INDUSTRIAL MINE FATALITY 1 killed in accident at Black Thunder mine (a power shovel crashed into two pickup trucks at a coal mine, killing one worker and injuring another – the accident occurred at about 1:30 a.m. – worker, 24, was pronounced dead at the scene, while worker, 38, was taken to a Hospital where he was listed in good condition – preliminary investigation indicates that the power shovel was traveling up a ramp when it rolled back into the two pickup trucks – the deceased had been an employee of the mine for about three years) LINEMAN ELECTROCUTED in BUCKET TRUCK Utility worker from Calif. electrocuted in Texas (a utility worker, 24, has been electrocuted after coming in contact with power lines the victim was a contract worker – the accident happened as he was in an elevated bucket working on a power line) MINE FATALITY Two drowned in Georgia mine accident (two miners have drowned in a mine as result of an accident as water flowed up while they were drilling in an old mine – miners, 51, shift leader, and another, 49, died as result of an accident – two other workers survived the accident) ASPHALT PLANT EXPLOSION One injured in asphalt plant explosion (a worker has been injured in an explosion at an asphalt plant when the blast occurred around 6:30 a.m. – the blast happened as workers were getting ready to make blacktop – one was taken to a hospital with non-life-threatening injuries) MINE FATALITY Contractor killed at Fortescue’s Christmas Creek mine (a worker has been killed at a mining and processing operations at about 8pm – the deceased is believed to be in his 20s and according to Radio 6PR the man either fell or was crushed by machinery) Man dies inside Taylor Steel (the coroner is investigating an accident after a worker, 50, died inside the plant – “The cause of death is unknown” – “they are looking into the possibility he was electrocuted and they are also looking at the possibility he suffered a medical problem” – the man was about 10-12 metres above the floor, possibly working on a crane – the man appeared to be a subcontracted employee who was inside the plant repairing a crane) FATAL AMMONIA EXPLOSION 3 dead, 10 injured by ammonia leak in south Mexico (three people are dead and 10 others are injured following an ammonia leak – the leak occurred after construction machinery operated by a private company hit a pipeline that carries ammonia to a petrochemical plant – the pipeline rupture caused an explosion – the three dead all appear to be construction workers employed on a highway expansion project – ten people were injured and 40 others suffered from inhaling ammonia – about 1,200 people were evacuated from nearby houses) AMMONIA Olymel hog plant back operating after ammonia leak (a hog slaughter and processing plant is back up and running, after about a week offline due to an ammonia leak in a refrigerated area – nine employees were taken to nearby hospitals as a precaution – the origin of the leak involved ammonia used in the plant’s rapid-refrigeration process) FORKLIFT Industrial Forklift Accident Sends Man to Hospital (a man, 56, is recovering, after he is run over by a forklift in an accident just after 3 p.m. at a pallet company – he was hit by the forklift and run over – he was air-lifted to the hospital with minor head trauma) CONSTRUCTION MANLIFT and ELECTRICAL Worker receives electric shock at Carbonear store (a man working outside a tire store is now being treated for unspecified injuries – the man received an electric shock and was found to be suspended in the air on a cherry picker – he was initially transported to a Hospital and later sent to the Health Sciences Centre – no details have been released about the man’s condition) TRENCH Worker in hospital after being partially buried in trench (a man is in hospital after being partially buried in a workplace accident – accident occurred when an open trench collapsed around 8 p.m. – he was transported in non-life-threatening condition but it’s not clear if he has been released) SCAFFOLDING COLLAPSE Scaffolding Collapse in Bulgaria’s Capital Kills Worker (the accident occurred on the construction site of a business building – a construction worker, 25, has died and another two have been injured after a scaffolding collapsed – the two injured workers have been hospitalized and have received medical treatment – the workers were just about to pour concrete when the scaffolding collapsed, burying one of them) ROD IMPALEMENT Metal Rod Embedded in Centre Worker’s Brain at Construction Site (a worker, 19, was working at a construction site when he met with an accident that left him with a metal bar embedded in the centre of his forehead – he was clearing rubble with the help of an excavator when the accident happened – rescue operation started and fire-rescue, police and Careflight were involved – he was given morphine so that he could hold the rod till they reach hospital – no details as to how this happened) WORKZONE Reno Highway accident injures 7 (the driver of one car and a worker from the Department of Transportation were taken by paramedics to a Hospital – the DOT employee was transported by CareFlight to a Medical Center – the accident occurred at about 9:30 a.m. – a car was traveling in the far right-hand lane – DOT is currently resealing the surface of the highway – the car apparently turned left across the highway and T-boned a Nissan SUV – because of the collision, one of the vehicles then struck the DOT worker – several DOT employees were operating equipment, while others were performing highway maintenance and repair-related duties on foot) FATAL FALL into 140’ HOLE 1 killed, 1 injured in New Prague construction accident (one worker, 28, has died and another has been injured in a transmission line construction accident – it appears a strap suspending the workers broke, causing them to fall to the bottom of a 50-foot hole dug for a power pole – the hole is dug to support a power pole that’s 140 feet tall – he died from head injuries – worker, 37, was treated and released) ELECTRICAL with BUCKET TRUCK Electric Worker Injured in Franklin (worker in bucket truck contacted electrical lines – he was taken to a Medical Center before he was flown to Vanderbilt University Medical Center – accident happened a little after 10 a.m. – the cause of the accident is unknown) SCAFFOLDING COLLAPSE 8 hurt when scaffolding falls (a construction accident injured eight workers after scaffolding collapsed – about 15 workers were on the scaffolding when it collapsed about 10:20 a.m. – the injured workers fell about 34-40 feet while working on an addition to the high school – some workers were able to jump and hold on to the wall until rescuers arrived and safely lowered them to the ground – one worker was suspended in the air for more than an hour while emergency personnel safely rescued him using an aerial ladder truck from the Fire Department – the two injured workers who had what were described as serious injuries were airlifted to hospitals – six others with nonlife-threatening injuries were transported to hospitals) ELEVATOR COLLAPSE FATALITIES Five Workers Killed in Elevator Accident in E China (five workers were killed after an elevator plunged to the ground at a construction site – no other details, but several photos) WORKZONE 2 Road Workers Hurt On I-70 (two highway workers are recovering from minor injuries after their vehicle was struck on at 2:18 a.m. – the workers, ages 51 and 56, were trying to pick up debris when their vehicle was struck from behind by a pickup truck – they were taken by ambulance to a hospital with minor injuries – both have since been released – the pickup driver also suffered minor injuries – it wasn’t immediately clear if the pickup driver would face charges) TUNNEL FATALITY Highway tunnel blast claims one victim (one man, 39, is dead and five are injured in the second major Hospital – the two men had been digging a trench that was 3m to 5m deep next to a sandbank when it suddenly collapsed and buried them) WORKZONE FATALITY Construction worker dies along I-5 in southern Oregon after commercial truck hits work pickup (a female construction worker, 60, died when a commercial truck collided with a parked work pickup and trailer as she was outside along Interstate 5 – she was picking up a portable sign close to the guardrail at a paving project about 1:05 p.m. when she was struck – she died at the scene – the pickup was parked on the right shoulder with its caution lights flashing before it was hit by the truck – further details of the collision have not been released – the commercial truck continued for about a half-mile and then stopped at a fueling station off I-5 – while there, a state trooper contacted the male truck driver, who was unaware someone was injured in the crash) CHEMICAL REACTION EXPLOSION Chemical reaction may be cause of Valdosta plant explosion (investigators believe a chemical reaction may have caused that huge fire at a chemical recycling plant that injured three people – two of them remain hospitalized – firefighters estimate about 75-percent of the plant is destroyed – roughly 10 employees were in the plant when it exploded – two were airlifted to a Hospital and are in stable condition – the other injured employee was treated at a Medical Center and released) TRENCH FATALITY Construction worker killed in trench collapse in Severn (a worker, 25, was killed in a construction accident when the trench he was digging collapsed on him – it took more than three hours for crews to recover the man’s body after he was buried at about 10 a.m. – he was part of a crew digging in a 7-foot-deep hole next to a home when the dirt collapsed into the hole) MISCELLANEOUS FARMING FATALITY with COMBINE Farm worker killed in combine harvester accident (a man, 39, was killed in a combine harvester accident around 8pm – the man was pronounced dead at the scene – local residents said they understood the combine header had become stuck and the man had tried to clear it when the accident happened) CONFINED SPACE FATALITY in SEWER Minden worker dies after sewer line accident (one of the men, 55, involved in an accident in the sewer lines has died – he worked in the Utilities Department for the city – he and the Utilities Superintendent, 51, were clearing a blockage in a sewer line when they were overcome by the toxic gases in the manhole where they were working – the Volunteer Fire Department was called to the scene at 3:26 p.m. and pulled both men to safety – the deceased was transported to a hospital in critical condition – the Superintendent was transported to a Hospital where he was treated and kept overnight for observation) FORKLIFT FATALITY Man Killed in Elk Grove Forklift Rollover (accident happened just before 3 a.m. – a forklift rolled over at a livestock feed company, pinning a worker underneath and killing him) Ministry of Labour investigates accident (an industrial accident occurred at Trucking business – two workers were installing a transmission in an oil transport truck – one of the workers had his hand caught in the equipment, was treated at hospital for non-critical injuries, and has since been released) Officials say worker burned in Metals America plant fire (an employee was burned and workers were evacuated after a fire broke out at a metals plant – the fire started when an employee slipped and his foot went into a vat holding molten copper and graphite – he was working on top of a vat of molten copper, and there was a cover where he was working and it was supposed to be over the vat – he had moved the cover over and the vat was open in the area he was working – he slipped and his foot went down into the vat – the fire caught on the employee’s clothing, but did not spread anywhere else – employees were able to put the fire out before fire trucks arrived – the injury could have been worse if the employee’s leg had reached the molten copper – the employee was transported to the hospital) MEAT GRINDER Rohnert Park man’s arm crushed by meat grinder (a grocery store worker, 28, had his arm crushed in a meat grinder – he suffered a compound fracture of his right forearm in the accident – the man had surgery and there’s no word on his condition) FATAL FLASH FIRE 2 Missoula auto body workers burned when fumes explode (a worker who suffered serious burn injuries after a fire at an Auto Body died early Tuesday morning – he was pouring lacquer thinner when fumes exploded in an enclosed area in the back of the shop – two workers were caught in the fire when static electricity caused paint fumes to ignite, and explode – two other employees rushed to the area after they heard someone yell fire, and doused the flames with a fire extinguisher – one man was seriously injured and another had minor burns and injuries related to inhaling fumes from the explosion) FORKLIFT Woman in forklift truck injury at Leeds supermarket (a woman in her 60s has been taken to hospital with leg injuries after an industrial accident – she suffered non-life
