– authorities are trying to determine what caused a fatal accident
at a municipal leaf composting facility – worker died after the
hydraulic bed of a box truck he was unloading fell on him at the
facility – he was working alone at the time and the truck was still
running when co-workers found him around 1:30 p.m. – he had been a
public works employee for 30 years – he sustained a head injury and
was pronounced dead shortly after he was found)
FORKLIFT FATALITY
Worker killed in forklift accident (a worker, 50, was killed in
a floor collapse at a business while operating a forklift over an
area not reinforced for heavy equipment – when fire crews got
inside, they saw a portion of the concrete floor had collapsed into
the basement – apparently an employee was operating a forklift or a
tow motor over a section of floor that was not reinforced for heavy
equipment – officials say the longtime employee was killed when the
floor gave way sending debris 14 feet into the basement – she had
been collecting products from the warehouse to bring it to the
production line and during that process the employee went over a
section of flooring that was not reinforced for heavy equipment)
FATAL EXPLOSION
Maintenance worker killed at Progress coal-fired plant (a
technician, 24, was killed after a unit at the coal-fired plant
exploded – he was performing maintenance on the unit when the
accident occurred just before 9 a.m. – no other employees were
injured – the unit, which cycles on and off based on customer
demand, was not in operation at the time – the cause of the blast is
under investigation – he had worked at the plant since 2004)

Engineer Falls to His Death Working on Launch Pad for NASA (an
engineer fell to his death as he worked on a launch pad – he did not
respond to emergency personnel who arrived at the scene of his fall
on Launch Pad 39A at the facility – an investigation into the fall

Worker injured near Bunge plant dies (a worker, 28, who was
injured in an industrial accident has died at the Mayo Clinic in
Rochester, Minn.- he was struck in the neck by a piece of metal
March 8 – accident caused him to lose oxygen to the brain and he
suffered irreparable brain damage – the accident occurred at a
soybean processing plant – he was standing near where other crew
members were cutting rail when an errant piece of metal struck him)
QUARRY ACCIDENT
Worker at cement plant injured after drilling tool fell on him
(a worker at a cement plant was injured when a drilling tool fell on
him in a quarry where workers have been pumping out water – the man
was taken to a Medical Center with a collapsed lung and is expected
to make a full recovery – the worker was conscious, speaking and
alert after the accident – the accident occurred when a {amp}quot;drill
steel{amp}quot; came lose from a drill)
CRUSHING FATALITY
Worker dies at industrial site (a worker, 40, died after
becoming pinned between two pieces of industrial equipment – upon
arrival, officials found an employee of the job site pinned between
two pieces of equipment that workers were attempting to erect at the
gas well site – officials pronounced the employee dead at the scene
and ordered an autopsy)
MINE FATALITIES
Seven die, 41 trapped in coal mine accident (at least seven
miners were killed and 41 others trapped underground when explosions
triggered a collapse in a coal mine – a total of 48 miners were
working around 1,200 metres underground in the mine and officials
said the chances of the missing surviving were slim – the mine was
poorly ventilated allowing poisonous gases to accumulate and cause
the three blasts – rescue workers had recovered seven bodies so far
– there is a huge quantity of methane gas inside the mine)

Vapor explosion injures worker in Skippack (an employee
repairing a milling machine was injured in a vapor explosion – the
victim, described by company officials as an {amp}quot;experienced, heavy
equipment maintenance person,{amp}quot; was flown by helicopter to a Hospital
for treatment – an investigation into the incident was being
conducted – there was really no damage to the building or the
machinery)
FATAL EXPLOSION
Two killed in Dombivli chemical factory blaze (two persons were
killed and three injured in a fire at the plant – thirty workers
were in the pesticide manufacturing unit{apos}s plant 4 when there was a
huge explosion at 1.45 pm – two workers succumbed on the spot due to
100% burn injuries, while three others were taken to a hospital with
burn injuries – one is in a serious condition)
REFINERY COOLING TOWER FIRE
Fire at South Philly Sunoco refinery (a fire broke out in a
cooling tower at the refinery around 4:10 p.m. – fire crews were
called to the refinery in order to assist refinery crews in dousing
the blaze, however, their services were not needed – fire was
declared under control at 4:32 p.m. – one worker was transported by
fire medics to a Hospital for treatment of non-life-threatening
injuries – no word yet on what caused the fire – no evacuations in
the neighborhood have been deemed necessary)

Haz Mat Team Responds to Ammonia Leak at Santa Clara Valley Water
Treatment Plant (Fire Department{apos}s hazardous materials team
responded to an aqueous ammonia leak at a water treatment plant –
about 250 gallons of ammonia leaked into a secondary containment
area at the plant – the leak, caused by a broken instrument probe,
was released from a 5,000-gallon holding tank and fully contained in
a concrete secondary containment area – as of 2:45 p.m., the fire
department determined there was no more leaking, lifted the
shelter-in-place warning for nearby residents and turned the site
back to the water district for cleanup by its chemical crews)

responders transported her to a Hospital – the accident occurred
just after 8 a.m. at the work site for the university{apos}s new science
center – the woman who was killed was a contract worker – the
woman{apos}s coworkers were performing CPR on her when emergency
personnel arrived)
WORKZONE FATALITY
Gilroy Road Worker Killed, Struck by Car (a city employee was
struck by a car as he was doing a routine inspection in the roadway
around 11:00 a.m. – at this time not much information is known about
the accident – the employee was a Maintenance Worker II, and was
hired by the City as a part-time employee in April, 1997, and as a
full-time employee in January 1998. He had a Bachelor of Science
Degree in Business and Human Resources from Sonoma State University.
He was 40 years of age)
MANLIFT FATALITY
Worker dies from injuries in Santa Fe auditorium collapse (a
man, 21, who was injured by steel beams that collapsed in a high
school auditorium that was under construction has died –
firefighters were called at 9:34 a.m. to a High School – no children
were involved in the incident – a crane at the construction site was
used to lift a 12-foot-long steel column a few inches to free the
most seriously injured worker – worker was tied to one beam that was
tied to another, which broke loose, causing him to fall about 50
feet – he was trapped in the debris – two other workers, 26 and 44,
suffered injuries that were not life-threatening – they were taken
by ambulance to the medical branch for treatment)
Accident claims life of Benton man (a contract worker, 37,
employed by a drywall contractor was killed at a construction site –
EMS pronounced him dead at the scene – the cause of the incident is
under investigation)

detector specialist was trapped when sand caved in and buried the
man in a hole at a Water Park – firefighters and law enforcement
worked to free a man who was buried in sand – he had located a

caved in on him, burying him – a concrete block also hit his exposed
head – his son was the only other person at the park at the time of
the accident – authorities were called in and were able to free the
man, who was taken to the hospital, but was not seriously injured)

injured after his leg was crushed by a road roller – firefighters
used a winch, hydraulic rescue equipment, and air bags to release
the injured man at 4.10pm – he was taken by land ambulance to a

was seriously injured after he was crushed between two 20,000-pound,

Grisly on the Job Accident as a Port Chester Man Gets Caught in
Stump Grinder (a horrific accident involving a tree trimming
company employee and an operating stump grinder has left the worker
fighting for his life – a worker suffered potentially
life-threatening injuries after one leg and both arms became
entangled in the wheel of a large commercial stump grinder servicing
a residential property – a commercial stump grinder is an industrial
machine with a serrated wheel that rotates to chip away the wood of
the tree stump)

Staples (an electrical contractor, 40, was crushed to death
installing light bulbs from a boom truck – he died while working
alone in the bucket of a high-lift truck and using controls in the
basket to move it into position to install lights at the front of a
store – he was to meet a co-worker onsite but got into the bucket
before that and was positioning it above the sidewalk near an
outside overhang when the accident occurred – he had worked for the
business the past two years- some of the possible causes include
operator error, a mechanical malfunction or a medical problem that
prevented him from stopping the bucket as it approached the overhang
– he was pronounced dead at the accident scene after having his
upper body crushed against the overhang – he had his back to the
building and facing the truck when initial contact was made – Cal
OSHA officials had examined the bucket and lift apparatus before the
vehicle – there were no mechanical problems)
Woman burned at dental laboratory (one person, 35, was seriously
injured when a burner ignited at a dental laboratory – a worker was
burned while working in an area near the alcohol lamp – she suffered
second-degree burns – the woman is a technician at the lab and has
worked there for a few months – she suffered burns to her face and
upper body -she was taken by Medic 1 to a Regional Medical Center
and was later flown to a Burn Center)
