INDUSTRIAL Worker’s legs trapped in wastewater plant accident (a worker was trapped by his legs when a load came off a truck at a wastewater treatment plant – he was taken to a Hospital with minor injuries after the accident) LOTO/GUARDING Worker at plate maker O.S. Kelly suffers arm injury (employee, 56, was taken to a Hospital after an industrial accident left him with a serious arm injury – his left arm appeared to have gotten caught between a steel pallet and the roller system that was moving it – the worker was “bleeding profusely” when the accident was reported at about 6:30 p.m.) UPDATE BURNS FATALITY AK Steel ‘deeply saddened’ by fatal accident (an employee, 48, died from injuries he suffered on the job April 7 – Barnes, 48, – the employee suffered burn injuries while he was preparing for maintenance repair work at 10 a.m. April 7 at the plant’s steelmaking shop – he had been hospitalized since the April 7 accident – he was a shift manager in the plant’s steelmaking department and had 16 years of service with the company) MINE FATALITY Cote Blanch Salt Mine worker dies in accident (an employee at the has died in an accident at the facility – he was part of a crew performing maintenance at the site when the accident happened – he was treated by emergency personnel at the scene, before being transported to a Hospital – doctors there could not revive him) Accident at Thompson Metal Fab takes life of Sheet Metal worker (worker, 33, was killed on the job as he was grinding metal on an oil derrick when he was crushed by a steel plate) MINE Two workers injured after accidents at Broken Hill mine (a worker has been injured in a rock fall at a mine, while another suffered serious injuries after a crushing accident – the 49-year-old jumbo operator suffered injuries to his head and leg when a rock fell at around midnight – emergency crews transported the man to a hospital – while in a separate incident at the mine a 50-year-old man was hospitalized after a crushing accident at the mine – the man suffered serious injuries after being crushed between two vehicles – the man was trapped for a short time before being transported to the surface) UPDATE Worker injured in W.Va. natural gas accident dies (a worker, 56, has died from injuries suffered in an accident at a natural gas operation – he died late Friday night – two other workers were injured in an accident at a pipeline operation – the incident was an explosion at a compressor station – company says it was a flash fire at a “pig receiving station” – initial reports indicate that natural gas liquids ignited in tanks during “pigging” operations and fueled the fire – the workers were employed by contractors) CRUSHING FATALITY Man, 21, dies in apparent industrial accident at Santa Rosa glass company (a 21-year-old man died in an industrial accident at a glass company – he was moving large panes of glass – it all fell on him, all seven pieces – they were 110 pounds each – sheriff’s authorities described the glass storage device as a mobile A-frame with rollers – accident occurred near a dumpster at the back of the large warehouse) CHEMICAL REACTION Grand Island plant evacuated for chemical accident (five workers at a meatpacking plant were taken to the hospital for treatment after a chemical accident – bleach accidentally was pumped into a lactic storage tank, causing a chemical reaction – all five workers were evaluated at the hospital and released – accident happened about 9:15 p.m. and the Fire Department was called in as a backup shortly before 10 – the plant was evacuated while employees ventilated the area with exhaust fans – the plant was not operating when the accident happened, and production was not affected) FLASH FIRE 12 workers injured in fire at ExxonMobil refinery (twelve workers were injured in a fire at a refinery at about 10: 30 a.m. in the processing unit – fire was quickly put out and the unit was shutdown for repairs – the injured contract workers of the company were taken to a hospital with a variety of injures – three of the worker were severely burned – an investigation was launched into the cause of the fire) Hamilton worker killed at Wasteco recycling plant (a worker was killed on the job at about 9 a.m. after the man was injured inside the waste and recycling facility – he was inside the recycling facility unloading the contents of a bin when he was struck by a metal bin door) LOTO/GUARDING FATALITY Worker dies in Perth brick factory accident (a workman has died after being caught in machinery at a brick-making plant – he became caught in machinery at a brick-making plant) FATAL EXPLOSION Texas blast death toll at 14; only 1 or 2 missing (a town was rocked by an explosion at a fertilizer plant – officials say 14 bodies have been recovered – five were firefighters and four were medics – explosion and fireball destroyed homes, businesses, a school and nursing home – only one or two people remained missing – at least three rescue trucks and one fire truck were also destroyed) EXPLOSION @ WWTP No one injured in explosion at water treatment plant (a garage door was blown off its hinges after an explosion at a wash bay on the grounds of a wastewater treatment plant – no one was hurt and the cause still hasn’t been made public – explosion happened in a bay where vehicles are washed – workers arrived to the building and saw flames 3 feet high coming from the sewer) ELECTROCUTION Plastic factory worker electrocuted (a laborer, 25, was electrocuted while working on a machine – he was immediately rushed to a nearby hospital, where he died during treatment) CONFINED SPACE FATALITY – ENGULFMENT Janesville man dies in Milton grain silo accident (an industrial accident at an ethanol grain silo killed a worker, 27 – after more than 12 hours of searching rescue efforts came to an end when the body of the worker was found at 11:11 p.m. – he was reported missing about 10:20 a.m. at the plant – there were 140,000 bushels of corn in the silo during the accident, less than one-quarter of the silo’s capacity of 680,000 bushels of corn) AMMONIA Ammonia leak at Oak factory (workers at the former factory had to be evacuated over the weekend after an ammonia leak at the site – the minor leak came from a valve in the old section of the factory – fire brigade crews arrived at 6.30am and hazmat crews identified the leak and repaired the pipe – while workers were evacuated, no one was harmed as a result of the leak) AMMONIA Ammonia leak in Calgary food warehouse (ammonia leak prompts industrial building’s evacuation – roughly 70 employees were evacuated from the warehouse around 7 p.m. after a leak in the building’s cooling system caused the release of ammonia – no injuries were reported and the cooling system was shut down while crews ventilated affected areas – maintainence workers told firefighters that the leak was confined to only one room of the building) CRANE COLLAPSE Crane collapses at Port of Portland’s Terminal 4; no one hurt (a crane collapsed at a Port – a crew was working on demolishing the crane, but a project expected to last months ended in about a minute – no one was hurt – crew was working on demolishing the crane when they heard a “pop” and some other strange noises – a minute later that crane collapsed – crane was old and had been out of use for more than 20 years – the crane was about 80 feet tall) CONSTRUCTION Conn. worker dies in Northampton construction accident (a worker, 56, has been killed in a construction accident at a Transportation Department facility – he died when he was pinned between two modular office units he and two others were working on – one of the other men was taken to a hospital with non-life-threatening injuries, the third man was not hurt – the men were assembling the temporary buildings when one of the units apparently shifted) WORKZONE FATALITY Two Workers Killed In I-55 WorkZone Crash (two construction workers, 34 and 55, were killed during an accident on I-55 – investigators say a pickup truck crashed into the rear of an asphalt paver where the two men were standing – they were killed instantly – the pickup swerved into a closed off construction lane hitting the workers – paving crews were staging along the inside shoulder of the interstate and had not yet started overnight paving work) FUMES FATALITY Worker found dead at City Hall (a worker employed to do a renovation project in City Hall was found dead – police officers were called to the city building and found a man who police believe died after being overcome by fumes emitted from a stain being applied during a renovation project) WORKZONE FATALITY I-20 road worker killed today when he was knocked off truck top by downed fiber optic lines (a man, 58, working on an Interstate 20 construction project died after he was knocked off the top of his truck by fiber optic lines around 8:30 a.m. – he was pronounced dead at the scene – he was standing on top of his truck’s dump bed on the side of the roadway when another truck got snared in downed utility lines – as the wires came down, they knocked him off the top of his truck) FATAL COLLAPSE One dead in Brazil stadium collapse (a construction worker, 34, died after the collapse of a grandstand being built at Brazil’s Arena Palestra football stadium – he died instantly when he was crushed by a concrete column at the stadium – another worker was taken to a hospital with minor concussion and back injuries) FLASH FIRE Indianapolis apartment blast injures 2 workers (an apartment explosion has seriously injured two workers – the workers were applying paint or stain to cabinet when a something ignited a buildup of fumes – the force of the blast blew the front door off the unit – the workers have been taken to the burn unit – firefighters extinguished the blaze in about 30 minutes) MISCELLANEOUS Worker Flown to Trauma Center After Accident (two landscape maintenance employees were attempting to start a vehicle – one employee was in the driver’s seat and another was in front of the vehicle – after the vehicle was started, the driver got up, however didn’t realize the manual transmission was in gear, which caused the vehicle to jump forward when the clutch disengaged – the front of the vehicle struck the second employee which caused him to fall backwards and trip over the tongue of a trailer – after falling backwards, the employee, 58, struck his head on the asphalt roadway – as a result of the head trauma, the employee was airlifted to a Medical Center) LOADING DOCK FATALITY Authorities ID UW worker killed in lift collapse (a maintenance worker, 62, was killed in a loading dock accident – he was repairing a loading dock lift outside the Arts Loft building on campus when the lift collapsed on him – he was taken to a Hospital but died a short time later – preliminary autopsy results indicate he died of injuries sustained in the collapse) HORSE PLAY Prank goes wrong as garage worker suffers serious burns when colleague sets the toilet he was using on fire (victim was forced to jump through a cubicle window after a dangerous prank went badly wrong – worker, 26, escaped jail despite setting the cubicle on fire while a co-worker was inside sitting on the toilet – worker poured brake cleaning fluid under the cubicle door before lighting it – a huge blaze erupted and the worker needed hospital treatment after suffering serious burns) FIRE Worker injured in North Providence fire (it appears a worker with a power tool started a fire at a local auto salvage yard that injured one person – one worker was taken to a Hospital with burns on his arms and is expected to recover – it’s not clear if the injured worker was the same employee who apparently started the fire – the fire began in a building where a few employees were working and spread to other parts of the property – the building was destroyed – cause remains under investigation) |
