INDUSTRIAL LOTO FATALITY Worker Crushed in Shelby Township Industrial Accident (worker, 23, died when a machine he was working on malfunctioned – he was crushed when he tried to clear an obstruction from a press machine at about 6 a.m. – he was dead when police and fire personnel arrived at the plant) GUARDING/LOTO on CONVEYOR Worker hospitalized after pretzel factory accident (a worker, 22, at a pretzel factor has been injured after her hand got stuck in a machine – her hand was stuck in a dough conveyor belt – rescuers were able to free the woman’s hand – she was taken by ambulance to a hospital with minor injuries) RAILCAR FATALITY Iowa Worker Injured In Rail Yard Accident (a railroad worker has been injured after falling under a train car shortly before 4:30 a.m. – the worker was taken to a Regional Health Center and then flown to a Medical Center) MINE 2 rescued from mine accident (two men were rescued after a mine roof collapse – one possibly had a shoulder injury and the other a possible back injury – workers at the scene helped the two men to the ambulance) RADIATION Thirteen workers exposed to radiation at New Mexico nuclear waste site (thirteen workers at a Waste Isolation Pilot Plant (WIPP), a nuclear waste burial site, were notified that they had inhaled radioactive materials during a leak that occurred on February 14, around 5 a.m. – the 13 workers were above ground during the initial leaking, conducting federally mandated oversight – tests are currently being run on the workers to determine the level of exposure) FLASH FIRE Worker Burned in Flash Fire (a maintenance worker suffered burns in a flash fire at an industrial plant at about 1:00 PM – other workers were forced to evacuate the plant, but no one else got hurt – worker was servicing a piece of equipment – employee was on top of a powder coat oven on the factory’s paint line servicing it when a spark touched off a flash fire on top of the machine – he is listed in critical condition) LOTO FATALITY ELECTROCUTION Worker electrocuted at INS Kalinga dies (one of the three contract workers, 20, who had sustained severe burns at the Naval establishment succumbed late on Saturday night at a city hospital – he was electrocuted while carrying out repairs on a pole and suffered 70% burns – two others were also injured in the incident, which is said to have taken place when somebody by mistake switched on the generator while the repairs were being undertaken) HOTWORK EXPLOSION 6 suffer burns in cargo ship explosion (six crew members of a cargo ship were hurt after an explosion rocked their vessel at port – blast occurred inside the ship, which was carrying construction materials, at around 10 a.m. – six crew members sustained burns on different parts of their bodies – some crew members were welding a tank inside the vessel before the explosion happened – all manholes of the ship were open at that time) FATAL CYLINDER EXPLOSION 1 killed, 3 injured in nitrogen cylinder blast (one person has died while three others have suffered critical injuries in a nitrogen cylinder blast at an aluminum dye casting unit – the injured workers were rushed to a hospital and are presently undergoing treatment – the fatal victim was killed on the spot after suffering a massive injury to his abdomen – there were four workers inside the unit when the workers were reportedly trying to refill the nitrogen cylinder of the hydraulic dye casting machine) HOTWORK EXPLOSION Two men injured in industrial explosion (two people were injured in an industrial accident – two men were welding on a tank when residual gas inside the tank ignited and caused an explosion – first responders took both men to a hospital – they were in serious condition at the time) HOTWORK Fire at Glatfelter Paper Company causes minimal damage (a fire at a Paper Company caused minimal damage to the facility around 5:45 p.m. – the fire started when crews were doing some welding and cutting on equipment) UPDATE Refinery: Hammer aimed at scaffold broke tube in diesel-producing unit, starting Feb 24 fire (a refinery says a worker trying to disassemble a scaffold apparently started a fire that damaged a diesel production unit on Feb. 24 – the worker’s hammer missed the scaffold and broke a half-inch-wide tube on the production unit – preliminary report says the accident released hydrogen gas and diesel-range hydrocarbons – the fire was out in just under an hour) REFINERY FIRE Fire at Formosa oil refinery in Mailiao injures two workers (two employees at a naphtha cracker complex suffered minor burns after a hydrogen leak – the fire broke out at about 2:10pm after hydrogen and steam started leaking from the flanges of a flow meter in the plant’s No. 2 residue desulfurizer (RDS) unit – the fire was extinguished by about 2:30pm, but the company is still trying to determine what caused the leakage – both suffered minor burns when they attempted to fix the problem) EXPLOSION Explosion Hits a Towanda Plant (an explosion at a factory went off in the production area of the plant – everyone inside the plant made it out safely and no injuries were reported) AMMONIA Hazmat Teams Clean 4,000 Pounds of Ammonia Leak after Roof Collapse (the roof collapse happened at 2:59 A.M. and caused the ammonia to leak – two tanks of ammonia outside feed into an internal piping system – when the roof collapsed it broke the system’s valves, causing the ammonia to leak, but it was contained inside – local businesses were evacuated and some local residents were advised After Ammonia Leak Call (workers were evacuated from a plant after ammonia may have leaked into the building – firefighters responded around 3 a.m. to the Plant after a valve malfunctioned, possibly leaking some ammonia into the interior of the building – workers were evacuated to an area outside of the building, where they remained until the leak was contained – scene was cleared a little more than one hour after the initial call – no one was hospitalized, and no one complained of any injuries) AMMONIA Ammonia plume clears, residents who were evacuated return home (dozens of people had to leave their homes and businesses due to an ammonia leak around 11 a.m. when a one-ton refrigerated trailer was getting picked up – it appears during the transfer, a tank on that trailer was punctured and a plume of ammonia escaped – three people were taken to the hospital for treatment – one employee had to be decontaminated after being exposed – they were treated with an agent that neutralizes the ammonia and their injuries were not life threatening – evacuation lasted about four hours – approximately 250 people were told to leave their homes and businesses) CHLORINE Chlorine gas leak at Elkhart Wastewater Treatment Plant (emergency crews responded to a chlorine gas leak at a Wastewater Treatment Plant – HAZMAT team was called to the scene, but no injuries have been reported – employees were evacuated from the plant – police closed off a Street for a little over an hour – they believe the leak is minor – no word of where the leak may have started) CONSTRUCTION Demolition worker killed in West Texas accident, silo tank falls on victim (a demolition worker, 43, has been killed when a silo tank fell on him – he was operating an excavator and helping take down a structure when the tank fell on him and the equipment) CRANE COLLAPSE Crane collapse in Docklands leaves worker seriously injured (a construction worker is believed to be seriously injured after a crane partially collapsed at around 4.15 p.m. – a worker, 35, was rushed to hospital suffering from head injuries) CRUSHED UNLOADING BEAMS Steel beams collapse, killing 65-year-old man at South Plainfield factory (a man, 65, died from the injuries suffered when a stack of steel beams fell on him at about 2:30 p.m. – the initial report indicated that an employee was pinned between several steel beams and a tractor-trailer – he died at the factory from his injuries) TRENCHING Worker hospitalized in Cedar Hills trench collapse upgraded to fair condition (worker, 21, was trapped under dirt and mud at the bottom of a 15-foot trench that caved in – the condition of the man who had about two feet of dirt collapse onto him and a colleague while replacing a sewer line has improved – he was trapped in an 11-foot-deep trench for more than three hours and was rescued by emergency responders – two co-workers were able to remove dirt from his face and chest, which allowed him to keep breathing until emergency responders arrived) NAIL GUN Contractor Loses Eye in Nail Gun Accident (a contractor lost an eye after accidentally being shot with a nail gun – he was working on the outside of a house around 1:40 p.m. when the nail gun accidentally went off – he was hospitalized in stable condition) TRENCHING EXPLOSION 20 homes remain uninhabitable after NJ explosion (the contractor working at the site of a massive explosion that killed one person and injured seven workers recently had been fined more than $100,000 by federal safety monitors for problems at two other work sites – authorities are still working to establish the ignition point in Tuesday’s blast at a town house development but may never be able to do so – at least 20 homes remained uninhabitable, though residents were allowed back in to retrieve medicine, clothing and other belongings) FALL from SCAFFOLDING Man falls from roof of Newton house (a man, 50, was airlifted to hospital after falling from scaffolding on a house – he was suffering from head injuries) Miscellaneous BURNS Holyoke worker suffered 3rd degree burns (a power equipment company worker, 63, is being treated for third degree burns he suffered in an apparent industrial accident – he suffered third degree burns on his arms and face when his clothing caught fire – fellow employees had smothered the flames with their own coats and sweatshirts before firefighters arrived – another co-worker used a water hose to extinguish the fire – interior of the building and a garage door were “scorched”) CRUSHED by DUMP TRUCK BED Sarasota man dies after being pinned by dump truck (a commercial hauler, 47, was killed while he was trying to address a problem with his dump truck at the county’s Waste-To-Energy Facility – he apparently backed up his dump truck into a building at the facility and started to tip the bed upward when it got stuck in an open or partially open position – he got out of the cab and was trying to repair the problem when the bed came back down, pinning his head against the frame rail of the truck – when he was discovered at 11:40 a.m., his knees were on the floor, and his head caught between the horizontal bed and the frame rail) GUARDING Worker, 17, injured at Algonquin snowboard park (a worker, 17, at a snowboard park was injured when he became entangled in a tow on a beginner hill – it took a ski patrol, made up of paramedics and fire officials, at least a hour to free the employee – worker was then taken to a Hospital) TRAIN FATALITY Biopic halted after crew member death (crew was filming on train tracks when woman, 27, was hit by train – a preliminary report on the accident, which also injured six crew members – the train, with two locomotives and 37 cars, was traveling about 58 mph) FATAL EXPLOSION 1 killed in explosion in NY auto repair shop (the owner, 56, of an auto repair shop has been killed in an explosion at the shop – the explosion happened when a co-worker and the owner were working on a car’s fuel pump when the worker left to answer the phone – he then heard the owner yell “Fire!” and the garage burst into flames) HOTWORK Firefighters Battle Blaze in Grand Prairie (a fire destroyed an auto repair facility – flames spread quickly to a nearby storage and mattress warehouse, the building will have to be demolished – the fire began when employees were welding in one of the
