INDUSTRIAL PRESSURE PIPE FAILURE Sanford worker injured in accident, lab says (a staff member with the Research Facility was injured while working underground at the facility – employee was near a pipe that burst from a build up of water pressure and was hit on the upper part of his body – the man was working with another employee who was able to get him help – he was treated at a hospital and moved to another for treatment and observation – he is now recovering at home – no one else was injured) CELL TOWER FATALITY Cell Tower Worker Decapitated, Body Left Hanging In Freak Accident (a man, 28, working on a cell tower was killed in a freak accident that decapitated him, ripping off his right arm and leaving his body suspended hundreds of feet above the ground – he was part of a four person team working on the tower, all of whom were in their 20s – two of the workers were on the ground, while two had ascended the cell tower at the time of the accident – team was attempting to lower an old boom and raise a new one on the tower when the accident occurred – early reports indicated that a cable snapped during the operation – as the 1,800 lb boom fell, the cable struck the worker, decapitating him and severing his right arm – his body was left hanging from the tower by his safety harness, 240 feet in the air) FLASH FIRE during HOTWORK Worker injured when spark ignites gasoline during replacement work at gas station (a spark caused gasoline to ignite as a worker was cutting old storage tanks at a gas station – worker was seriously injured in the accident – worker was in a pit next to the gas pumps cutting away parts of gasoline storage tanks that are being replaced when a spark ignited the fuel – his clothes caught fire – he was taken to a Medical Center – fire was out before firefighters arrived) TRAIN Worker critically injured in train accident (a railroad worker was critically injured when he was apparently struck by a passing train about 7:03 a.m. – worker was found alongside the tracks – he was flown in critical condition to a Medical Center) LINE BREAK One injured after pipe bursts in worker’s face (one person was injured after a pipe burst – CareFlite transported the worker to a hospital just before 5:30 p.m. – worker was cutting into the pipe when it burst in his face – metal flew into the employee’s face – worker suffered injuries to his face – no one else was injured in the accident) FATAL FALL from OIL RIG Worker dies in Tom Green County oilfield accident (oil worker, 34, fell from an oilfield tower and died about 2 p.m. – he fell about 58 feet and investigators believed the fall resulted from improper use of a harness) FedEx identifies employee who died after Memphis World Hub injury (employee, 19, died after suffering an injury around 2:53 a.m. – an ambulance took one patient to a Medical Center in critical condition) FATAL EXPLOSION GM Stamping Plant explosion (a chemical explosion killed a contractor, 48, and injured eight others – the explosion and subsequent chemical spill involved chlorine dioxide, which can be dangerous to the skin or if inhaled – nine employees were exposed after the blast and had to go through what’s called gross decontamination – although five of them were eventually released with minor injuries, one worker died of his injuries an hour after the blast – he died of blunt force trauma to his head due to multiple skull fractures) ALUMINUM RECYCLER FATALITY Worker Dies In Oswego County Industrial Accident (contractor, 31, working in the recycling area of an aluminum plant was killed in an industrial accident – he had worked at the plant for several years – company has not said what he was doing when the accident occurred) CHEMICAL REACTION Falls man injured in Wheatfield industrial accident (a worker, 44, suffered burns to his face after chemical explosion – he was attempting to neutralize hydrochloric acid with sodium hydroxide powder in a large vat – it appeared the man was dumping the sodium hydroxide powder in the vat with the hydrochloric acid when the chemicals reacted – the reaction caused the chemicals to explode out of the vat and sprayed onto the man’s face causing burns – it appeared the man was wearing a plastic type face shield) SANITATION TRUCK Longview sanitation worker pinned between garbage truck, passenger car after wreck (a sanitation worker was pinned in between a trash truck and the passenger car – two sanitation workers were taken to the hospital with life threatening injuries – reports indicate the passenger car hit the back of the city vehicle – driver of the vehicle was taken to the hospital with minor injuries) LOTO FATALITY Worker dies at plastics plant (worker, 34, has died in an industrial accident at a plastics plant – he was cleaning an injection molding machine when he got caught inside and was killed) FORKLIFT FATALITY Man who died in Minneapolis forklift accident is identified (worker, 36, died after a stack of wood fell from his forklift and crushed him – he was stacking bundles of a wood product when he apparently got off the forklift – that’s when the stack fell and crushed him – he died of multiple blunt-force injuries) FLASH FIRE Fire injures Lord Corp. worker (a worker at a chemical plant was injured in a flash fire at noon – the injured worker was burned from a small fire that started during the cleaning of equipment – he was treated immediately at the plant before being taken by ambulance for further treatment – fire was confined to the equipment and extinguished quickly) BOILER EXPLOSION @ REFINERY Shell worker in hospital after Ontario refinery blast (worker is in stable condition in hospital after an explosion at a refinery – a contractor also injured in the explosion was treated in hospital and released hours later – explosion occurred at about 11:30 a.m. in a boiler at the refinery’s steam plant – the two workers were doing maintenance work in the steam plant) HOTWORK Fire reported at Stillwater Mine (a fire in old timbers prompted an evacuation but no injuries – workers doing welding noticed the timbers smoldering at about 4 p.m. – after workers were evacuated, firefighters from the mine and volunteer fire departments re-entered the mine to fight the blaze using water and foam – the small fire near the surface level of the mine was fully extinguished by 2 a.m.) HOTWORK Fire out at Fostek building in Bedford, no one hurt (fire caused about $30,000 in damage – no one was hurt – investigators believe the fire was caused by a crew cutting a pipe inside the building – the torch ignited some cardboard and the fire then spread to the walls of the room) CONSTRUCTION CRUSHED FATALITY Worker crushed to death by metal plate in Guelph (worker, 22, is dead after being crushed by a metal plate at an auto parts plant at 10:20 a.m. – the man was working on a new mezzanine area when he fell and a steel plate fell on top of him – he was an employee of a third-party construction company) WORKZONE Construction worker hospitalized after hit-skip on Interstate 76 eastbound (a construction worker was hospitalized after a pickup truck struck the worker on Interstate 76 – worker was painting lines on the road when the truck struck him and kept driving – police described the truck as a blue, 2007-2009 Chevy or GMC pickup truck with ladder racks and extended mirrors on either side – the extended mirror on the passenger side was torn off by the impact with the worker – anyone with information about this accident can call Wadsworth police at (330)334-1511) WORKZONE FATALITY Construction Worker Killed On Highway 3 (a construction worker, 55, was hit by a westbound pick-up truck while he was directing traffic around a boom lift being used to help construction workers – he was pronounced dead on the scene, just after the late morning accident – the two men in the pick-up truck were not injured) FATAL FALL Construction worker dies after fall at St. Peter Claver Church in Lexington (worker, 60, died after he fell while working at a Church – he died of blunt-force trauma when he fell 30 feet – he was pronounced dead at a Hospital) Chip plant work halted (a veteran sheet metal worker, 54, died in a construction accident – he and five other workers were attempting to move an air exchanger through a fourth-floor opening of one of the buildings at the site when the accident occurred) Construction worker killed in Milton accident (construction worker was killed in an accident involving a dump truck shortly before 9 a.m. – no other details) FATAL FALL into HOLE Man Dies After Falling into 15-Foot Hole by I-95 (one man was killed after he fell 12 feet into a hole and was buried by a foot-and-a-half of dirt – he wasn’t working in the hole at the time of the accident – he was standing on an excavator and landed on the end of the trench which caused the ground to give way – the hole the man fell in was originally 15-feet wide – after he fell into the hole, other workers jumped in to try to rescue to the man to no avail – ire rescue workers arrived but only the man’s legs were visible in the hole when they got there – took fire fighters around 15 minutes to reach the victim after preliminary work was done at the scene – once the man was found he was pronounced dead) LADDER and ELECTRICAL LINES Maryville construction worker suffers electrical shock (construction worker, 19, suffered a severe electrical shock at about 10:55 a.m. – he was in stable condition – he was part of a crew trimming trees at a house construction site – the men were using an aluminum extension ladder and were attempting to move the fully extended ladder off a tree when it fell backward onto a group of power lines – the men were working on a bank located several feet higher than the road, and the workers were unable to correctly balance the fully extended ladder when they tried to move it – injured worker and two other men were holding the ladder when it fell but the two others only received minor electrical shocks) CONCRETE BOOM TRUCK and ELECTRICAL Shocked PPT worker remains in critical condition, family asking for prayers, blood donations (construction worker, 24, was severely shocked and listed in critical condition – a work crew was pouring a concrete retaining wall – he was holding on to the hose connected to a concrete pump truck directing the flow of the concrete, when the boom of the truck hit a high-voltage power line – when firefighters arrived at the scene worker had no pulse and wasn’t breathing) HOTWORK Welders cause small fire at Morton East (a small fire reported at a High School was quickly contained – a small fire was ignited on the roof of the building by contractors during the installation of an air handler unit – contractors were welding I-beams on the roof were believed to have caused some insulation to smolder, which led to a small fire the following morning – the building was empty at the time of the fire and no injuries were reported – the damage as minor and mainly contained to the insulation) HOTWORK Edmonton warehouse blaze sparked by torch (fire investigators have determined the cause of a west-end industrial fire that resulted in over $800,000 in damages – the cause of the fire has been attributed to a torch that had unknowingly ignited some material and was left to smolder when construction crews, who were tarring the roof, had packed up for the day – crews responded to the call at around 5:30 p.m. on Friday evening after the fire began – later, her condition was listed as critical) FARMER ELECTROCUTED Farmer electrocuted during repair work in Lancaster County (a farmer, 46, was electrocuted while trying to fix an elevator – he collapsed as he was working on a manure elevator at his farm – a worker found him and the farmer’s son tried to revive him with CPR, but he was pronounced dead a short time later) ACETYLENE EXPLOSION Farm workers experimenting with combustible gas caused Coryell County explosion (an explosion at a county farm that killed a man and injured two others was caused by workers experimenting with a combustible gas – incident happened shortly after 6 p.m. – three men who worked at a farm shop on the property were filling large plastic bags with acetylene gas – the men placed the bags of acetylene in a large plastic container with plans to take it into an open field and ignite the gas to see the resulting explosion – as the men filled the bags, a spark ignited the gas, causing the explosion – worker, 55, was killed during the accident – worker, 50, was taken to a Medical Center for treatment, while a worker, 24, was taken to a Hospital for injuries sustained in the explosion – both remain hospitalized in serious condition, according to the release) |
