INDUSTRIAL MINE FATALITIES At least 151 dead, 76 injured in Turkish coal mine blast (an explosion and fire at a coal mine in Turkey killed at least 151 people and injured dozens of workers underground – the dead workers are believed to have died from suffocation and burns and dozens of workers had been taken to hospital – there were 787 workers inside the coal mine during the explosion – the number of injured workers at 76 – a fire, triggered by an electrical fault, erupted and that workers had been killed) CRUSHED by PIPE @ WWTP Savannah worker killed on job (a 12-year city employee died during an accident at a water treatment plant – he was a senior maintenance mechanic and was fatally injured while making repairs to a pipe shortly after noon when a heavy piece of piping fell on his chest – he was taken to a Medical Center where he was later pronounced dead) MINE FATALITIES West Virginia Coal Mine Workers Killed After Structural Failure (a coal mine accident has left two workers, 46 and 48, dead – there were structural failures within the mine – multiple reports have stated that the accident was caused by a “coal outburst” which may have been the result of stress on the mine’s structure – the outburst resulted in a ground failure around 8:45 PM – the ground failure reportedly trapped two workers whose bodies have since been recovered by rescue crews members) LOTO/GUARDING FATALITY Industrial accident victim leaves behind wife, 3 children (worker, 49, died of accidental asphyxiation after he was trapped on a conveyor belt in the 1:49 a.m. incident – Fire Department freed and tried to resuscitate the man, but he was pronounced dead at the scene) LOTO FATALITY w/ CRUSHER BELT Worker dies in freak accident in cement factory (worker was killed while cleaning the belt of a crusher in a cement plant – the worker who was cleaning the crusher belt got trapped in the machine – he was rushed to a Hospital but he died on the way) GUARDING on CONVEYOR BELT Worker gets hand caught in conveyor belt at Azusa trash sorting facility (a worker was hospitalized after getting his hand stuck in a conveyor belt at a trash sorting facility shortly after 10 p.m. – worker apparently got his hand caught in a small gap in a large conveyor belt that was being used to sort paper materials – due to the noise in the facility, coworkers did not immediately realize what had happened, and the injured worker remained trapped for an unknown amount of time before being freed from the conveyor belt) UPDATE now a FATAL EXPLOSION 5 months after Aerojet explosion, worker dies (one of the workers hurt in an explosion last November has died – he was previously being treated at a Medical Center – his exact cause of death hasn’t been determined – the industrial explosion took place inside a hangar at the complex about 8 a.m. Nov. 19) Worker hurt in glass factory accident (worker, 24, was stable but sedated in a hospital after he was crushed in an incident at a glass factory – no other details) UPDATE on MINE FATALITY Report: Dead Elko County miner caught clothes in drill (an experienced worker, 53, who was killed in an accident at a gold mine apparently got his clothing entangled in equipment while drilling rock underground – MSHA’s preliminary investigation found that he was operating a jackleg drill when his clothes got caught in the machinery – another miner found him unresponsive at about 11:30 a.m. – he had 32 years of experience in the mining industry and had worked at this mine for 11 years) MINE FATALITIES 2 workers killed at Sudbury nickel mine (two workers at a Mine have died in an accident underground – workers, 34 and 49, were drillers – one was a dedicated and experienced underground miner with over 20 years of experience, and the other had about four years -“ a fall of material” which was “preceded by seismic activity” is believed to have been a factor in the accident) UPDATE now a FATAL EXPLOSION Bryan utility worker dies after plant fire (a worker at an electric power plant has died following a fiery explosion that left two other employees hurt – the victim, 59, died Sunday at a hospital where he was taken following the April 29 accident at the utilities plant – no cause of death released – he had worked for the utility for more than 13 years) PROPANE EXPLOSION Ontario worker burned in propane tank explosion (plant employee suffered facial burns after several propane cylinders exploded at a filling station, spreading the blaze – See video of the explosion here – happened around 9:30 p.m.) EXPLOSION 1 hurt in Lafayette metals recycling plant blast (an explosion at a metals recycling company has injured an employee and possibly damaged a passing automobile – the worker was cutting metal when the blast occurred, injuring the man’s face – the employee was treated at a hospital for non-life-threatening injuries – the cause of the explosion is under investigation – a passing motorist reported hearing the explosion and then his rear window shattered) HOTWORK FATAL EXPLOSION Man killed in explosion at Southern California trucking company is identified (a worker, 51, was killed in an explosion at a trucking company while he was welding a petroleum tanker truck – another worker was taken to a hospital in critical condition – the blast blew a hole in the roof of the building but didn’t cause a fire) CONFINED SPACE Sewage worker rescued from manhole (worker had to be rescued from inside a manhole when he passed out after apparently inhaling a blast of methane gas – he was working on a sewage pipe when he lost consciousness – his co-worker attempted to come to his aid but started to become overwhelmed by the gas and had to be helped out of the manhole himself – ambulance crews pulled the stricken man to safety and delivered first aid – both workers were in a stable condition in hospital – A spokesman for the sewage company described the incident, which took place at around 11 a.m. yesterday as a “freak accident” that could have ended in tragedy) FATAL HOTWORK EXPLOSION Worker killed when tank explodes (worker, 29, died about 4 p.m. after he was seriously injured when an oilfield bulk-storage tank exploded inside a metal building – he was standing on top of the tank and welding a hook onto it when the explosion occurred – the worker and the tank went upward and hit the roof of the building, then fell about 20 feet to the concrete floor – when firefighters arrived, they found co-workers spraying the fire with extinguishers, and the deceased wedged between the burning tank and an inside wall of the building – another man who was welding on the side of the tank at the time was blown against the wall, and went to a Hospital to be checked out) AMMONIA Kitchener, Ontario meat plant evacauted for ammonia leak (plant was evacuated after two ammonia leaks were discovered – crews arrived around noon to find 500 lbs of ammonia leaking from two spots – building was evacuated – media reports there are more than 700 employees at the facility – one leak was stopped by fire crews; the other was contained but hadn’t been shut down as of late Tuesday afternoon – residents were asked to stay indoors, keep windows and doors closed as well as turn off ventilation – one person was checked by paramedics but not taken to hospital – could not estimate how long it would take to stop the leak or when employees would be able to return) AMMONIA Ammonia leak briefly disrupts work at Gainesville plant (nobody was hurt when a leak was discovered in an ammonia line – an alarm sounded just before 8:30 a.m. and employees left the facility – the ammonia leak was restricted to a mechanical room – employees were allowed to returned to work within an hour) CHLORINE Chemical vapors force plant evacuation (about 30 employees at a beverage manufacturing plant were evacuated because of a chlorine gas discharge – authorities were called to beverage plant after reports of smoke – firefighters responded and found the smoke was a chlorine vapor that had escaped from a holding tank while chemicals were being delivered to the plant – no injuries were reported) CONSTRUCTION CRANE Injured crane workers info released by WorkSafeBC (three ground workers were struck by a falling crane load – crane was delivering insulation material using a sling when the material dislodged and fell striking three workers on the ground – all three workers sent to hospital, details of injuries not known at this time but not believed to be critically injured – all three individuals were released from hospital and were at home by the afternoon) FATAL FALL Hillsboro native dies in North Dakota accident (a construction worker, 48, died after falling off a four-story structure – he as working on the roof of a structure just after 1 p.m when he tried to gain control of a piece of plywood that had been blown up by a gust of wind – he stepped on another loose piece of plywood and slid off the roof – he had momentarily removed his tether to reposition himself on the roof when the incident happened – he was taken to a Hospital where he was pronounced dead) WORKZONE Road Worker Seriously Hurt In Accident On M-14 (two people, including a construction worker, were hurt in an accident that closed down a local freeway around 1:15 p.m. – an SUV was approaching a construction zone when the driver apparently lost control, striking a road worker as the vehicle rolled over – road worker was airlifted to a Hospital with serious injuries – driver of the SUV was hospitalized with minor injuries) FATAL COLLAPSE Five killed in south China staircase collapse (five construction workers have died after a staircase in a building they were working on collapsed – accident happened in an under-construction building at 8:15 a.m. when seven workers were on-site – one worker was killed on the spot while four others died in hospital – two other workers were injured but are now in a stable condition) CRANE Worker injured when crane flips, pins him on bridge (a construction worker was injured when a crane toppled over – a crew was working at a road site when the crane fell over and pinned the operator between it and the bridge – accident took place at approximately 9:30 a.m. – the operator, who was not identified, suffered minor injuries – the operator possibly broke his pelvis but was expected to make a full recovery) WORKZONE FATALITY Two Highway Workers Killed In El Paso Accident (two highway workers have died after being hit by a vehicle in a drunken driving investigation – one bridge worker was dead at the scene – the other worker died at a hospital – police detained an 18-year-old driver – the driver ignored a traffic barricade) ELECTROCUTION Brazilian worker electrocuted at site of World Cup stadium construction (a worker, 32, at a World Cup stadium in Brazil died in an electrical accident – the construction worker received an electric shock and died more than half an hour later of a cardiorespiratory arrest – officials were not clear on what caused the accident – this is the eighth death from injuries while building stadiums for the world’s biggest soccer tournament) AERIAL LIFT FATALITY OSHA investigates after man falls 40 feet while working on billboard (worker, 29, died after he plummeted 40 feet while installing a billboard – around 3:45 p.m. a construction crew was installing a large billboard and the worker was in a bucket inside a cage suspended from a crane about 40 feet above the ground – he was working on a large metal pole that holds up the billboard – the pole shifted, hit the bucket and caused him to fall about 40 feet) TRUCK Police: Man crushed by construction vehicle in Clearfield County (worker was flown to a hospital after being crushed by a truck just after 10 a.m. – he had been conscious at the time of the accident; his condition was not known – crew was doing clean-up work outside the fence of a substation when a worker was struck by one of that company’s trucks) FLASH FIRE Paint Cause of South Tulsa Explosion That Left Man Burned (a man burned in an explosion at home is recovering – the man had been painting in an area of the kitchen prior to the explosion – the paint fumes had ignited leading the blast – no other injuries were reported at the scene of the accident – officials added that the home was being repaired from previous fire at that same location earlier this year) HOTWORK I-15 bridge collapse occurs after worker’s blowtorch ignites wooden supports (an I-15 bridge collapse caused a traffic nightmare for commuters traveling to California on I-15 – the three lanes of Interstate 15 were closed in both directions after the blowtorch of a construction worker accidentally burned the wooden supports of an overpass that was under construction – dozens of firefighters battled the blaze despite the wind and limited access to water – the bridge collapse occurred just hours after the fire erupted while laborers worked to complete the demolition and begin cleaning the area) TRENCH FATALITIES Two dead in Espoo construction site cave in (two men were killed at a construction site when an area excavated for the installation of civil engineering works collapsed – the two men were working at the bottom of the trench when it caved in – a third person on the site attempted unsuccessfully to dig the two out – according to an emergency services representative, no supports had been installed on one side of the 3 x 6 metre trench) Miscellaneous QUAD ACCIDENT Farmer in critical condition after quad bike crash (a farm worker, 64, is in critical condition after the quad bike he was riding rolled over on a hillside at about 8.30am – he was doing some fencing and was riding the bike up a fairly steep hillside when the accident happened) CONTROL BURN FATALITY Western Nebraska irrigation district worker dies on the job after falling into ditch fire (an irrigation district employee, 67, has died after falling into a blazing ditch, where he was burning weeds – the accident occurred when he slipped down a ditch embankment and fell an estimated 11 feet into the fire – he was taken to a hospital with severe burns over 75 percent of his body and was pronounced dead at the hospital a short time later) HOTWORK El Dorado Fire Department called to storage tank fire (worker was using his torch to cut apart the tank – he had checked it out, but didn’t realize the bottom side of the tank was made of fiberglass – the fiberglass ignited shortly after the man started the job and the tank was quickly engulfed in smoke and flames – no one was hurt) HOTWORK Fire crews extinguish flames in Waterman Building (the blaze was caused by a cutting torch used to remove a sign from the building) AMMONIA Ammonia leak at Schneider plant in Kitchener (officials say they were re-charging the air-conditioner which caused the leak – it is a regular procedure – police had residents in the area close their windows and stay inside – the leak occurred on the seventh floor in the office towers and was cleared around 2 PM – no one was injured and the ammonia levels were not high enough to cause concern) |
