State Docks (a man was seriously hurt when he fell approximately one hundred feet into the hull of a ship shortly before midnight – the worker fell between two larges pieces of metal that were only 18 inches apart – he was in serious condition when he was transported to a Medical Center) LOTO/GUARDING FATALITY Man dies in Seneca Foods industrial accident (a worker, 42, was killed in an industrial accident – he died after getting tangled in a piece of industrial equipment just after 4 p.m.) HOTWORK Sparks from welding rod cause of warehouse fire (there is an indication that sparks coming from a welding rod caused the fire – the fire started at the ongoing construction – while the workers were using the welding machine, sparks from the welding rod may have hit the plastic wares or textiles inside the warehouse – firefighters had difficulty in stopping the fire, because of the flammable materials inside the warehouse – fire destroyed P2-million worth of plastic wares, woods, and textiles inside the warehouse – fire was put out almost 12 hours later) HOTWORK Early morning fire sparked in oil pit at Portage warehouse (welding may have sparked fire in an industrial oil pit – Fire Department responded at 5:10 a.m. to find a large fire in an oil pit – crews found smoke coming from multiple locations of the large steel processing warehouse – crews located the fire in an oil pit underneath a rolling machine in the middle of the warehouse – due to the contents of the pit, firefighters used special foam to extinguish the fire, along with 1,000 gallons of water – maintenance workers said they were doing repairs to the machine and were using a welding machine when the fire started – damage was contained to the oil pit and rolling machine directly above the pit – there were no injuries) REFINERY FIRE Fire at Japanese oil refinery injures several workers (several workers were injured when a fire broke out in the residue hydrocracking unit – fire spread from the reactor of the 31,000 barrel per day (bpd) hydrocracking unit during a cleaning at about 4:40am – the unit has been shutdown since early last month for maintenance – the fire had been brought almost under control by pumping nitrogen into the unit – six workers were treated at hospitals and one was admitted – it was not yet clear what caused the fire, how much damage had been done, nor when the unit after a first-stage pressure dump valve failed at a geothermal plant – a small amount of gas was released into the atmosphere and several loud bangs were heard when a secondary safety release valve was triggered at the geothermal electricity generation plant around 5.30am – the single staff member on overnight duty activated the emergency procedure to automatically shut down the plant and summoned the Fire Service before vacating the site safely – the site holds 30,000 litres of pentane gas – the build-up occurred in a vaporizer unit which transfers heat from geothermal liquid into the pentane which, in turn, drives the turbines – a first-stage pressure dump valve which is supposed to release pressure as it built did not operate as it should have – a secondary safety release valve the accident) CONSTRUCTION TRENCH Ladder truck used as crane to lift injured worker from 18-foot-deep Vegas trench (firefighters used a ladder truck as a crane to pull an injured worker out of an 18-foot-deep trench – the man had been in the trench when an accident happened – the man appeared to have a back injury and they didn’t want to aggravate it, so they decided to lift him out carefully – the worker was secured and lifted out to a waiting ambulance, then taken to the hospital – injuries don’t appear to be life-threatening – rescue took about 60 minutes) WORKZONE City worker injured
