-in-place order – the leak sent at least 10 people to the hospital, including four firefighters who were exposed to the corrosive chemical compound – the leak happened at about 11 p.m. – ten people were taken to a Medical Center for treatment for exposure – four were firefighters, and the rest were workers or contractors from the facility – all were treated and released – there were also unconfirmed reports that as many as seven residents from neighborhoods near the leak site were taken to the hospital after complaining of symptoms from exposure – the cause of the leak or how much acid was released was unknown) NUCLEAR Dept. of Energy confirms tank leak at Hanford nuclear reservation (The Department of Energy has confirmed that its oldest double-shell tank is actively leaking radioactive and hazardous chemical waste from its inner shell – DOE made the announcement after a video inspection of the area between the shells showed more waste in one place than a video taken earlier showed – although there’s no good way to measure the amount, it could be a couple of tablespoons of additional waste between the video inspections – Tank AY-102 is the first of the facility’s double-shell tanks known to have leaked waste from its inner shell – the Hanford nuclear reservation has 28 double-shell tanks that are being used to hold waste from older single-shell tanks, many of which have leaked in the past – together, the two types of underground tanks hold 56 million gallons of radioactive waste left from the past production of plutonium for the nation’s nuclear weapons program – Tank AY-102 is a year or two past its 40-year design life – the waste is not believed to have leaked from the outer shell, which serves as secondary containment for the tank, into the soil beneath it – no waste has been found in the leak-detection pit beneath the tank) CONSTRUCTION WORKZONE FATALITY Construction worker dead in 4-vehicle crash on I-35 (a construction worker is dead and two others injured after a four-vehicle crash – the wreck was called in at about 12:34 a.m., when a truck-tractor traveling south struck the rear of a road construction truck – the force of the blow cause the road construction truck to sideswipe a road sweeper, which in turn struck a parked GMC pickup – the deceased man, who was on foot, was positioned between the road sweeper and the pickup) FATAL FALL Newfoundland worker dies after fall at downtown construction site (a worker is dead following a 24-metre fall at a construction site – a break in a wooden railing going around the edge of the building could be seen and it was being examined by police investigators – the industrial accident occurred just before 4 p.m.) CRANE FATALITY Worker killed in HK-Macau-Zhuhai Bridge accident (a construction worker was killed and 14 others injured after falling steel rods at the construction site of the bridge hit them – preliminary investigation showed the steel rods which were being lifted fell suddenly and the group of workers were struck) SCAFFOLDING COLLAPSE 4 workers injured, 3 seriously, after scaffolding collapse at Shaughnessy construction site (three construction workers remain in serious condition after they were reportedly caught in a scaffolding collapse – another worker was less seriously injured in the accident, which happened around 6:30 p.m. – initial reports saying the workers were injured after falling around six metres following the scaffold’s collapse – the seriously injured workers were taken to a Hospital) ELECTROCUTION Worker killed when he touches an electrified wire while painting a tower (a man hired to paint a tower was electrocuted when he came in contact with an electrified, 26,000-volt wire – the worker fell from the tower and on his way down struck another man also working on the tower – both men fell 50-plus feet – the second man was taken to an area hospital in serious condition, he was not electrocuted – neither man wore a harness – the man who touched the wire appeared to have died of electrocution and the fall) WORKZONE Semi Collides With MoDOT Truck, Shuts Down NB Lanes of I-229 (a tractor trailer was headed northbound when he collided with a Department of Transportation truck – five DOT workers were sweeping the bridge when the accident happened – one was in the truck that was hit, but no injuries have been reported) WORKZONE FATALITY Conroe construction worker killed (just before 8:30 a.m. a construction vehicle pulling an 18 foot trailer, towing a sign-board trailer, had just stopped in the left lane to install signage and begin working – worker, 27, exited the Dodge flatbed and was working near or in between the truck and the trailers – a 2010 Honda Accord was traveling southbound in the left lane and allegedly failed to control speed, striking the rear of the trailer towed by the truck – worker was hit by the truck and the trailer and did not survive the impact – he was pronounced dead on scene) CRANE FATALITY Chinese worker crushed by crane in Madaripur (a construction worker, 52, died as a crane fell on him around 11:00am – he was rushed to a Hospital where doctors declared him dead) CRANE Large Crane Falls and Crushes Rare Jaguar XKR-S and BMW 6-Series Coupes (a large crane trying to position a huge fuel tank for a new gas station being built tipped over and crushed a pair of cars parked on the lot – the two vehicles were a rare Jaguar XKR-S coupe (the 2008 model), of which only 200 examples were built for the European market and one he reportedly had just…sold for €150,000 (US$194,000), and a previous generation BMW 630i Coupe – the accident was caused when the ground under the 60-ton crane collapsed from the weight sending both the truck and its load to the ground and onto the premium sports cars) MISCELLANEOUS STRUCK BY VEHICLE FATALITY Man killed by milk truck at Brampton dairy plant (a man hit by a milk truck at a dairy plant around 1:30 p.m. has died of his injuries – the victim was transported to hospital where he was pronounced dead – police are still investigating how the accident occurred) FATAL FALL from ROOF Worker dies in fall at Lafayette hotel, 2 others fall in West Lafayette (a maintenance worker, 42, at a hotel working in the attic slipped off a rafter and fell through the building’s facet, tumbling three stories to his death about 3 p.m. – it is unknown why he was in the attic area of the hotel) CRANE FATALITY Officials identify Maui worker killed in crane accident (a crane operator, 62, was fatally injured when the crane tipped over, crushing him beneath it – he was moving equipment when the crane he was operating tilted to one side at 8:44 a.m.- he sustained fatal injuries and died at the scene) HYDROCHLORIC ACID Worker burned by acid at city pool (a city employee suffered a minor acid burn on his arm when he was exposed to a chemical used to clean pools – employees were repairing pipes in the building next to the pool at City Park about noon when one worker accidentally stepped on and cracked a pipe containing hydrochloric acid – the employee was treated by a city doctor – Fire Department set up decontamination pools, suited up and entered the building – they found that the acid, a liquid, had flowed into a building drain and into the sewer
