FATAL EXPLOSION MagneGas explosion that killed one worker not its first, company helping with investigation (cause of an explosion at a fuel technology company remains unknown – at least 20 employees fled the 5,000-square-foot building – one employee died and responders flew another to a Hospital – the rest were treated at the scene for minor injuries)

TRUCKING BACKING FATALITY U.S. Steel worker dies in trucking accident (employee died and a man is in custody after a trucking accident at the Cold Mill Shipping – worker, 41, was run over by a semi tractor-trailer around 1 p.m. – she had been an employee of the steel manufacturer for three years and worked as a shift supervisor, doing office work and shipping – “The semi didn’t see her,” XXXXXXXXX said. “Someone tried to get her attention, but she had headphones on and didn’t hear.” – the 47-year-old driver of the tractor-trailer is in custody but did not release further details of the investigation)

LOTO/GUARDING FATALITY Man, 50, killed in Elk Grove Village industrial accident (investigation is underway after a 50-year-old man was killed in an industrial accident around 12 p.m. – he was reportedly pinned in a piece of machinery)

ELECTRICAL Midwest Energy worker hurt in accident (lineman was injured while working on a transformer at approximately 11:15 a.m. – a four-man crew was working on a pad-mount transformer – one lineman received an electric shock to his arm and was taken by ambulance to a Medical Center – after being treated there, he was taken by air ambulance to another Hospital for further treatment)

CRANE FATALITY Crane fall kills worker at Yangzhou Wanlong Shipbuilding (a crane fall at a Shipbuilding site has killed a worker – the new crane has been recently put into operation, and was in the debugging process when the accident happened – worker was alive when rescued from the control room, but died during emergency treatment in hospital)

ELECTROCUTION Police: Man dies after electrocuted at central Ohio factory (worker has died after he was electrocuted at a factory shortly around 12:15 a.m. – plant makes steel wire and other wire products – the worker reached into a piece of machinery and then suffered an electric shock – he was taken to a nearby hospital, where he died)

BURNS Employee burned while working on thermal unit, Hydro says (worker was seriously injured in a workplace accident – worker was burned while working on a thermal unit – no other details)

CONFINED SPACE FATALITIES 1 worker who survived tanker blast was atop railcar wearing harness (railcar explosion killed two workers, 40 snd 44 – one worker survived the explosion at a rail yard – workers died while cleaning a tanker railcar – one of the deceased workers was exiting the car and was blown off the railcar by the explosion, and the other deceased worker was trapped inside when the ladder blew out – a third worker was atop the railcar and wearing a harness, which prevented him from suffering any serious injuries when he too was blown off)

FLASH FIRE Chemical blast burns Ohio worker (worker was transferring a chemical from a 55-gallon drum into a larger container – worker was burned on his face and neck and was flown to a hospital – doesn’t know the cause of the blast)

FATAL EXPLOSION Tanker Blast Kills Four at Belawan Port (an explosion occurred on a tanker while the ship was under repair – three victims died on the spot, while the fourth worker died in a hospital succumbing to fatal injuries – it was docked at the port for repairs when it caught fire and exploded – cause of the explosion remains unknown, as investigators were not allowed to enter the explosion site due to safety concerns)

BURNS Worker burned at Gallatin Hoeganaes plant (worker was transported to the hospital after suffering burns – worker was in stable condition – no word on cause)

COM DUST EXPLOSION Investigation continues in grain elevator explosion (four people are hurt after a massive explosion at a grain elevator – four workers remain hospitalized after the explosion – two are at a burn unit, one in critical, the other in serious condition – explosion at the facility left it heavily damaged)

HOTWORK Fire at GM Fairfax plant causes $600,000 in damage (fire caused an estimated $600,000 in damage – sparks from a welding torch ignited lubricating oil stored inside a paint shop at the plant – building’s sprinkler system doused most of the flames – $100,000 in damage to the structure and $500,000 in damage to its contents – no injuries were reported)

EXPLOSION Spectacular moment lightning strike caused a chain reaction of explosions at a chemical plant caught on camera – as huge storms cause havoc across the South and Midwest (wastewater facility went up in flames after a fracking tanker was hit by lightning, setting off a series of explosions and oil fires – nearby homes were evacuated, however no injuries were reported – fire began when lightning struck a water storage tank, launching it into the air – it landed 60 feet from the site – the water contained traces of hydrocarbons and petroleum as a result of hydraulic fracturing)

CONSTRUCTION

CRUSHING FATALITY Utah worker killed by falling modular home (worker, 39, was killed when a modular home he was helping to move fell on top of him – he was an employee of a professional moving company – prior to the 12:25 p.m. accident, one section of the home had been successfully moved with a set of jacks onto some sort of trailer for relocation – as the second section was moved, it somehow shifted and fell, crushing the worker who was beneath the building – a second worker who was also underneath the home was not injured)

FATAL FALL Worker dies after 80 foot fall in University City (worker, 42, died after an approximate 80 foot fall at a construction site around 8:10 a.m. – victim was taken to a Hospital where he was pronounced dead at 8:43)

AERIAL LIFT FATALITY Worker injured in accident at State Archives dies (state worker was killed when a piece of equipment fell on him outside the State Archives – two workers were setting up a scissor lift at about 9 a.m. when the lift toppled over and landed on the worker – they were working on exterior lighting at the archives building)

FORKLIFT FATALITY Worker killed in Petaluma construction accident (construction worker, 28, was killed after he was struck by a rolling pipe in a job-site accident – he died after being crushed beneath the 40-foot, 8,000-pound pipe at 7:16 a.m. – he was standing in front of a forklift that was moving the pipe when it rolled onto him)

ELECTROCUTION Worker dies after electrical accident at King of Prussia Mall (one of two workers, 36, injured in an electrical accident at the mall has died – cause of death has been ruled electrocution and classified as accidental – incident happened at 7:20 a.m. as crews worked on the mall’s expansion project – two workers were inside a scissor jack lift cutting electric cables beneath the second story floor of an existing section of the Mall near a newly constructed portion – worker was shocked and seriously injured when an energized cable carrying 13,000 volts of electricity was cut by one of the men – the other worker, 27, suffered non-life-threatening injuries)

FALL from BEAM Construction worker injured in fall on University of South Dakota campus (construction worker, in his 30s, was injured when he fell nearly 20 feet – he was flown to a hospital in critical but stable condition – it is believed his safety harness failed – he was standing on iron beams while installing steel girders as part of the project)

AERIAL LIFT into ELECTRICAL Worker shocked in construction accident in North Philadelphia (worker, 30, was shocked while working at a construction site just after 10:30 a.m. – worker was in a cherry picker when he came in contact with live wires and received an electrical shock – he was taken to a Hospital in critical condition)

Construction worker injured at Eugene airport (worker, 52, suffered a head injury in the Airport’s main terminal when he was struck by a tool in a construction area – he was wearing a hard hat at the time of the accident when a tool hit him in the head – his injury will likely require a couple of stitches)

FATAL FALL thru SKYLIGHT Roof worker killed after fall through warehouse skylight (worker, 59, fell to his death from the roof of a disused warehouse – he crashed through a skylight and fell around 10 metres onto a concrete floor below – he was unresponsive with severe head injuries and he was pronounced dead at the scene)

TRENCHING Worker trapped in construction pit rescued by Lacey firefighters (construction worker who was trapped in a pit was rescued by firefighters – multiple fire units responded for a trench rescue after the worker became trapped in a pit where excavators have been tearing up the parking lot near its gas station tanks – worker was alert and talking with firefighters during the rescue operation)

EXCAVATION GAS LINE EXPLOSION 14 injured in ‘major’ gas line explosion in Fresno (gas explosion at the county gun range injured multiple people – fourteen people were rushed to the hospital with burns after a major gas line explosion – blast occurred behind the range about 2:30 p.m. when a county worker leading a team of inmates constructing a dirt road accidentally struck the gas line – county worker leading the team and at least one of the inmates were critically injured in the explosion, which sent a fireball 100 feet into the sky – investigators were still working to identify what ignited the blaze)

ELECTROCUTION Worker electrocuted (construction worker, 27, died after he was electrocuted while working in a building at about 3:30 p.m. -victim was fixing the electrical circuit with the assistance of a co-worker when he accidentally touched a live wire)

HOTWORK Welding spark may have caused Target fire (fire in the roof of a furniture store may have been caused by a welding spark as contractors renovated the building – fire caught on building paper lining the inside of a 9-metre-high arched iron roof – the suspected cause of the fire was a spark from welding work being done in the roof area at the time – no reports of injury)

MISCELLANEOUS

County worker injured clearing storm debris (an equipment operator, 48, on a road crew was injured when he was struck by a tree on a job site – he was reported to be in stable condition – the road crew was clearing debris left by the February ice storm at the time of the accident)

MEAT GRINDER AMPUTATION Golden Corral worker loses 4 fingers to meat grinder (restaurant employee, 19, is missing four fingers after a meat grinder caught his left hand – he was grinding pork when a co-worker heard screaming – restaurant staff members had cleaned the area and thrown the remains of the fingers and ground pork away by the time police officers and firefighters arrived – an officer reported that the fingers on his left hand, with the exception of the thumb, had been ground off – he was taken to a Medical Center in stable condition)

Gatwick Airport worker was crushed by truck (worker was left seriously injured underneath a plane after being crushed between a toilet truck and a baggage vehicle – significant damage was also caused to a plane – a toilet-servicing truck had reversed further than intended and trapped a worker on the ground up against a baggage conveyor vehicle – it had also collided with the Boeing 787-800 aircraft – victim had been guiding the truck as the driver reversed into position – driver then accidentally pressed the accelerator instead of the brake)
