Oilfield worker killed on the job (an oilfield worker has been
killed and two other workers are in hospital after being exposed to
a deadly gas while on the job – the workers were steaming a gas
production line when the machine they were using released the
poisonous gas H2S – the two injured workers were taken to a Hospital
where they are recovering – the other worker was found deceased on
scene)

Another female worker at nuke plant exposed to radiation above limit
(a second female radiation worker, in her 40{apos}s, at the crisis-hit
Fukushima Daiichi nuclear power plant has been exposed to radiation
exceeding the legal limit, after news of the first was reported
Thursday – she has no health problems so far and will be seen by a
doctor – she has been exposed to a total of 7.49 millisieverts of
radiation, against the legal limit for female workers of 5
millisieverts for a three-month period, when she was working at the
six-reactor complex following the March 11 earthquake and ensuing
tsunami which hit the plant – the woman concerned was found to have
suffered more internal than external radiation exposure, with the
internal exposure reaching 6.71 millisieverts, while she was
providing care to workers who had become sick, at a building on-site
used as an operation center to deal with the crisis – the woman may
have inhaled radioactive substances which had been attached to the
clothing of other workers in the early days of the crisis because,
for some time after the accident, workers were not wearing masks
inside the building)
AMPUTATIONS
Teenage apprentice loses fingers in guillotine accident (an
aviation firm has been fined and ordered to pay compensation to a
teenage employee who suffered partial amputations to the tips of his
right middle and left index fingers whilst working – the worker, 19
at the time, had been assigned the task of cutting stainless steel
with a large guillotine – he came into contact with the moving part
of the machine, suffering partial amputations to the tips of his
right middle and left index fingers, as well as a de-gloving injury
as he pulled his fingers away)
ELECTROCUTION
Worker electrocuted at Hartwell plant (investigators are looking
into the electrocution death of a worker, 49, who was a maintenance
electrician – he was found unconscious in a manufacturing area of
the plant – members of the company emergency response team
immediately began to perform CPR on him until paramedics arrived)

Phosphorus Gas Likely Caused North Bay Explosion (phosphorus gas
was being blamed for an explosion that forced the evacuation of a
high-tech company and critically injured one worker – the chemical
explosion set off fire alarms at about 10:30 a.m. – approximately

explosion happened while two maintenance workers were cleaning lab
equipment, a process involving some eight chemicals – the equipment

killed when he stepped on a live electrical line that had been
knocked down – he and another utility worker were checking on a
downed line – he was walking and looking up toward where the line

area – his partner could not help his stricken colleague because he
fell on the live power line – he was pronounced dead at the scene of

Three workers choke to death inside drainage line (a labour
contractor and two labourers, 22, 26, and 28, died inside a 50-feet
deep drainage line – their bodies were taken out by Fire and
Emergency Services after an hour-long operation – officials got a
call at 7 pm that some men were stuck in a drainage pipeline –
workers wree found unconscious and brought out – they had got inside
the drainage line without informing anybody – they wanted to get
back their tools of trade which they had left behind during cleaning
work last month – the trio died due to breathlessness caused by
poisonous gases present in pipelines)

Worker Burned At Plaskolite (a man, 47, was injured on the job
just after 8:00 a.m. – he was changing a mold when back pressure
blew out plastic pieces that burned his face, chest and arms – he
was taken to a Hospital for treatment and was expected to be
transferred to a burn Hospital)

(worker, 51, was working when he was knocked unconscious about 12:55
p.m. – a rescuer suffered a jolt a short time later – the worker was
working in a utility room on a pump at the time)
HOTWORK FIRE
Welding sparks ignite stubborn fire at Recycle Ann Arbor (a
small but persistent blaze caused by welding sparks just before 9
a.m. kept fire crews busy for two hours at a Recycle business – the
call for help went out when employees using extinguishers were
unable to douse the flames – the sparks ignited “probably paper
products” beneath a conveyor belt that was under repair – employees
escaped uninjured – the conveyor, used for moving recyclables in the
sorting process, was situated in a concrete “pit” inside a stainless
steel housing – with no access door opening into the housing,
firefighters had to use rotary saws on the hardened, plated steel to
make openings for the fire’s suppression)

others (an explosion at a munition disposal plant has killed one
worker and wounded three others – a shell accidentally exploded

accident at a Sewerage Plant after he got stuck between a forklift
and a wall – he was operating a pallet truck at the time of the
accident – while walking behind the equipment the lift didn{apos}t stop
and the man was pinned between the equipment and a large metal tote
containing various chemicals – it was difficult for the lift to stop
on time due to the moisture and humidity on the floor – the handle
that operates the piece of equipment was up against his chest – he
was able to free himself before emergency personnel arrived)

Sulfuric Acid Leak at Alexandria, Va., Plant (a sulfuric acid
leak has been reported at a power plant – it poses no threat to
public safety – an estimated 1,300 gallons of sulfuric acid leaked
from one tank into a secondary tank below – no one has been injured
in the incident – officials are awaiting crews to clean up the leak
– cause of the leak is under investigation)

Three city workers injured on the job (an accident involving a
cement truck left three city workers injured – recuers treated three
persons, sending one person to a Medical Center with a critical, but
stable, head injury – the ground was soft and the truck tipped –
when the truck tipped, the concrete boom struck the city workers
beneath – the street division workers were installing a new sidewalk
using a rented pump truck – the truck’s right rear outrigger lost
footing, and the truck tipped and brought the boom down on the
workers)
Superport worker was crushed to death (a man, 59, died as a
result of crush injuries – he had been working at the superport as a
subcontractor when an accident happened and he was crushed to death
– his body was found by colleagues at around 4am – no other details)

from collapse (a worker, 65, injured when part of a second level
collapsed on him during demolition of a hotel has died of his
injuries – he was injured when a half-ton slab fell on him during
demolition)

Worker partially buried in collapse of pool excavation (a man
helping install an in-ground pool was trapped briefly when a wall of
the excavation collapsed on him)

Killed after School Construction Site Collapses (six workers
were killed and five more were injured when a school hall under
construction collapsed – six workers died after being buried in the
accident that occurred at 11:50 a.m. – rescue work started right
after the collapse and the bodies of the six dead were all recovered
as of 9:05 p.m. – three workers suffered bone fractures and another
two sustained minor injuries in the accident)

3 workers die after fall from roof (three construction workers
died after falling from the rooftop of an under-construction
building – the deceased, 25, 28, and 28 fell from the rooftop of a
seven-storey building while trying to remove a lifting machine

their work, one of them lost their balance and was about to fall

Works employees were injured when a car struck them as they picked
up road debris – accident occurred about 11 a.m. while the workers
were under the Interstate 94 bridge – the workers, 41 and 35, parked
their pickup truck under the bridge in the farthest right lane
facing north – the truck{apos}s hazard lights were on – the men were
cleaning the street behind the truck when a car traveling north hit
them – it appears that they were thrown over the car – they were
taken to a Hospital – one was in serious condition and one was in
fair condition)

Worker killed at CSX Transportation rail yard in Waycross ( a
man died shortly before 6 a.m. in a collision involving a train and
a pickup truck at a rail yard – it is early in the investigation,
details of the accident were not immediately available from the
company or investigators)

Wash Car Accident (three people were hurt after an accident at a
car wash just before 12:30 pm at a car wash – a worker was driving
out of the car wash and ran into another car that was being dried –
two workers and a customer were all injured – one of the workers
suffered a serious leg injury)

Tree worker falls when truck tips in Weare (a worker from the
tree-cutting service took a 60-foot fall when the truck holding the
bucket he was in tipped over – the accident knocked out power to
thousands of homes and businesses for several hours – the ground
underneath a portion of the truck apparently gave way, causing the
pole-like device stabilizing the large vehicle to sink into the
ground and the truck to tip – the man inside the bucket had several
broken bones and was taken to a Hospital – another worker, who was
inside the bucket of a second tree-cutting truck, had to be rescued
by firefighters in mid-air when his boom got stuck)

Man killed in tragic accident at county funfair (a worker, 45,
was killed in an accident at a fairground – he died when he fell
under the wheels of a reversing articulated vehicle during the
setting up of the fair – at around 6pm, a fairground ride was being
manouevred into position on the temporary site – as the ride was
being moved, a member of the fairground staff was crushed beneath
the rear wheels of the ride)
FATAL EXPLOSION
Worker killed in explosion, fire at Aberdeen Proving Ground lab
(a contractor died in an explosion and fire at a laboratory – the
on-site fire department responded to a fire alarm and 911 call from
the laboratory facility around 3 p.m. – the fire was quickly
extinguished, but firefighters found the man dead – no one else was
injured)

2 men hurt by explosion in Ohio apartment complex boiler room, gas
leak suspected (an apparent natural gas explosion at an
apartment complex has left two men injured – authorities believe a
maintenance man was trying to re-light a furnace before blast in the
complex boiler room – investigators are trying to learn if gas was
leaking at the time – the maintenance worker was in fair condition
at a hospital – a friend accompanying him in the boiler room was
treated at the scene for minor injuries)
