INDUSTRIAL MINE FATALITIES 25 workers killed in Afghanistan coal mine collapse (at least 25 workers have been killed and 15 others injured when a coal mine collapsed – there are reports that 25 of the miners have been so far found dead) CAUSTIC BURNS Two Alcoa workers burned by chemicals remain hospitalized (two of five employees burned by chemicals while cleaning a piece of equipment remain in a Burn Center – they were still receiving treatment for their injuries, which occurred about 5 p.m. in what a company spokeswoman called the “digestion” area of the plant – another female employee was transported by ambulance from the plant to a Medical Center – she was treated and released – two other plant employees suffered minor injuries and were treated at the plant – four employees and a supervisor were doing cleanup work when a caustic liquid sprayed them) LINEMAN ARC FLASH PSE&G worker burned in New Jersey; outages continue (a worker is hospitalized after he was burned while trying to restore service following heavy storms – the worker was repairing electric lines when he suffered severe burns – the man did not come in direct contact with the energized power line – but there was some sort of flash between the two, which caused a small fire – he was taken to a hospital where he is recovering) POWER PLANT EXPLOSION Worker injured in explosion at Texas power plant (one person was injured by an explosion at a coal-fired power plant that took place shortly before 11 a.m. – one employee suffered minor scratches from the blast – the explosion occurred in the system that feeds coal from a massive storage pool to the plant, not inside the plant itself) CONFINED SPACE FATALITY Worker pulled from vat of solvent at Carlstadt paint plant (a worker was overcome by fumes and fell into a large vat containing a solvent at a paint plant – an employee was found unconscious and transported to a Medical Center – the worker, who was unconscious when he was taken to a hospital, was pulled out of the vat by another employee – the second worker, who also was taken to a hospital, was conscious and alert when emergency medical workers arrived at the scene – the vat contained a chemical called toluene and was capable of holding at least 500 gallons – the accident, which occurred some time after 4 p.m., took place in a tank located behind a two-story, gray concrete manufacturing building) FATAL FALL @ CEMENT PLANT Worker dies at cement plant (a worker, 44, has died at a cement plan – there were early reports the man was crushed by a scissor lift while tightening bolts on machinery; however, the Department of Commerce said the man was understood to have died in a fall – the death is the second at the site in 11 months – in October, a man died after being buried alive in a sand silo) FORKLIFT FATALITY Worker dies in Houston forklift accident (a worker, 33, has died when a forklift he was operating overturned and fell on him – he was dead at the scene – he was operating a forklift at a business and moving a hopper containing scrap metal – he was driving across the yard when the forklift fell over the edge of a drain area, he was ejected and the equipment landed on top of him) UPDATE on FATAL FIRE @ REFINERY HPCL refinery toll rises to 27 (the toll in the refinery accident rose to 27, with one more contract worker succumbing to burn injuries – of the 27 workers who died in the accident, one was an employee of the refinery and the rest were contract workers – currently seven workers are undergoing treatment here in different private hospitals and six are at the National Burns Centre – the condition of the 6 is reported to be stable) UPDATE Venezuela discloses evidence of alleged sabotage at Amuay refinery blast (the Venezuelan Oil Ministry disclosed that nuts and bolts on a key pump in a pipeline that transported olefins to the neighboring refinery were intentionally loosened – the analysts allege that the frequent mishaps are the result of insufficient maintenance at plants run by the energy monopoly – it’s the first piece of evidence offered by officials since they declared last month that the 2012 accident, the deadliest ever experienced by the local oil industry, was the result of sabotage aimed at destabilizing the country’s leftist government – the year long investigation concluded that nuts and bolts on a key pump in a pipeline that transported olefins to the neighboring refinery “were intentionally loosened” – the dislodging of the connection line helped create a “massive gas leak” around much of the refinery’s storage areas, which was then set ablaze by the ignition of a vehicle used by National Guard members patrolling the area) FIRE Major fire damages wear down SK Hynix’s wafer plant (the fire accident is now revealed to have originated from its Chemical Vapor Deposition (CVD) machinery – other than the severely burned down equipment, the fire, heavy smoke, and power shortages are known to have also contributed to the direct contamination of the clean room and the notably damaged wafer production lines) AMPUTATION Man loses fingers in accident at plant (one person was taken by CareFlight to a Hospital after an accident at a manufacturing plant shortly before 9 a.m. – a man in his 20s lacerated four fingers on his right hand in a cutting machine – another employee put a tourniquet on the victim and was able to stop the bleeding – the four fingers have been recovered in hopes that doctors will be able to reattach them to the victim’s hand) FATAL EXPLOSION 5 killed in chemical plant blast (five people were killed in an explosion in a plant at around 10 a.m. – workers of the factory were checking material tanks when the explosion occurred – five workers were killed in the accident – the blast caused fire, but the fire is under control) EXPLOSION Explosion leads to fire at south Georgia factory (a factory is on fire after an explosion – there were no immediate reports of any injuries – plant produces pine shavings to make animal bedding) CHEMICAL REACTION = CHLORINE Chlorine gas leak leaves 25 hospitalized in Taoyuan (a chlorine gas leak from a factory left 25 people sick – they were sent to nearby hospitals, but none of them had life-threatening conditions – the leak resulted from a chemical reaction occurring inside a pipeline when the electroplating factory used it to transport diluted sulfuric acid – the pipeline had been used to carry sodium chlorate but had not been cleaned before the sulfuric acid was pumped in, triggering a chemical reaction) FORKLIFT FATALITY Forklift accident costs Saegertown teen his life ( a worker, 17, was killed when a forklift fell on him at a Forest Products logging company – he was reportedly moving lumber inside the shop around 1 p.m. when he backed the forklift off a loading dock, supposedly ejecting him from the operator’s seat – the forklift then rolled on top of him – County Coroner pronounced the teen dead at 1:30 p.m. from multiple blunt force trauma – he had not been wearing the forklift’s seatbelt, corner said) CONSTRUCTION FATAL FALL Construction worker who fell from OU building site dies (a construction worker, 46, who fell about 50 feet from a building under construction has died – he fell from a temporary scaffolding on the side of the building) PAVING MACHINE FATALITY Worker Dies In Paving Machine Accident (a construction worker, 42, at a job site has died after an accident with a piece of heavy paving machinery – he became trapped in the paving machine around 12:45 p.m. – rescue crews were working to extract the body from the machine) Construction Worker Dies After Being Pinned Under Equipment (a construction worker, 48, has died after being pinned and trapped under a piece of large equipment – it wasn’t clear whether the man was trapped under a crane or some type of other equipment – it took a number of crews to free his victim from the machine and he was declared dead at the scene shortly before 1 p.m. – the victim was experienced, with nearly 20 years of road construction) DEMOLITION FATALITY Saints ballpark work halted in wake of fatal accident (a heavy-equipment operator working for a demolition subcontractor, died after a section of concrete collapsed onto his backhoe) TRENCHING FATALITY Wendell Employee Dies after Ground Caves In (a city employee, 43, died when the ground caved in and buried him – he was using a jackhammer when the trench walls gave way and poured mounds of earth on top of him – a co-worker scrambled to free his head and shoulders, but he died at the scene) FATAL FALL Worker identified in Northville accident (a worker, 48, was killed when he fell through the roof of a building under construction – five men were on the roof installing insulation and tin on top of it when the man stepped through a spot where there wasn’t any metal to support him – he fell at least 20 feet – the man killed was subcontractor) FALL from SCAFFOLDING Worker seriously injured in midtown industrial accident (a worker fell off some scaffolding inside a building, and suffered serious head injuries – he was unconscious and without vital signs when paramedics arrived just after 11 a.m., but has been revived and now has a pulse – he remains in hospital in critical condition) EXCAVATOR and ELECTRICAL Cape Coral construction worker nearly electrocuted in construction accident (a contractor working on a road project was shaken after the earth-moving equipment he was operating came in contact with high-voltage power lines – emergency crews arrived to find an excavator that had come in close proximity to overhead power lines while working on adding a turn lane – a significant electrical arc flash impacted the metal arm of the excavator, giving the male worker a jolt but no serious injuries) CRANE 2 workers injured on the job (a pair of construction workers were injured while working on an apartment building – some plywood that was being lifted off of the second floor landing on some apartment buildings that are under construction was being raised up by a crane, using nylon strapping to lift it up, and as they were getting ready to lower it down, that strapping malfunctioned, dropping several pieces of plywood onto two construction site employees – paramedics transported the men to a Hospital in stable condition – they are expected to be OK) BUCKET TRUCK into ELECTRICAL Bucket truck causes power outage (a utility bucket truck came in contact with a live power line, starting a fire and shutting down power breakers – the operator of the bucket truck was not injured in the fire and refused treatment at the scene – workers were in bucket trucks working on overhead power lines – the boom of a truck made contact with the overhead lines causing an arc flash, which caused the hydraulic hose on the truck to become compromised and hydraulic oil ignited – worker did jump into another bucket) FATAL FALL Man dies after seven-story fall from building on Prospect St. (a construction worker, 39, died after a seven-story fall – he was part of a crew working on repairs to the seven-story building – incident occurred at about 10 a.m. – he was taken to the hospital in extremely critical condition) EXCAVATOR DEMO FATALITY Worker dead in church collapse (the owner of an excavation company has died after a church demolition when part of the building collapsed on him around 11:30 a.m. – he was working an excavator when part of the building fell and covered the machine and its operator in steel beams and bricks – workers rushed to free him, using a saw to cut a beam pinning him inside the excavator – he was rushed to the hospital, but died a short time later) WORKZONE FATALITY Worker killed in hit-and-run crash in Rockford (a worker, 54, was killed in a hit-and-run crash – he was struck by a red minivan about 11 a.m. – he was working in the area installing fiber optic cable – he was either installing or removing a temporary fence around a hole that was dug when he was struck by the minivan – he was knocked into the ditch and killed instantly) CRANE Man Hurt In Industrial Accident (a crane moving a large piece of precast concrete was blown by high winds, causing the concrete to hit a building – part of the boom arm of the crane collapsed – the crane operator was taken to the hospital with minor injuries – no one on the ground was hurt) Miscellaneous TRACTOR FATALITY South Texas cotton gin worker dies after being run over by tractor (a worker, 67, at a cotton gin has died after being run over by his tractor he fell from his tractor around 7:30 a.m. – there were no witnesses to the accident – he had worked there for about 20 years and had a history of heart problems) CONFINED SPACE FLASH FIRE 2 Workers Seriously Burned In Tank Explosion At Manchester, NH Gas Station (two workmen were badly burned after an explosion at a gas station – the men were relining an underground storage tank with fiberglass – there is a report that there was a light that had been dropped and broken inside of the tank – the two victims were flown to a hospital) TREE TRIMMER and ELECTRICAL Tree cutter burned on power line in Smithfield Township (a tree worker suffered electrical burns after he apparently touched wires as he cut trees at a residence – the worker with a local tree service was flown to a hospital with electrical burns – his partner was able to use a wooden pole to get the man away from the wires) WAREHOUSE GUNPOWDER EXPLOSION Guangzhou warehouse blast death toll rises to 8 (death toll from a warehouse explosion has risen to eight – blast happened at 11:50 a.m. in a roadside storehouse when workers were unloading goods from a container truck – a total of 36 people were injured – investigation showed the goods unloaded were bullets with powder for toy pistols – loads of bullets exploded in the truck container, and the explosion then ignited goods in a room of the storehouse) AMMONIA Railroad car leaks 20,000 gallons of ammonia in Harrington (an ammonia leak that prompted a shutdown of a portion of the town has been contained – all highways in the area are now open – around 20,000 gallons of the ammonia leaked from a railroad car – School District was placed in lockdown as a precaution, and State Route 28 was temporarily shut down) CRANE 2 inspectors rescued after crane accident leaves them dangling from Victory Bridge (firefighters saved two inspectors who were left dangling about 35 feet above the ground – the arm of the crane the inspectors were using snapped – the inspectors were in the crane’s bucket for up to 20 minutes before firefighters reached them with a ladder truck – no one was injured) |
