INDUSTRIAL

FORKLIFT Injured worker airlifted to Royal Blackburn Hospital (worker, in his forties, was airlifted to a hospital after an accident involving a skip lorry just after 10.30am – it is believed that the accident happened when the employee was in the cab of the lorry at the textile manufacturing company – the mechanism on the skip failed and it jolted downwards, the force of which caused injuries to the man’s back and neck)

STEEL PLANT LADLE ACCIDENT 13 injured in steel melting shop accident in Vietnam (thirteen workers were injured, some seriously burnt when a ladle containing some 150 tons of melted steel at nearly 1,600 degrees Celsius exploded at a factory around 6.30 pm – 13 people were working at the scene when the ladle exploded)

MANLIFT FATALITY 2 workers killed in Bourne crane accident (two workers, both 34, were killed when a boom truck collapsed – the utility workers, both men, were 150 feet in the air working on electricity lines when the boom collapsed – the men died on impact – Police Chief said it was not particularly windy at the time of the accident and the ground under the truck appeared to be solid)

CRUSHED Worker ‘guarded’ after pinned by steel (worker, 48, was taken by Mercy Flight to a Hospital following an industrial accident about 7:30 p.m. – he was working when a stack of steel rolled onto him – one of his legs was trapped by a 20-foot bundle of steel weighing about 5,000 pounds – it took firefighters about 40 minutes to free him – he was initially trapped by two larger bundles, weighing about 12,000 pounds, against his torso, but those were removed by co-workers using a crane and chains – he suffered a serious lower leg injury and possible internal injuries, but was conscious during the rescue)

UREA PLANT MAINTENANCE –LINE BREAK w/ NAPTHA Chemical Spill at Fertiliser Factory Leaves Two Critical (two senior quality analysts were hospitalized in a critical condition after they suffered severe burns caused by spillage of a high temperature liquid substance on them – incident happened when they were carrying out a routine check in the factory’s urea and ammonia production lines – a total of seven persons were injured in the incident in which two employees suffered over 80 per cent burns – three analysts were involved in taking samples of naphtha, a highly inflammable substance that would be heated up to nearly 300 degree Celsius in the extraction process – but due to faulty valve or improper handling of the safety valve, the substance, which was very hot, started spilling on the workers – plant has been under maintenance for the past one month and the injured employees were engaged in the final phase of testing before restarting the unit – totally four persons were injured in the incident)

Man critically hurt at work (no word on what caused the head injuries while working – the man was somehow hit in the head while at work around 4 am – he was taken to the hospital in critical condition – No other details have been released)

MINE FATALITY Mine worker dies in mishap (worker, 49, was killed in a mine accident – he was driving a loaded dumper downwards on a slope in the mine when its engine failed – he lost control due to which the vehicle fell in a water sump nearby killing him)

DRILLING FATALITY Accident in India kills ONGC worker (an accident at a drilling site reportedly killed one worker and injured three others – the deceased worked as a chief engineer – nature of the accident was unclear, but the newspaper said the engineer sustained a head injury that proved fatal – the three injured workers were moved to a hospital and are expected to recover – a cause had not yet been established)

CRUSHING FATALITY Rice Energy Worker Killed Over the Weekend in Belmont County (a worker, 43, was killed when a piece of machinery fell and crushed him)

FATAL EXPLOSION Two killed, 16 injured in blast at chemical factory in Vizag (two workers, 31 and 36, both working as chemists were killed and 16 injured in an explosion at chemical plant – a reactor in block-A of the factory caught fire and exploded at around 6 am – some 30 workers were on the premises at the time – cause was being probed)

MINE FATALITIES Seven die in Ukraine mine blast (seven men have died in a gas explosion at a coal mine – no other details)

LAB EXPLOSION 2 injured in chemical explosion at Asinex in Winston-Salem (two people were taken to the hospital following a small chemical explosion inside a biotechnology lab just after 10 a.m. – the injured were taken to a Medical Center for evaluation – fire officials described the injuries as minor – explosion took place under a vented hood in a fourth-floor lab – “a well-controlled, normal reaction experiment somehow went awry, and a worker’s hands were exposed” – fire officials described the chemicals as sodium azide and a trade chemical)

FLASH FIRE No injuries after flash fire at business (employees safely evacuated their building after a flash fire just after 9 a.m. – workers had already evacuated the building and gathered at a pre-arranged meeting spot – workers told firefighters the flash fire had been extinguished – crews entered the building to confirm the fire suppression system had extinguished the fire – employees were allowed to return to the building just after 9:30 a.m. – fire appears to be accidental)

HOTWORK FIRE Welding accident caused Storm Lake plant fire (officials ruled the fire that heavily destroyed a turkey plant was caused by a welding operation – Fire Department and Iowa State Fire Marshal’s office announced the accident cause after an investigation of the March 22 fire at the plant – report cites property damage above $30 million – fire started in an east wall following a welding operation – the blaze took about three hours to extinguish – plant repair could take several months before reopening, although the company has announced no timeline – plant’s more than 600 employees continue to receive their pay and benefits until the plant is rebuilt)

CONFINED SPACE FATALITIES 2 die, 4 pass out due to asphyxiation at Vietnam paper mill (two workers died and four others lost consciousness allegedly due to gas asphyxiation at a pulp-filtering trench at a factory – accident happened when the workers were cleaning up an underground trench where waste paper pulp is filtered at the mill – two of them entered the trench first shouted for help when they began suffocating – the others came to help their colleagues, but they too became asphyxiated – upon being informed, police and firefighters came to the site, but they could not get all the victims out of the trench at once because its entrance was just about one square meter wide – the victims, who were not wearing face masks, were found unconscious in the trench – they were rushed to a Hospital, but two of them died on the way – four others, including two in critical condition, are now in intensive care – the workers were possibly asphyxiated by methane, a nontoxic gas that can displace oxygen in an enclosed space)

REFINERY FIRE Small Fire At Sarnia Suncor Refinery (on site sirens sounded around 9:30am after smoke was detected in a tower that was being opened for regular maintenance activity – the tower was quickly isolated and source of smoke extinguished – an all clear sounded just before 10:30am)

CHLORINE LEAK @ WWTP No injuries in Streamwood chlorine leak (no injuries were reported after a chlorine leak at a water treatment plant – firefighters were able to stop the leak by tightening a leaky valve – employee stated that he was disconnecting a 100-pound chlorine cylinder when the leak started – he evacuated the building and was not injured)

CRUSH FATALITY Moline man dies in industrial accident (worker, 54, died in an industrial accident when he became trapped under a large object he had been sandblasting at his workplace shortly after 1 p.m. – an autopsy determined the cause of death to be traumatic asphyxiation – “It was a big enough object that had to be lifted off of him with a forklift”)

CONSTRUCTION

Worker killed by falling digger on London building site (a building collapse killed a construction worker, 33 – he died instantly after being crushed by a mini-digger which plunged through the second floor of the gutted building – the man operating the digger was treated in hospital for minor injuries)

TRENCH FATALITY Fatal labour accident (worker, 32, was crushed to death when the walls of the ditch he was working in collapsed – the victim was working on the sewerage project – he was laying a pipe in a two meter ditch, when the sides gave way, crushing him to death – he was freed by fellow workers and first responders and taken to a Hospital where doctors declared him dead on arrival)

FATAL FALL Worker Falls To His Death From Building On Manhattan’s West Side (worker was killed after failing from a building – investigators believe the man fell from the roof of the building onto the sidewalk shed below – worker was rushed to a Hospital, where he was pronounced dead)

ELECTROCUTION Maintenance man electrocuted at Park Tower on Thursday (maintenance man, 43, died while updating the building’s fire alarm system – building maintenance crews were installing a low-voltage line in the building – he was working alone on the ninth floor – WARNING GRAPHIC DEPICTION of injuries in article)

EXCAVATOR CRUSHING Toronto man suffers severe leg injuries from industrial accident (worker, 25, was rushed to a hospital after he was trapped by heavy machinery in an industrial accident – he suffered severe leg injuries – he is expected to recover – the man’s leg had been crushed by a 30-ton excavator – he was alert and speaking as crews worked for an hour to free him)

WORKZONE Construction Zone Crash Critically Injures Worker in Gretna (a construction worker, 21, is in the hospital with broken bones and internal injuries after a crash – a man in a truck had a seizure and crashed into a tractor in a construction zone – the tractor was broken in half – an air compressor that was attached to the tractor toppled a worker – he was flown to the hospital – driver of the truck wasn’t seriously hurt)

CRUSHED by ELEVATOR Worker hospitalized after being pinned in elevator shaft (worker, in his 30s, is in the hospital with critical, life-threatening injuries after an elevator pinned him at a construction site – the elevator pinned the man’s head – he was one of the workers working inside the elevator shaft when the elevator was activated, pinning his head between a concrete surface in that shaft and the steel of the elevator – the incident happened as the man worked to remove old radio communications towers at the construction site)

WELDING FATALITY One worker killed in complex construction site (worker, 38, was killed while working at a skyscraper construction site around 8:40 a.m. – the worker was doing plumbing work on the 12th floor and died after getting hit in the face with a metal pipe when he was working on a joint weld in the section of the pipe)

PFAS SAVES ANOTHER LIFE!!!  Construction worker falls, suffers head wound (a construction worker installing a roof on a home fell and suffered a head wound, but has since been released from the hospital – when fire crews arrived he was “semi-conscious and was taken by helicopter to a Medical Center – the man, who was wearing a safety harness, fell and hit his head on the way down – the worker was properly wearing his harness, which supported him during the fall -“ The harness saved his life” – the worker fell about 4 feet)

FATAL FALL from SCAFFOLDING Man dies after fall from scaffolding (worker, 72, died from injuries received after a fall from scaffolding on a job site – he had climbed the scaffolding to about 15 feet from the ground, when he slipped and fell, landing on his back and head – he was transported to a Hospital where he was stabilized and airlifted to another hospital but died from his injuries)

FATAL FALL from ROOF Man dies after fall through roof at Shaw Industries plant in Dalton, Ga.(worker, 48, died after he fell through the roof at a plant – the roofing contractor fell between 30 and 35 feet and suffered a head injury but was conscious and alert when first responders arrived at 9:10 a.m. – the crew immediately started treatment and called for a Life Force helicopter, but conditions were too foggy for the helicopter to fly – he was transported to a Medical Center by ambulance but died as the ambulance was arriving at the hospital – fire department did not note whether the man was wearing safety equipment)

IMPALEMENT Man impaled at Bayfront Park horror movie shoot (a member of a film company making a gothic horror movie was impaled on a piece of metal on the first day of filming – the crew member was leaving a trailer – the man, who was in his 60s, fell while walking down some stairs and was impaled on what he described as a “stair stabilizer” – the object was “unbolted” and the man was taken to hospital with a piece of it “impaled in his buttocks” – the injuries were described as not life-threatening)

OXYGEN CYLINDER EXPLOSION @ MEDICAL CENTER Hospital Worker Seriously Injured in Hospital Explosion Nurse injured in explosion in critical condition (a nurse seriously injured in an oxygen tank explosion at a Medical Center has been moved to a local burn center and is listed in critical condition – a patient and a visitor who also were in the third floor room at the time were treated for smoke inhalation and released – the preliminary investigation has determined that the oxygen tank caused the explosion and a brief fire that followed, but the reason for the blast remains under investigation – when firefighters arrived on the scene, there was only one paper item smoldering, but the room had obvious damage from the blast – “the ceiling tiles buckled and came down and the walls were damaged (in the room)”)FIRE Landscaping Company’s Trailer Ignites Near Park, Injures Worker (a landscaping company’s trailer, filled with equipment and gasoline, burst into flames about 11:30 a.m. and quickly engulfed the trailer – an employee was nearby when the fire ignited, but wasn’t sure what started it – another employee was injured by the flames and was taken to the hospital)

HOTWORK Welding Torch Blamed In Dallas 3-Alarm Apartment Fire (Fire-Rescue officials say the “careless use of a welding torch” by maintenance workers is to blame for a three-alarm fire that burned around 8:30 p.m. at the apartments – no injuries were reported in the fire, but a total of 30 units were burned, leaving 40 residents displaced – fire was a result of workers doing “hot work” on “an air conditioning conditioner mount on the roof”)
